05 · Side effects at 5mg

What to expect when you move up to 5mg

Expect the same kind of effects you may already know from 2.5mg, which are usually mild and settle. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1

A step up in dose can bring those effects back for a short time, particularly after the first injection at the new strength. That is a reason to plan, not a reason to avoid the step. Some practical ideas:

  • Choose an injection day when you can take things gently. Many people choose a weekend morning so early side effects fall on days off.1
  • Eat smaller, plainer meals if your stomach feels unsettled, since the medicine slows how quickly the stomach empties.1
  • Keep a note of how you feel each day after the injection so you can describe it accurately at your review.

If something is severe, persistent or new, speak to your prescriber or pharmacist. For severe, persistent stomach pain, swelling of the face or throat or difficulty breathing, get medical help straight away. If you take insulin or a sulfonylurea, tell your prescriber before moving up, because doses of those medicines may need adjusting to guard against low blood sugar. If you use an oral contraceptive, Mounjaro slows stomach emptying, which can reduce how well the pill is absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

06 · Higher doses and tolerance

Are higher doses more effective, and does the body build tolerance?

Trial averages rise with dose, but that does not mean everyone should chase the top strength. The SURMOUNT-1 averages climb from 5mg to 15mg, yet results vary and many people settle at 5mg, 10mg or 15mg.2 Your prescriber will guide the dose that suits you.

At 72 weeks in SURMOUNT-1, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2

If you feel your appetite control has changed, raise it with your prescriber rather than changing the dose yourself.

Response is formally reviewed too. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Treatment, in other words, is judged on a proper timescale and not week to week.

07 · Making it work better

How to make Mounjaro work better at 5mg

Take it on the same day each week, alongside a reduced-calorie diet and increased physical activity.2 The medicine helps you feel fuller sooner and less hungry between meals, which supports eating less, but it does not replace the eating pattern itself.1

Practical habits that fit that picture:

  • Build meals around protein and vegetables so smaller portions still feel satisfying.
  • Walk, and add strength work you enjoy, because activity is part of the licensed approach.
  • Weigh at a regular time and judge the trend, not the single reading.
  • Think in months, not days; trials measured results at 72 weeks.1

What not to do is just as important. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

08 · Injecting and timing

Injection day, timing and the pen

There is no best time of day: Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.1 Pick a day and time you will remember and stick to it.

The 5mg pen is used the same way as the starter pen. It is injected under the skin once a week, in the stomach area, the front of the thigh or, if someone else gives the injection, the back of the upper arm. Rotate the site with each dose.1

You set the dose by turning the knob until the correct dose shows in the window.

If you forget an injection: take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1 You can also change your injection day as long as at least 3 days (72 hours) have passed since your last dose.1

Keep pens in the fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 For travel, carry pens in hand luggage, ideally in a cool bag with your prescription label.

09 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

10 · Going beyond 5mg

After 5mg: what comes next and the NHS picture

Staying at 5mg is a valid long-term choice, and stepping up again is optional, not compulsory. On the NHS, Mounjaro is available but access is being phased in over several years.
