Where do you inject in the thigh?

Aim for the front of the thigh, the area the licence names, and choose a patch of healthy skin. Sit comfortably with your leg relaxed, so the area is not tensed. The aim is to deliver the dose under the skin, which is what subcutaneous means.1

If you are unsure about a particular spot, ask the pharmacist or your prescriber to talk you through it, and follow the instructions in the patient leaflet that come with your pen.13

Sensible, everyday precautions apply to any injection: use clean, intact skin, and avoid areas that are sore, bruised or irritated. If a patch of skin looks or feels wrong, pick another licensed site and mention it at your next review.

How to give a Mounjaro injection in the thigh

In outline:

  • Take the pen from the fridge and check the label and the dose before you start.
  • Sit down and choose a patch on the front of the thigh, different from last time.
  • Clean the skin and let it dry, as your leaflet directs.
  • Check that the dose indicator in the window shows your correct dose.
  • Inject and hold the pen as the pen's instructions say.

06 · Rotating your sites

Rotating injection sites between doses

The licence is clear that you should rotate the site with each dose.1 You can move between the thigh and the stomach, or use a different patch of the same thigh, or alternate legs. Any of these counts as rotating as long as you do not return to the same spot each time.

A simple habit helps: keep a note in your phone, or use the free GLPTrackr companion app, to record which site you used. Then the next dose is a conscious change rather than a guess. Staying on the front of the thigh and swapping legs each week is a perfectly workable pattern if you like the routine.

The stomach has a rule of its own: stay at least 5cm from the belly button. For the thigh, keep to the front of the thigh and vary the patch.1

What if the shot goes wrong?

What happens if you accidentally inject Mounjaro into muscle?

Mounjaro is injected under the skin, so deeper delivery is not the intended route.1 Contact your pharmacy or prescriber, describe what happened and follow their advice. Do not take an extra dose to compensate.

My Mounjaro shot did not go in. What could be the problem?

Check the dose window and the pen instructions first. Follow the steps in the pen's instructions for use. If you are not sure whether you received the dose, do not simply repeat it. Contact the pharmacy or your prescriber before doing anything else.

You can report any suspected side effect or problem with a medicine through the MHRA Yellow Card scheme.19 Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

Missed or late doses

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1 You can change your weekly injection day if at least 3 days (72 hours) have passed since your last dose.1

Is Mounjaro effective in the thigh?

Mounjaro is licensed for use in the thigh, and the trial results are for the medicine as a whole, not for a specific body site. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Where you inject is not what decides the outcome. Taking the right dose on the same day each week is what matters.

Which injection site is right for you?

No site is ranked above another, only licensed ones. The thigh, the stomach area and (with help) the back of the upper arm are all named in the licence. The right place for you is the one you can use correctly and rotate.

Should I pinch my stomach when injecting?

That depends on the pen's instructions, so follow the leaflet rather than a general rule.

How to speed up weight loss on Mounjaro?

Changing injection site or dosing will not speed it up. Weight loss on Mounjaro is gradual by design. The early weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days.1

What should I avoid while on Mounjaro?

Avoid altering your dose outside the licence, taking two doses close together, or using a pen that has been frozen. Pens should be kept in a fridge at 2–8°C and never frozen. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 Keep the pen cap on to protect it from light. Your prescriber can advise on anything specific to your medicines or health.

Starting out and getting support

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1

Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary.

Mounjaro is also available on the NHS, though access is being phased in over several years.
