The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and these usually settle.13 For most people the effects cluster in the first days after an injection and ease as your body adjusts. If you have severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis), get medical help straight away. Pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems), or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing, also need immediate medical help.

The starting dose is not meant to deliver the headline results. It exists to let your body adapt before the dose rises. Starting at 2.5mg is a deliberate design, not a sign that the medicine is weak. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals, depending on how you respond and tolerate it, so your own pace may differ from a friend's.

If you feel nothing, do not take extra or shorten the gaps between injections. Increasing the dose is a prescriber decision, not a way to speed things up.

Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

The key point for timing is the length of the trial. These figures were measured at 72 weeks, far beyond the first few weeks you are probably wondering about.

For a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. That is a trial average at the highest dose over 72 weeks, not a prediction or a target for any individual, and it says nothing about how quickly the loss arrives.

06 · Plateaus and slow progress

What if progress stalls or feels slow?

Weight loss on Mounjaro is not linear. Plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 A stall is not proof that the medicine has stopped working.

Within the licence, the responses are practical. Review food intake honestly, because portions drift back up. Protect protein and activity. Check the basics such as sleep and alcohol. Then discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.1

A plateau is not a reason to double doses or add unlicensed products.1

In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises.

If you have type 2 diabetes, Mounjaro is also licensed for that condition, and your diabetes team will track blood-sugar measures on their own schedule. Do not adjust any other diabetes medicine yourself. Insulin and sulfonylurea doses may need adjusting because of the risk of low blood sugar, so tell your prescriber what you take.1

How long it stays in your system

After a final dose, tirzepatide takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1
