Is there a natural alternative to Mounjaro? The honest answer

No natural product replicates what Mounjaro does. Tirzepatide activates two gut-hormone receptors at once, GIP and GLP-1, which helps regulate appetite, slows how quickly the stomach empties and improves the body's response to insulin.1 A supplement sold as "natural GLP-1" does not do the same job. These products are not medicines, are not regulated for this use and have no comparable trial evidence.1

That does not mean you have no options. It means the realistic choices are: lifestyle change on its own, other licensed medicines, or Mounjaro itself. Which is right depends on your health, your goals and what a prescriber advises.

03 · Choosing

How to decide which route suits you

Start with what you need the treatment to do. If you want a modest, gradual change and have no medical reason for medication, structured diet and activity is a sensible start. It is also part of every licensed treatment, because Mounjaro is used alongside a reduced-calorie diet and increased physical activity, not instead of them.2

If you have tried lifestyle change and want medical support, ask whether a licensed treatment fits. The question is then which one. In SURMOUNT-5, the first large head-to-head trial, adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg. Both are effective licensed treatments, and the right choice depends on your health profile and prescriber advice.15

04 · Berberine and supplements

Berberine and "natural GLP-1" supplements

Berberine is the supplement most often sold as a natural Mounjaro. It is not a medicine, it is not regulated for weight loss and it has no trial evidence comparable to tirzepatide's.1 Labels with names such as "nature's Ozempic" or "poor man's Ozempic" are marketing nicknames, not descriptions of equivalent medicines.

Supplement dosing is not standardised, so products differ in what they contain. They are available cheaply without assessment, which is part of the risk: there is no prescriber checking whether the product suits your health or other medicines.

If you take a supplement and notice a new symptom, speak to a pharmacist or your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme.19

The same applies to "natural Mounjaro recipes" and drinks. A recipe cannot activate GIP and GLP-1 receptors the way tirzepatide does.

05 · Diet and lifestyle

What diet and lifestyle can realistically do

Lifestyle change is the foundation of every weight-management plan, but it is not a like-for-like swap for medicine. In SURMOUNT-1, which enrolled 2,539 adults, the lifestyle-only group lost on average 3.1% of body weight over 72 weeks. People on tirzepatide alongside diet and exercise lost on average 15% on 5mg, 19.5% on 10mg and 20.9% on 15mg. These are trial averages, and individual results vary.2

For someone starting at 100 kg, 3.1% is about 3 kg and 20.9% is about 21 kg.

Mounjaro supports a reduced-calorie diet by helping most people feel fuller sooner and less hungry between meals.1 It does not replace the diet.

06 · Appetite and hunger questions

Natural appetite suppressants and "tricking" hunger

No natural appetite suppressant has trial evidence that matches a licensed medicine, so there is no "most powerful" natural option to recommend.

What does change appetite in a tested way is the licensed GLP-1 group. Tirzepatide helps regulate appetite and slows how quickly the stomach empties.1 Two GLP-1 tablets are now licensed in the UK: Wegovy tablets (oral semaglutide), approved by the MHRA in June 2026, and Foundayo (orforglipron), approved in August 2026. Both are prescription-only.2021

So if you are asking about "the pill that makes you not want to eat", the honest answer is a prescription tablet, not a supplement. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Our page on a natural alternative to weight loss injections covers the tablet route in more detail.

07 · Licensed alternatives

Licensed alternatives to Mounjaro

If your concern is injections, cost or tolerance, there are licensed options, each with different trade-offs. There is no tablet form of Mounjaro: tirzepatide is injection-only in the UK.1

Wegovy (semaglutide 2.4mg) is a once-weekly injection. Its pens run 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least 4 weeks to reach the 2.4mg maintenance dose from week 17.18 In STEP 1, adults on it lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.9 In that trial, gastrointestinal side effects were reported by 74.2% on semaglutide versus 47.9% on placebo, and 7.0% stopped because of side effects versus 3.1%.9 You can see the Wegovy pen, with a first pen at £99.99.

Saxenda (liraglutide) is a daily injection supplied at 6mg per millilitre, unlike once-weekly Mounjaro and Wegovy. Orlistat is a tablet that blocks some dietary fat absorption and produces smaller average losses.1

Ozempic is not a weight-loss alternative. It is semaglutide licensed for type 2 diabetes, prescribing it privately for weight loss is off-label, and a regulated UK pharmacy will not supply it for that purpose.1 For weight loss, the licensed injectable choices are Mounjaro and Wegovy. On the head-to-head evidence, SURMOUNT-5 enrolled 751 adults for 72 weeks. At least 15% of body weight was lost by 64.6% on tirzepatide versus 40.1% on semaglutide, and average loss was 22.8 kg versus 15.0 kg.15 Individual results vary, and the trial averages do not predict your own result.

08 · Warning signs and stacking

Warning signs: "over-the-counter tirzepatide" and stacking

Be wary of any product claiming to be over-the-counter tirzepatide or a copy of it. Mounjaro is a prescription-only medicine supplied through a regulated pharmacy after a consultation. Anyone offering to supply it without a consultation or prescription is outside the regulated pathway.

Mounjaro must not be combined with other GLP-1 medicines, unlicensed "research" peptides (such as retatrutide, which has no UK licence) or compounded copies. These combinations have no safety data and no regulated supply chain.1 If something promises faster results outside a licensed pathway, that is the warning sign.

09 · NHS and private route

NHS or private: how to access Mounjaro

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Ask your GP whether you are in an eligible group. If you are not, or do not want to wait, private treatment is a regulated alternative to unregulated supplements.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.
