Staying on schedule, holding, or asking for help

The decision most people face now is whether to carry on, or ask the prescriber to hold the current dose for longer. If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. Speak to your clinical team rather than pushing through severe symptoms.1

How much weight have you lost by now?

Less than the headline trial figures, and that is expected. Weight loss on Mounjaro is gradual by design, so think in months, not days, and judge progress by trend, not individual weigh-ins.1

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

Those are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Early loss also includes water weight, which is why the trend over weeks matters more than any single weigh-in.1

04 · Side effects after a step up

Mounjaro week 6 side effects

Side effects at this stage are mostly digestive and tend to follow the most recent dose increase. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.1

The most common — affecting more than 1 in 10 people — are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.1 Common effects (up to 1 in 10) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.1

In the weight-management trials, nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.1 Most side effects appear in the first days after a dose increase, peak early, and settle as your body adapts.1

05 · Easing the symptoms

What helps with nausea, constipation and tiredness

Small practical changes help most people. Eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated, and avoid lying down straight after eating.1

For constipation, fluid intake comes first, then fibre from vegetables and whole grains, and daily movement. If it persists, a pharmacist can recommend a suitable laxative.1 Contact your clinical team for pain, bloating or more than a few days without a bowel movement.

Fatigue usually reflects the body adjusting and simply eating less. Keep meals balanced, with protein especially, stay hydrated and keep gentle activity going. For most people energy returns to normal within a few weeks. Tell your clinical team if tiredness is severe or not settling.1

Injection-site marks are also common. Mild redness, itching, a small bruise or a tender lump normally settles within a few days. Rotating sites each week and letting the pen warm briefly out of the fridge reduces stinging.1

06 · When to get help

Side effects that need urgent attention

Serious side effects are uncommon, but they need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis.1

Get medical help straight away for pain in the upper right abdomen, fever or yellowing of the skin or eyes, which could signal gallbladder problems. Signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing, need immediate medical help.1

If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team, because dehydration needs medical help straight away. Lightheadedness can be linked to eating and drinking too little, so mention it, especially if it is severe or does not settle.1

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.1 You can report any suspected side effect through the Yellow Card scheme.19

07 · Judging progress fairly

How to tell whether it is working

Early signs are mostly about appetite and routine, not the scales. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties. In practice, most people feel fuller sooner and less hungry between meals.1

So ask yourself whether portions feel easier to finish at a smaller size, and whether between-meal hunger has quietened. Those are better early signals than a daily weight.

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.1 Weight is not the only change either. In trials, weight loss came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures. These are trial-average secondary outcomes, not promises.2

08 · Pens and cost

Pen storage

Store unopened pens in the fridge at 2–8°C until first use. They keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. What should I avoid while taking Mounjaro?
While adjusting to a dose, it helps to avoid very large meals, eating past the first feeling of fullness, and lying down straight after eating. Plain lower-fat foods are easier on the stomach. Tell your prescriber about any other medicines, especially insulin or sulfonylureas, as those doses may need adjusting.
Do Mounjaro pens really only last 30 days?
Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days, then must be thrown away. Unopened pens stay in the fridge at 2–8°C and keep until the printed expiry date. Follow the patient information leaflet, and ask the pharmacy if you are unsure about a specific pen.
Can I use the same Mounjaro pen beyond its in-use window?
No. The in-use window is 30 days (not above 30°C), and beyond that sterility and potency are no longer assured. If you are unsure how your pen is meant to be used, follow the leaflet and contact your prescriber.
Is lightheadedness at this stage a reason to worry?
Dizziness is listed as a common side effect, and it can be linked to eating and drinking less than usual. Check your fluids and meals first. Tell your clinical team if it is severe or persistent.
