Both are effective, licensed, once-weekly injections for weight management in the UK, and which suits you depends on your health profile, tolerance and prescriber advice rather than the headline number alone.15

Mounjaro tends to suit people whose prescriber recommends it, and who are comfortable with a six-strength dose ladder from 2.5mg to 15mg. Wegovy tends to suit people whose prescriber prefers semaglutide, who have already tolerated it, or who find its fixed five-step ladder easier to follow.

Appetite effects are described in the product information as feeling fuller sooner and less hungry between meals.1 That is the closest description of what people mean by quieter food thoughts. It does not show that one medicine silences intrusive thoughts more than the other.

04 · What food noise means

What food noise means, and what has been measured

Food noise is an informal phrase for persistent, intrusive thoughts about food, such as planning the next meal while still eating the last one. It is not a diagnosis and not a term in the product information for either medicine.

What the product information does describe is appetite. Tirzepatide helps regulate appetite and slows how quickly the stomach empties, and in practice most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 People who say their food noise has gone quiet are usually describing that experience in their own words. It is a reasonable reading, but it is an interpretation rather than a measured outcome.

A medicine that produces more weight loss on average may well reduce appetite more, but that is an inference, not a result.

What to expect rather than hope for

Responses differ between people, and trial averages do not predict your own result. Some people notice a change in hunger early; others notice it more as the dose rises. Neither pattern means the medicine is failing or working. Your prescriber reviews your progress monthly and can adjust the dose to your response and tolerance.

If food thoughts feel distressing or out of your control rather than merely annoying, tell your prescriber or GP. Persistent preoccupation with food can have causes that a medicine alone does not address, and a conversation costs nothing.

05 · How each one works

How Mounjaro and Wegovy work on appetite

Mounjaro contains tirzepatide, which activates two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow stomach emptying and improve the way the body responds to insulin. Wegovy contains semaglutide, a single-action GLP-1 medicine.1

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide.1 It is a genuine pharmacological difference. What it means for any one person's experience of food thoughts is not something the evidence can say.

Same format, different ladders

Both medicines are once-weekly injections, and both start low and rise in steps so your body can adjust. Mounjaro comes as a multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Wegovy pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, with the dose stepping up after at least 4 weeks and reaching the 2.4mg maintenance dose from week 17.18

The 2.4mg dose is the licensed maintenance dose of Wegovy for weight management. Mounjaro has more rungs, which gives a prescriber finer control over where you settle.

How long it stays in your system

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears.

Why mechanism is not a verdict

It is tempting to read two receptors as twice the effect. Pharmacology does not work that simply, and the only direct test is a trial. That test, SURMOUNT-5, measured weight, and it favoured tirzepatide on average.

06 · What the trials show

What the trials show: SURMOUNT-5, SURMOUNT-1 and STEP 1

SURMOUNT-5 is the first large head-to-head trial, and it found higher average weight loss on tirzepatide. Adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy). Both are effective, licensed treatments.15

The detail gives a fuller picture. In SURMOUNT-5, 751 adults took tirzepatide or semaglutide 2.4mg for 72 weeks. The share losing at least 10% of body weight was 81.6% on tirzepatide versus 60.5% on semaglutide; at least 15%, 64.6% versus 40.1%; at least 20%, 48.4% versus 27.3%; and at least 25%, 31.6% versus 16.1%. Average loss was 22.8 kg versus 15.0 kg.15

A worked example

For someone starting at 100 kg, 20.2% is about 20 kg and 13.7% is about 13.7 kg. Treat that as arithmetic on an average, not a forecast. Trial averages do not predict your own result.

Each medicine against its own comparator

In SURMOUNT-1, which enrolled 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. At 72 weeks, 85% on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of body weight, and half on 10mg and 57% on 15mg lost at least 20%.2

In STEP 1, adults on semaglutide 2.4mg lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo. Of those, 86.4% lost at least 5%, 69.1% at least 10% and 50.5% at least 15%, against 31.5%, 12.0% and 4.9% on placebo.9

These two trials had different designs, so their numbers should not be set against each other. Only SURMOUNT-5 supports a direct comparison.

What none of these trials tell us

None of these trials measured how often food thoughts occur or how intrusive they feel. If you are choosing purely on food noise, the honest summary is that both medicines reduce appetite, the larger average weight loss was seen with tirzepatide, and your own response is the thing you will learn only by trying a supervised course. 07 · Side effects and tolerability

Side effects: what STEP 1 showed for Wegovy

In STEP 1, gastrointestinal side effects were reported by 74.2% of people on semaglutide 2.4mg, compared with 47.9% on placebo, and 7.0% stopped treatment because of side effects, compared with 3.1% on placebo.9 Nausea, vomiting and diarrhoea with Mounjaro are most common while the dose is being increased and become less frequent over time.

That is a useful baseline for Wegovy, and it answers a related question people ask: why do some people stop? Side effects are one documented reason. Cost, supply and personal circumstances can also play a part.

Why a dose ladder helps

Both medicines start at a low dose and step up, so your body has time to adjust. That design is the main practical tool for tolerability. If a step feels too much, tell your prescriber rather than pushing on; the dose plan is reviewed each month and can be adjusted to how you are coping.

