Can you get Mounjaro through Pharmacy First?

No. NHS Pharmacy First lets you see a pharmacist for a defined list of common conditions without a GP appointment, and in some cases the pharmacist can supply treatment. Weight management with Mounjaro is not a walk-in service under it. You cannot simply turn up, ask, and leave with a pen.

Mounjaro (tirzepatide) is a prescription-only medicine. It needs an assessment of your BMI, medical history and current medicines before anyone decides whether it suits you.¹ That assessment can happen through the NHS pathway or through a private prescriber.

The two routes that actually work

Route 1: the NHS. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Ask your GP practice whether you fall into a current group.

04 · What Pharmacy First covers

What NHS Pharmacy First covers, and why Mounjaro sits outside it

NHS Pharmacy First is a scheme that lets you get advice and, for a defined list of common conditions, treatment from a community pharmacist without booking a GP appointment. The current list is published by the NHS, so check the NHS website, as schemes are updated over time.

The scheme is built around short, self-contained episodes: a pharmacist assesses a minor illness, decides whether treatment is appropriate, and supplies it or refers you on. Long-term weight management is a different kind of care. It involves a baseline assessment, a dose that is stepped up over months, regular review and an ongoing plan for diet and activity. That is why Mounjaro for weight management is not something you can ask for at the counter under Pharmacy First.

Can you just turn up for Pharmacy First? For the conditions it covers, many pharmacies accept walk-ins, though some prefer you to phone ahead or use a referral from NHS 111 or your GP practice. But turning up and asking for Mounjaro will not result in a supply, because there is no walk-in route for it.

So how does NHS Pharmacy First work in practice? You describe your symptoms to the pharmacist in a private consultation area, they assess whether the condition is on the list and suitable for pharmacy treatment, and they either supply or signpost you. If your problem is outside the list, they will tell you to see a GP or another service.

NHS access to Mounjaro itself is a separate matter. It runs through the phased primary-care rollout described by NHS England, and NICE sets who is recommended.⁶ Your GP practice is the right first contact if you think you may qualify.

05 · The NHS route

The NHS route to Mounjaro: who qualifies and when

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. What does that mean for you? If you are in a highest-need group, speak to your GP practice about whether you can be referred or prescribed now. If you are not yet in a group, the NHS route may not be open to you at the moment, and the practice can tell you where you stand locally.

You can get an NHS prescription without seeing a GP only in limited circumstances, typically via services designed for specific conditions. Mounjaro for weight management needs a clinical decision made within the NHS pathway, so expect an assessment with a prescriber in primary care.

The NHS prescription charge in England is £9.90 per item, although many people are exempt, and prescription arrangements differ in other UK nations. That charge is separate from a private prescription, where you pay for the medicine itself.

Why do many people choose private treatment even though the NHS route exists? Mostly timing. Full rollout is expected to take years, so people who meet the licensed criteria but are not yet in a priority group often prefer regulated private treatment rather than waiting.

Neither route is better in every case. The NHS route suits those already eligible and able to wait for a slot. The private route suits those who want to start sooner and are comfortable paying. In both cases the same clinical standard applies: a prescriber decides whether Mounjaro is suitable for you.

06 · Private eligibility

Who can be prescribed Mounjaro privately

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹¹¹

This is wider than the NICE threshold used for NHS access, which is why some people qualify privately before they would qualify on the NHS. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you. Meeting a number is not the same as being prescribed.

07 · How Mounjaro works

What Mounjaro is and how it works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). It is not a reason to assume one is right for you; your prescriber considers your own history.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹

Treatment starts low and steps up, and your prescriber reviews each stage with you.

Because it is a once-weekly injection, it differs from daily liraglutide products such as Saxenda, which are supplied at 6mg per millilitre. Weekly dosing suits many people who prefer a simple routine.

The patient information leaflet that comes with the pen is the reference for how to use it and what to watch for. Read it before your first dose, and report any suspected side effect through the MHRA Yellow Card scheme.¹³¹⁹ If a symptom is new and unexplained, discuss it with your prescriber.

Mounjaro does not replace diet and activity. It is used alongside a reduced-calorie diet and increased physical activity, and that combination is what the trials tested.

08 · What trials show

What the main trial found

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages, and individual results vary.

Beyond the averages, at 72 weeks 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.²

To make the percentages concrete: for someone starting at 100 kg, 20.9% is about 21 kg, and 15% is about 15 kg. That is straight arithmetic on the trial average, not a prediction for any one person, and trial averages do not predict your own result.

Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. The comparison group in the trial also followed lifestyle changes, so the difference between 3.1% and the tirzepatide figures reflects the medicine added on top.

Which dose do you end up on? It depends on how you respond and tolerate each step, and your prescriber reviews this with you.
