Not Losing Weight on Mounjaro 15mg? Why It Happens (October 2026)

If you are not losing weight on Mounjaro 15mg, the usual causes are a plateau, a body that now needs fewer calories, gradual calorie creep, or a scale that hides real change. 15mg is the highest licensed dose, so the next step is a prescriber review, not a bigger dose. Judge progress by the trend over weeks, then ask your prescriber to look at diet, activity and your overall plan. Current as of October 2026.

A quick overview

What to know
before you start.

  • 15mg is the top of the licensed ladder, so a stall is a cue to review habits and treatment with your prescriber, not to adjust the dose yourself.
  • The scale is a blunt tool. Fluid shifts, a changing body shape and day-to-day variation can all hide progress.
  • Hunger can return or feel patchy on any dose. It is worth raising with your prescriber rather than ignoring.
  • Eating very little does not promise a falling number. Look at the pattern over weeks, including drinks, snacks and activity.

01 · The core answer

Why weight loss can stall on Mounjaro 15mg

Weight loss stalls on 15mg for ordinary, fixable reasons far more often than because the medicine has stopped working. Mounjaro (tirzepatide) activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 The key phrase is supports a diet. The medicine makes eating less easier. It does not replace the calorie balance.

These common explanations, and what to try for each, are a checklist for a conversation with your prescriber, not a diagnosis.

Possible reasonWhat it looks likeWhat to try
Natural plateauSteady loss for months, then a flat patchJudge by the trend across several weeks, not single weigh-ins
Lower calorie needsSame intake that once worked now holds your weightReview portions and daily activity honestly with your prescriber
Calorie creepAppetite has eased back, snacks and drinks add upKeep a short food diary, including liquid calories and bites between meals
Fluid and scale noiseWeight jumps up and down while clothes fit betterWeigh at the same time on the same day and track waist and clothes fit
Missed or mistimed dosesIrregular injection days, doses skippedFollow the missed-dose rule and keep the same weekly day
Other health factorsUnexplained gain or sudden changeTell your prescriber so they can review your wider health and medicines

If several of these fit, you have a plan to work with. If none do, that is also useful information for your prescriber, who can review your treatment as a whole.

02 · Your next step

What to do next if 15mg has stopped working

The most useful next step is to book a review with your prescriber and bring a few weeks of honest data. Useful things to bring are your weekly weight trend, your injection dates, what a typical day of eating and drinking looks like, and any new symptoms or medicines. This turns a vague worry into something they can act on.

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you (NICE TA1026). That is a conversation, not a verdict, and it covers options beyond simply going higher.

If you already take Mounjaro through us, the monthly reviews are the natural place to raise this. If you are comparing providers, you can read about the Mounjaro pen and its consultation, where the 15mg pen is priced at £329.99 per month at the standard price (prices checked 12 October 2026). New customers save £40 on their first order with code FIRST40, and every later order is £25 off the standard price. Our wider weight loss clinic page explains how support works.

03 · Plateaus and pace

Plateaus and realistic pace on the top dose

A plateau is a normal feature of gradual weight loss, not proof that treatment has failed. Weight loss on Mounjaro is gradual by design. The early weeks on 2.5mg are an adjustment phase, and most measurable progress builds from 5mg onward, month by month.1 The trials measured results at 72 weeks, so the right frame is months, not days, and the trend matters more than any individual weigh-in.

By the time you reach 15mg, you have often lost a meaningful amount already. A smaller body uses less energy, so the gap between what you eat and what you burn narrows. That is why the same routine that produced steady loss earlier can now produce a flat line. It is a sign you need to reassess, not that your effort was wasted.

It also helps to compare against realistic benchmarks. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Some people lose more and some lose less. A stall at the top dose is a reason to look closely at your own pattern, not to compare yourself with an average.

04 · 15mg is not a guarantee

What 15mg does and does not mean

15mg is the maximum licensed weekly dose, not a promise of the biggest result. The dose ladder starts at 2.5mg, steps up at a minimum of four-week intervals, and tops out at 15mg weekly depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1

In SURMOUNT-1, the higher doses did produce larger average losses, and At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Those figures still describe averages across a group, working alongside a reduced-calorie diet and increased physical activity, not instead of them. They do not tell you what will happen to one person at one dose.

The practical consequence is simple. If you are on 15mg and stuck, there is no higher licensed dose to move to. The levers left are your eating pattern, your activity, the regularity of your injections, and a review of the treatment plan with a prescriber who knows your history.

05 · Scale versus body

Weight not dropping but you look slimmer

A static scale with a changing shape usually means the number is hiding progress, so track more than one measure. The early losses on treatment include water weight, which is why the trend over weeks matters more than any single weigh-in.1 Day-to-day body weight also moves with fluid, meal timing and what you ate the evening before, none of which reflects fat change.

If your waist is not shrinking either, take that seriously but do not panic. Measure at the same point, at the same time of day, and keep a record over several weeks rather than comparing one week with the next. Clothes fit, photos taken in the same light and how you feel climbing stairs are all legitimate signals alongside the scale.

If neither the scale nor your waist has moved over a sustained stretch, bring those records to your prescriber. They are far more persuasive and more useful than a general impression.

