Not Losing Weight on Mounjaro 12.5mg? Causes and Next Steps (October 2026)

If you are not losing weight on Mounjaro 12.5mg, the most common explanations are a normal plateau, hidden calories that cancel out the appetite effect, or changes in body shape the scale does not show. Mounjaro works alongside a reduced-calorie diet and increased physical activity, not instead of them, and individual results vary.2 A stall at this strength is a reason to review, not to change your dose yourself.

A quick overview

What to know
before you start.

  • A flat scale on 12.5mg is often a plateau, hidden calories or body recomposition, not proof that the medicine has stopped working.
  • Trial results are averages, so some people lose more and some lose less. Your own result will not match an average exactly.
  • Talk to your prescriber about any change to your plan.
  • Track food, drinks, activity, measurements and side effects for a few weeks before your review so the conversation is specific.
  • Ongoing prescriber reviews exist for exactly this situation, and a stall is a good reason to book one.
In this guide

01 · The core answer

Why you may not be losing weight on Mounjaro 12.5mg

The usual reasons are a plateau, a calorie intake that has crept back up, changes in body composition and, less often, other health or medicine factors. Mounjaro helps you feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 It does not remove the need for that diet.

Possible reasonWhat it often looks likeWhat to do
Normal plateauSteady loss for a while, then a flat spell with no change in routineKeep logging, stay consistent and review the trend over a longer window rather than day to day
Hidden caloriesAppetite is low, but you graze, drink calories or eat small portions of very rich foodLog everything honestly for a few weeks, including oils, sauces, snacks and drinks
Body recompositionScale is flat but clothes fit differently or your waist is smallerTake measurements and photos alongside weight
Eating too little, too irregularlySkipped meals followed by large, calorie-dense eating later in the dayBuild regular, balanced meals; ask your prescriber for guidance
Other medicines or health conditionsStall that persists despite a well-kept routineAsk your prescriber to review your medicines and history
Missed or mistimed dosesGaps in the weekly scheduleFollow the leaflet instructions and speak to your prescriber if doses were missed

None of these needs a dramatic fix. They need an honest look at the pattern, which is why a short period of careful tracking is the most useful thing you can do before anything else.

02 · Your next step

What to do next if the scale has stopped moving

Start by tracking, then speak to your prescriber, and do not change your dose on your own. Write down what you eat and drink, how active you have been, your measurements and any side effects. Bring that record to your review.

Your prescriber can decide whether the plan still suits you, whether your current strength remains appropriate and whether anything else needs checking. Mounjaro is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber, and it is used alongside a reduced-calorie diet and increased physical activity.

If you are a current customer, the monthly review and follow-up assessments are the right place to raise this. If you are comparing options, see the Mounjaro pen prices and consultation page. Each strength has a standard price per pen, and 12.5mg is £319.99 per month at the standard price (checked 12 October 2026).

03 · Plateau or true stall

Plateau or true stall: how to tell

A plateau is a flat spell after earlier progress, while a true stall is a sustained absence of change despite consistent habits. The difference matters because they call for different responses. Weight rarely falls in a straight line, and short flat spells are a normal part of losing weight on any plan.

A better test is the trend: look at your weight, measurements and how clothes fit across a long enough stretch to smooth out daily swings from fluid, meals and hormones. If the trend is flat and your routine has been steady, that is the moment to ask for a review.

It also helps to remember what the trial figures mean. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

An average always hides a spread. Some people sit above it and some below it, and nobody loses weight at a perfectly even pace. Comparing yourself with a friend, a social media post or a headline figure is rarely helpful. Comparing your own trend with your own earlier months is far more informative.

04 · Hidden calories

Hidden calories and food habits

The most common practical cause of a stall is a calorie intake that has drifted up without you noticing. Mounjaro helps you feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Support is not the same as control, so what you choose still counts.

Smaller appetite can make people eat more carelessly. Because large meals are unappealing, many people switch to small, frequent, calorie-dense foods that are easy to tolerate. Biscuits, crisps, pastries, chocolate and fast food can be eaten in small amounts that still add up.

Liquid calories are another common gap. Sweetened coffees, juices, smoothies, milkshakes, sugary soft drinks and alcohol can add up quickly without making you feel full. Cooking oils, dressings, creamy sauces and spreads are similar: easy to underestimate and easy to forget when logging.

For weight loss the practical answer is to limit foods and drinks that are high in calories and low in fullness: fried and heavily processed food, sugary drinks, large portions of snacks and regular alcohol. Build meals around lean protein, vegetables, fibre-rich carbohydrates and sensible portions of healthy fats. If particular foods make you feel unwell, tell your prescriber.