Eat in a way that eases the stomach

Because the stomach empties more slowly on tirzepatide, large portions sit heavily, and many people feel worse after very fatty, fried or very sugary food. Smaller, slower meals work better.1 Strong coffee on an empty stomach can also aggravate nausea or reflux for some people.

Fluids matter more than you think

Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly. Keep water within reach, and treat being unable to keep fluids down as a reason to get medical help straight away, not something to push through.1

Reporting what you notice

If you notice a new symptom, discuss it with your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme, which applies to both Mounjaro and Wegovy.19

09 · Switching and what to avoid

Switching, stacking and what not to try

There is no licensed direct conversion between semaglutide and tirzepatide, so switching is always done under prescriber supervision. Typically the previous medicine is stopped, a washout interval is observed, and Mounjaro is started at a dose your prescriber judges appropriate, often the 2.5mg starting dose, sometimes adjusted based on your history.1 Never run two GLP-1 medicines together.

The reverse direction, moving from Mounjaro to Wegovy, is likewise a prescriber decision rather than a self-directed step. Your prescriber decides when to switch and which dose to start on; Wegovy's licensed dose ladder starts at 0.25mg.

Unlicensed peptides and tirzepatide vs retatrutide

Retatrutide has no UK licence. Mounjaro must not be combined with unlicensed research peptides such as retatrutide, or with compounded copies. These combinations have no safety data and no regulated supply chain. If something promises faster results outside a licensed pathway, that is the warning sign.1

Ozempic is not the same product as Wegovy

Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro and Wegovy (semaglutide 2.4mg), so a comparison of Mounjaro with Ozempic for weight loss is really Mounjaro versus Wegovy. Tablets and natural alternatives

There is no tablet form of Mounjaro. Two GLP-1 weight-loss tablets are now licensed in the UK: Wegovy tablets (oral semaglutide), approved by the MHRA in June 2026, and Foundayo (orforglipron), approved in August 2026. They are different medicines with their own dosing, eligibility checks and trial results, and both are prescription-only.2021

"Natural GLP-1" supplements such as berberine are not medicines, are not regulated for this use and have no comparable trial evidence. They are not substitutes for prescribed treatment, and dramatic weight-loss claims attached to them should be treated with suspicion.

10 · Eating well as appetite drops

How to stop food noise taking over: eating habits that help

Treatment is used alongside a reduced-calorie diet and increased physical activity, so the aim is a sustainable deficit, not a perfect food list.1

No food is banned on Mounjaro, including bananas. Prioritise protein at each meal, vegetables and fibre, and regular fluids. Many people feel worse after very fatty, fried or very sugary food, and large portions sit heavily because the stomach empties more slowly, so smaller, slower meals work better.

Protect your muscle

Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. The two levers are adequate protein, with a protein source at every meal, which is harder when appetite is low, and resistance exercise two or more times a week. This matters more, not less, as the dose rises and appetite falls.

Move in a way you can sustain

Nothing special is required: regular moderate activity such as brisk walking, plus resistance work. Start where you are and build. On injection days with early side effects, lighter movement still counts. Exercise also protects against regain if you later stop treatment.

Coffee, alcohol and supplements

There is no interaction between Mounjaro and caffeine, so coffee and tea are fine, though high-calorie coffee drinks are an easy place for the calorie deficit to leak. "Coffee rules" circulating on social media have no clinical basis. There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual, and alcohol adds calories. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar; doses of those medicines may need adjusting, so speak to your prescriber.

No supplement is required, but eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net. Discuss anything beyond that with a pharmacist.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

12 · NHS, private and ordering

NHS or private: how to get either medicine

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Side effects are one documented reason. In STEP 1, 7.0% of people on semaglutide 2.4mg stopped treatment because of side effects, compared with 3.1% on placebo. If you are thinking of stopping, speak to your prescriber first: they can review your dose, manage side effects or discuss alternatives, and any switch to another medicine should be supervised.
Are Ozempic and Wegovy the same thing?
They contain the same active ingredient, semaglutide, but they are licensed differently. Ozempic is licensed for type 2 diabetes and is not a UK weight-loss medicine; prescribing it privately for weight loss is off-label. Wegovy is semaglutide 2.4mg licensed for weight management. The licensed weight-management options are Mounjaro and Wegovy, and a regulated UK pharmacy will not supply Ozempic for weight loss. So for weight loss, the real comparison is Mounjaro versus Wegovy.

How do I stop food noise?
The practical basics are protein at every meal, vegetables and fibre, regular fluids, smaller slower meals and regular activity including resistance work, all alongside the reduced-calorie diet the licence requires. Can I take Mounjaro with retatrutide or another GLP-1?
No. Mounjaro must not be combined with other GLP-1 medicines, unlicensed research peptides such as retatrutide, which has no UK licence, or compounded copies. These combinations have no safety data and no regulated supply chain. If you want to move between Wegovy and Mounjaro, a prescriber supervises the switch and decides when to switch and which dose to start on. Never run two together.