06 · Hunger and eating little

Still hungry, or barely eating but not losing

Appetite effects differ from person to person, and feeling hungry on 15mg does not mean the injection was done wrong. Tirzepatide helps regulate appetite and slows stomach emptying, and most people feel fuller sooner and less hungry between meals.1 Most is not everyone, and the effect can feel stronger on some days and weaker on others. Hunger that returns is worth mentioning to your prescriber, who can review your regime and wider plan.

The opposite problem is just as common: eating very little yet seeing no change. Self-reported intake is notoriously hard to judge. Small amounts add up, particularly liquid calories such as juices, milky coffees and alcohol, and snack-sized bites through the day. A short, honest diary for a few weeks often reveals more than memory does.

Eating very little is not a goal in itself. If you are eating far less than feels sensible, struggling to eat regular meals, or feeling unwell, tell your prescriber rather than cutting further. Our guide on not eating but not losing weight looks at this directly.

07 · Gaining on 15mg

Gaining weight on 15mg

A small rise on the scale is usually noise, but a steady gain over weeks deserves a conversation with your prescriber. Day-to-day weight shifts with fluid, digestion and timing, and treatment effects are judged on the trend rather than on individual weigh-ins.1

Sustained gain can have several causes: calorie intake creeping back as appetite settles, missed or irregular doses, reduced activity, or changes elsewhere in your health or medicines. Only a prescriber review can sort out which applies to you. Bring your injection dates, a recent food diary and a list of anything new you have started.

If you notice a new symptom you think may be linked to the medicine, tell your prescriber and report it through the Yellow Card scheme.19 Check the patient information leaflet too, which lists the recognised side effects.13 Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

08 · Injection habits to check

Injection habits worth checking

Regular, correctly timed injections matter, so it is worth confirming your routine before assuming the medicine has stopped working. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

On timing, there is no best time of day. Mounjaro works the same morning or evening, so what matters is consistency. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.1

Storage matters too. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.13 If you are unsure about a pen that has been stored badly, check with your pharmacy before using it.

09 · What not to do

What not to do when you are stuck

Do not try to force a result by changing the dose yourself. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

Be wary of anyone offering to supply stronger, cheaper or alternative products without a consultation or prescription. Mounjaro is a prescription-only medicine, and genuine supply matters. Low-cost supplements marketed with dramatic weight-loss claims come with no clinical oversight and are not a substitute for a reviewed treatment plan.

Finally, avoid extreme restriction. Cutting food drastically to restart the scale tends to backfire and can make you feel unwell. A steady, sustainable pattern that you can keep up is the one that works over months.

10 · Reviewing your treatment

Reviewing your treatment with a prescriber

If 15mg has stalled despite careful habits, the right answer is a structured review, which can include continuing, adjusting or stopping. Some people find their prescriber suggests holding at a dose that is easier to tolerate, rather than pushing on at the maximum.

Remember that higher is not automatically better. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace. If you would like to compare how this looks at other strengths, our pages on stalling on 12.5mg and stalling on 7.5mg cover those doses.

At EverydayMeds, ongoing monthly clinical reviews are included, with each order reviewed by a GPhC-registered prescriber (GPhC 9012878). Repeat orders re-check suitability, and nothing ships automatically. Support is available Monday to Friday, and the free GLPTrackr companion app can help you log weight, doses and measurements, which makes review conversations much easier.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
Why am I not losing weight on Mounjaro 15mg?
The commonest reasons are a natural plateau, lower calorie needs in a smaller body, gradual calorie creep from drinks and snacks, and scale fluctuations that mask change. Irregular injections can also play a part. Because 15mg is the highest licensed dose, speak to your prescriber about reviewing your overall plan rather than adjusting the dose yourself.
What stops Mounjaro from working?
Mounjaro works alongside a reduced-calorie diet and increased physical activity, so changes in eating, activity or injection regularity can slow results. Missed doses and incorrect storage can also matter. If you have followed the routine and results have stopped, your prescriber can review your treatment and wider health, including any new medicines.
Why am I still hungry on Mounjaro 15mg?
Appetite effects vary between people and over time. Tirzepatide helps regulate appetite and slows stomach emptying, and most people feel fuller sooner, but not everyone feels the same on every day. Tell your prescriber if hunger has returned. They can review your routine and your regime together rather than leaving you to cope alone.
Why am I not losing weight even though I am barely eating on Mounjaro?
Intake is hard to judge from memory, and small amounts, liquid calories and snack bites add up. Weight also fluctuates with fluid. Keep a brief honest diary and track your trend over weeks. If you are eating very little and feel unwell, tell your prescriber rather than cutting back further.
Why am I gaining weight on 15mg of Mounjaro?
A small rise is often day-to-day variation, but steady gain over weeks needs a review. Possible causes include calorie creep, missed or irregular doses, lower activity or wider health changes. Bring your injection dates, a food diary and any new medicines to your prescriber so they can look at the full picture.
Why is my weight not dropping but I look slimmer?
Body shape can change while the scale stays flat, and early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Track waist measurements, how clothes fit and photos taken in the same light, as well as weight, and share the pattern with your prescriber.
Can I increase my dose above 15mg if I am not losing weight?
No. 15mg weekly is the maximum licensed dose, and altering doses outside the licence means unknown dosing accuracy and no safety data. If results have stalled, ask your prescriber to review your treatment, which may include continuing, adjusting or stopping.
When will my prescriber review my response to Mounjaro?
Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you. You can also ask for a review sooner if you are worried about progress.