A short, honest food diary is the quickest diagnostic tool. Weigh or measure for a short period, include drinks and bites between meals, and compare what you find with what you believed you were eating. Many people are surprised, and the gap is usually fixable without drastic changes.

05 · What 12.5mg means

What 12.5mg means in the dose range

12.5mg is the second-highest strength of Mounjaro, which comes as a pre-filled multi-dose pen (KwikPen) in 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Because it is so high in the range, a stall here can feel like the medicine has run out of room. That is understandable, but it is not the right conclusion.

In SURMOUNT-1 the averages were for 5mg, 10mg and 15mg, and those are trial averages with individual variation.2 Seeing 12.5mg sit between strengths does not tell you what you personally should expect.

Whether to stay on 12.5mg, move to a different strength or pause is a prescriber decision. It depends on your response, tolerability, goals and health history, and it is made at your review. Never move up or down on your own, and never treat a pen as something to stretch across extra weeks. If you want to see how others at nearby strengths approach a stall, read our guides on not losing weight on 10mg and not losing weight on 15mg.

Follow the patient information leaflet for how to use the pen.13

06 · Body recomposition

When the scale lies: body recomposition and measurement

The scale measures everything at once, so it cannot tell fat from muscle, water or the contents of your gut. If you have started strength training or become more active, your shape can change while the number barely moves. Body recomposition is a reasonable explanation, but it is only plausible if your measurements or clothes confirm it.

Use several measures rather than one. Weigh yourself at a consistent time under consistent conditions, ideally once or a few times each week rather than daily. Record your waist, hips and other areas with a tape measure, and take photos in the same light and clothing. A flat weight with shrinking measurements is encouraging. A flat weight with flat measurements, flat photos and unchanged clothing points back to intake or another cause.

Daily fluctuations are normal and can be large. Salt, hormones, bowel habits, hydration and training can all shift the scale from morning to evening. Chasing a daily number makes people frustrated and can lead to harsh restriction that is hard to sustain. A weekly trend is more honest than any single weigh-in.

Keep activity sensible and consistent. Increased physical activity is part of how Mounjaro is used, and it helps maintain muscle and general health. If you are not sure how to build it up safely, ask your prescriber or GP, especially if you have joint, heart or other health concerns.

07 · Medical factors

Medical and medicine factors worth raising

If your routine is steady and you are still stuck, ask your prescriber whether another factor is involved. Some health conditions and some medicines can make weight harder to lose.

Be open with your prescriber about every medicine you take, including prescribed treatments, over-the-counter products and supplements. Tell them about changes in sleep, stress, mood, mobility and eating patterns. Share any symptoms that are new or worrying. Your prescriber can decide whether further checks, a conversation with your GP or a change to your plan is appropriate.

Mounjaro slows how quickly the stomach empties, which is part of how it works.1 This is also why the leaflet carries specific advice on certain medicines. Insulin and sulfonylureas, for example, may need their doses adjusting because of the risk of low blood sugar, and that should be managed with your prescriber.13

People sometimes ask whether some are simply resistant to Mounjaro. Responses to treatment differ from person to person, and trial figures are averages, so some people respond more strongly than others.2 If you have followed the plan consistently and your prescriber has ruled out other causes, the next step is a conversation about whether this treatment still suits you.

08 · Habits that help

Habits that help you get unstuck

There is no trick for beating a plateau, but a few steady habits make it easier to restart progress. They are unglamorous and they work best together.

  • Eat regularly. Planned meals with protein, vegetables and fibre make it less likely that you graze or overeat late in the day.
  • Log honestly. Include drinks, bites and sauces. Our free companion app, GLPTrackr, is designed for tracking a GLP-1 journey on iOS and Android.
  • Move more, steadily. Walking, resistance exercise and anything you can sustain count more than occasional extremes.
  • Protect your sleep. Tired people tend to crave energy-dense food and find it harder to stay active.
  • Manage stress and alcohol. Both can nudge eating patterns in the wrong direction.
  • Hydrate. Thirst can feel like hunger, and water is a calorie-free default.

Honest answer: the aim is not to trick anything. Tighten the basics, give changes time to show up in the trend and ask for a review if they do not. Crash dieting or extreme restriction is more likely to leave you tired, unwell and unable to keep it up.

If you want a wider look at the same issue, our page for people who say I'm on Mounjaro and not losing weight covers the general picture, and our guide on not eating but not losing weight addresses a common paradox.

