01 · The short version
What are the official Mounjaro storage instructions?
The official storage instructions for Mounjaro (tirzepatide) are to keep pens in a fridge at 2–8°C, never freeze them, and keep the cap on to protect against light.1 That is the whole rule for any pen you are not currently injecting from, and it applies to the pens you hold as spares between deliveries.
The rule for the pen you are using is more relaxed. You can still keep an in-use pen in the fridge if you prefer. The out-of-fridge allowance is a convenience, not a requirement.
The UK fridge range is 2–8°C. If you ever see a different range, check it against the patient information leaflet that came with your pen rather than trusting a snippet.13 Our dedicated pages on Mounjaro storage temperature and storage conditions go into the temperature side in more detail.
| Pen status | Where it lives | Key rule |
|---|---|---|
| Unopened, spare pens | Fridge at 2–8°C | Never freeze. Keep until the printed expiry date. |
| Pen currently in use | Fridge, or out of the fridge for up to 30 days (not above 30°C) | Discard after 30 days, even if medicine appears to remain. |
| Pen that has frozen | Nowhere | Never use it. Contact the pharmacy. |
| Pen in transit | Hand luggage, cool bag | Never in hold luggage, which can fall below freezing. |
02 · Your decision
Is my pen still fine to use?
Your pen is fine to use if it has been kept cold, has not frozen, is within its expiry date, is clear and colourless, and has not been in use for more than 30 days, or above 30°C. If any of those is in doubt, do not inject until you have spoken to the pharmacy.
Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days in use, or above 30°C.1 Treat that sentence as your pre-injection routine. Look at the solution, check the date, and think about where the pen has been.
If you are new to treatment and want to know how storage fits around your dosing schedule, read our guide to Mounjaro dosage instructions. If you are still deciding whether treatment is right for you, our weight loss clinic explains how it works, and you can see the current Mounjaro pen price and consultation page: the 2.5mg starter pen is £169.99 at the standard price, with all six strengths in stock.
03 · Unopened pens and the fridge
Storing unopened Mounjaro pens in the fridge
Unopened Mounjaro pens belong in the fridge until you start using them, and they keep until the expiry date printed on the box.13 The target range is 2–8°C, and the other half of the rule matters just as much: never freeze them.1
In practice, that means a few habits. Put the pens inside their original carton so the pen stays protected from light. Place the carton on a shelf inside the fridge rather than the door, because the door is where temperature swings most each time it opens. Keep pens away from the freezer compartment and away from the back wall where cooling elements can chill items too hard. These are sensible household habits aimed at the two stated risks, warmth and freezing. They are not extra rules on the label.
If you receive a delivery of several pens at once, put the spare pens in the fridge straight away and keep only the pen you are using nearby. You do not need a special medical fridge. A normal domestic fridge that holds the usual cold range is what the product information describes.
Our pages on general Mounjaro storage and storage requirements cover the same ground from a slightly different angle if you want a second read.
04 · The in-use pen window
How long can a Mounjaro pen be out of the fridge?
A Mounjaro pen that is in use can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use.13 Once the pen reaches 30 days of use, it should be discarded.
Equally, people ask how to make a pen last longer. If your supply timing feels awkward, tell your prescribing team at your monthly review rather than improvising.
Some people prefer to keep the in-use pen in the fridge anyway, taking it out shortly before injecting. That is allowed. The out-of-fridge allowance simply gives you flexibility for a short trip, a busy week or a forgotten fridge moment. Follow the pen's own instructions for preparing and giving the injection, and keep the cap on between doses.
05 · Freezing and warm storage
What happens if you don't refrigerate Mounjaro or it freezes?
A frozen Mounjaro pen must not be used. That is the clearest rule on the label: never use a pen that has been frozen.1 A pen that has frozen may look normal once it thaws, which is exactly why the rule is absolute. You cannot judge it by eye.
Freezing happens more often than people expect. The usual causes are a pen resting against the back wall of the fridge, a pen left in a car in cold weather, a pen packed with a hard ice pack in direct contact, or a pen carried in hold luggage on a flight. If you suspect any of these, do not inject. Contact the pharmacy, explain what happened, and ask what to do.
What if a pen was simply left out? The answer depends on whether it is in use or unopened. An in-use pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, so a day on the kitchen counter is not a problem. An unopened pen is different. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.1 We would add a practical point: write down roughly when and for how long the pen was out, so the pharmacy can advise you properly.
Heat is the other side. A pen must not be used if it has been above 30°C. Leaving any pen in a hot car, in direct sun or beside a radiator is outside what the leaflet describes. If you notice a pen has been somewhere hot, treat it like any other doubtful pen and ask before you inject.
Our page on storage temperature for Mounjaro is a good companion if temperature is your main worry.