09 · What not to do

What not to do when progress stalls

Do not try to force a result by going outside the licensed way of using the medicine. A stall is frustrating, and that is exactly when risky shortcuts look tempting.

  • Do not take extra or early doses. Mounjaro is a once-weekly medicine, and the schedule is set by your prescriber.
  • In-use pens can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.
  • Do not combine unlicensed products. Low-cost supplements marketed with dramatic weight-loss claims have no clinical oversight, and their dosing is not standardised.
  • Do not buy from sellers offering to supply without a consultation or prescription. Counterfeit and unregulated products are a real risk; the MHRA publishes alerts and runs the Yellow Card scheme.7
  • Do not cut food drastically. Extreme restriction is hard to sustain and can make you unwell.

If you notice a new or worrying symptom, tell your prescriber, and consider reporting it through the Yellow Card scheme.19 Reporting helps regulators monitor safety, and it is open to patients as well as clinicians.

If you are tempted to switch to a different strength, provider or medicine because of a stall, speak to your prescriber first. A change should be a clinical decision, not a reaction to frustration.

10 · Reviews and support

Reviews, support and when to rethink the plan

A stall is exactly what ongoing reviews are for. With EverydayMeds, ongoing monthly clinical reviews are included, along with dose-titration and follow-up assessments by the prescribing team and side-effect support. Repeat orders re-check suitability, and nothing ships automatically. Your care is reviewed by a GPhC-registered prescriber, not a scoring script.

Ahead of a review, prepare a short summary: your starting weight if you know it, your recent trend, your measurements, your typical week of food and drink, your activity, any missed doses and any side effects. The more specific you are, the more useful the conversation. You can contact the team Monday to Friday, 9am to 5pm, by email at support@everydaymeds.co.uk, through the account portal chat or by phone on 0333 207 0413.

Your plan may be adjusted, kept as it is or paused. Mounjaro is a prescription-only medicine and decisions about it sit with the prescriber. If you stop treatment, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system, and nothing needs doing while it clears.1

Cost is part of the picture too. The standard price of a 12.5mg pen is £319.99, and 15mg is £329.99. New customers save £40 on their first order with code FIRST40, and an automatic £25 reorder discount applies to every future order (code DOSE25). Our weight loss clinic page explains how the service works.

Finally, keep perspective. Treatment works alongside a reduced-calorie diet and increased physical activity, and individual results vary.2 A stall at this strength is a signal to look closer, not a verdict. Earlier-strength guides such as not losing weight on 7.5mg, on 5mg and on 2.5mg may also help you compare notes.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
Why am I not losing weight on Mounjaro 12.5 mg?
The usual reasons are a plateau, hidden calories from drinks, sauces and snacking, or changes in body shape the scale does not capture. Mounjaro supports a reduced-calorie diet but does not replace it, and individual results vary. Track food, activity and measurements for a few weeks, then discuss the pattern with your prescriber rather than changing your dose yourself.
How can I overcome a Mounjaro plateau?
Tighten the basics: log everything you eat and drink, keep meals regular, stay active and use measurements as well as the scale. Give changes time to show in the trend. If nothing shifts despite a steady routine, book a review so your prescriber can check your plan, medicines and health history.
How many weeks is considered a weight loss plateau?
There is no set number of weeks, and a fixed figure could mislead you. Weight rarely falls in a straight line, so short flat spells are normal. Look at the trend across weight, measurements and clothing over a sustained period. If the trend stays flat despite consistent habits, ask your prescriber to review your treatment.
Are some people resistant to Mounjaro?
Responses vary from person to person. Trial results are averages, so some people lose more and some less than the headline figures, and treatment works alongside diet and activity. If you have followed the plan consistently, ask your prescriber whether other factors or a different approach are worth considering.
How to speed up weight loss on Mounjaro?
There is no safe shortcut. Keep to a reduced-calorie diet, cut liquid and hidden calories, build regular physical activity, sleep well and track your progress. If you want faster progress, ask your prescriber whether your plan still suits you.
What foods should I avoid while on Mounjaro?
For weight loss, limit foods and drinks that are high in calories and low in fullness: fried and heavily processed food, sugary drinks, large portions of snacks and regular alcohol. No food is banned on Mounjaro. Build meals around protein, vegetables and fibre. If specific foods make you feel unwell, tell your prescriber.
Should I change my dose if the scale will not move?
No, not on your own. Dose decisions belong to your prescriber, who considers your response, tolerability, goals and health history. A stall at 12.5mg is a reason to book a review and bring your tracking notes.