06 · Travelling with pens
How do you travel with Mounjaro?
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing.1 Ideally, pack them in a cool bag with the prescription label or a copy of your prescription.
That covers a typical holiday for a pen you are already using. Spare, unopened pens should stay in the fridge until needed and keep until the printed expiry date.
A short checklist helps:
- Keep pens, needles and your prescription label together in hand luggage.
- Use a cool bag or insulated pouch rather than a hard ice pack pressed against the pen, because freezing is the bigger risk.
- Check destination rules for travelling with prescription injectables before you fly.
- Keep the cap on so the pen is protected from light.
- Do not leave the bag in a parked car or in direct sun.
If you are staying somewhere with a fridge, such as a hotel room with a minibar, use it for spare pens, but check that it is not so cold that it freezes contents. If you are unsure, an insulated pouch kept in a cool, shaded spot is the safer option.
Planning treatment around a trip? Tell your prescribing team ahead of time. They can talk through timing and supply so you are not caught short, and our storage conditions guide has further practical tips.
07 · Delivery and cold chain
What should arrive and how should it look?
A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless.1 Those are the things to check on the day your parcel lands.
Our orders go out in temperature-controlled, discreet packaging with free next-day tracked delivery, and we dispatch the same day when you order by 3pm. Pens are genuine KwikPens supplied through official LillyDirect supply. Needles, a sharps bin and injection guidance come with every order. When the parcel arrives, open it promptly, check the contents against your prescription label, and put spare pens in the fridge. If you will not be home, arrange for someone to take the parcel in, rather than leaving it on a doorstep in heat or frost.
If the packaging looks tampered with, the pen is missing a batch number or expiry date, or anything about the supply seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.12 Our registration number is GPhC 9012878, and you can check it directly on the register.
Be especially wary of anyone offering to supply tirzepatide without a consultation or prescription, or in a form other than the licensed pre-filled pen. Counterfeit medicine alerts and the Yellow Card scheme are covered by the MHRA.7 For the idea that pens need preparing first, see Mounjaro mixing instructions. The licensed UK product is a pre-filled multi-dose pen, so there is nothing for you to make up at home.1
08 · Spotting a pen that has gone bad
How can you tell if Mounjaro has gone bad?
You can tell a Mounjaro pen is unfit by looking at the solution and checking its history: it should be clear and colourless, and it should never have frozen or passed its limits.1
Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use.1 Run through that sentence before every injection. It takes a few seconds and removes most of the risk.
Two cautions. First, a normal-looking pen is not proof of a safe pen. A pen that has frozen can look fine, so history matters as much as appearance. Second, do not try to rescue a doubtful pen by warming it, shaking it or topping up doses. If you are unsure, the pharmacy is the right call, not a search engine or a social media post.
If a pen has failed or been damaged in a way that was not your doing, contact us promptly. Our policy on dispensed medicines allows faulty, damaged or wrong items to be reported within a short window, so it is worth getting in touch quickly rather than sitting on the problem.
If you ever experience a new symptom after an injection from a pen you were unsure about, speak to your prescriber and report it through the Yellow Card scheme.19
09 · Handling, light and disposal
How should you handle and dispose of pens?
Handle each pen as the patient information leaflet describes: keep the cap on to protect it from light, and follow the pen's instructions for each injection.13
Between doses, put the pen back in its carton or a clean, cool place with the cap on. Do not store it loose in a bag with keys or sharp objects. Do not share a pen with anyone, and keep it out of the reach of children.
When a pen reaches the end of its in-use window or runs out, put it in the sharps bin supplied with your order. Do not put used pens or needles in household waste. When the sharps bin is full, follow local arrangements for disposal, which your pharmacy or council can explain.
It also helps to keep a simple note of the date you started each pen. A phone reminder or the free GLPTrackr companion app can hold that date for you, which makes it easy to know when the in-use window ends. This is a practical suggestion rather than a label requirement.
10 · Stopping treatment and leftover pens
What if you pause or stop treatment?
If you pause or stop treatment, any unopened pens should stay in the fridge until the printed expiry date, and you should speak to your prescriber before restarting. The medicine does not disappear straight away either: tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system, and nothing needs doing while it clears.1
Do not restart from stored pens without advice. Where you restart in your dose schedule is a clinical decision, and your prescriber can advise. Our aftercare includes ongoing monthly reviews, dose titration and follow-up assessments, and repeat orders re-check suitability, with no auto-shipping, so you will never receive pens you did not ask for.
Remember also why storage discipline matters: treatment works alongside a reduced-calorie diet and increased physical activity, and individual results vary.2 A pen that is unfit for use is not worth risking, and replacing it is better than injecting from one you doubt.
