01 · The decision
NHS or private: which route suits you?
The NHS route suits you if you meet the current NHS criteria and are happy to wait for your area and GP practice to reach your group. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years. The private route suits you if you meet the licensed criteria but not yet the NHS ones, or you do not want to wait. Mounjaro is licensed for adults with a BMI of 30 or above, or 27 to 30 with at least one weight-related condition.1 That is a wider group than the current NHS priority groups. Many people who meet the licensed criteria choose regulated private treatment rather than waiting.

What private Mounjaro costs
Private treatment is paid per pen, and one pen lasts four weeks. Your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99). Every later order is £25 off the standard price (code DOSE25). You can see the full dose range on the Mounjaro pen page. All six doses are in stock.
If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg. These are standard prices before discounts. A new customer's first order is £40 less, and every later order £25 less.

04 · Reviews versus evidence
What Mounjaro reviews can and cannot tell you
Reviews tell you how individual people felt about a treatment; trials tell you what happens on average across thousands of people. Both have limits. A star rating cannot tell you your starting weight, your diet, your dose, or whether the reviewer was taking the medicine as prescribed.
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve how the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That dual action distinguishes it from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).
When you read any review, ask a few questions. What dose was the person on, and for how long? Were they also changing their eating and activity? Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Did they have a prescriber reviewing progress? And is the reviewer describing an effect that is listed in the patient leaflet, or something unrelated?
If you notice a new symptom, discuss it with your prescriber and report it through the Yellow Card scheme.19 For the first days on treatment, read what to expect in week 1.
05 · Trial results
What the SURMOUNT-1 trial found
In SURMOUNT-1, a trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks. People using lifestyle changes alone lost 3.1%.2 These are trial averages, and individual results vary.
The same trial reports how many people reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
To make percentages concrete, here is a worked example. For someone starting at 100 kg, 15% is about 15 kg, and 20.9% is about 21 kg. Your starting weight, dose and circumstances will differ, and a trial average does not predict your own result. Some people lose less than the average, and some lose more.
What happens after you stop is another question reviews rarely answer well. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Continuing treatment to maintain weight reduction was studied in SURMOUNT-4, which is worth discussing with your prescriber if you are thinking about the long term.14
06 · NHS availability
Is Mounjaro available on the NHS in the UK?
Yes, Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially people with a BMI of 40 or above and four or more qualifying conditions.46 Full rollout is expected to take years.
What does that mean for you in practice? It means your eligibility depends on both your own health and where your local service has reached in the phased rollout. Someone with a BMI of 36 and high blood pressure meets the NICE recommendation on paper, but may not be in the first group offered treatment. Your GP practice is the best place to ask where you sit in the queue.
The NHS describes tirzepatide as a medicine to manage type 2 diabetes and treat obesity, and it is used alongside diet and activity changes.5 NICE has separate guidance for type 2 diabetes, where tirzepatide has been used for longer.8
A sensible way to approach your GP:
- Know your BMI and any weight-related conditions, such as high blood pressure, high cholesterol, sleep apnoea or prediabetes.
- Ask whether your practice is offering tirzepatide for weight management and which group you fall into.
- Ask what support (diet, activity, follow-up) comes with it.
- Ask what to do if you are not yet eligible.

If you do not meet the NHS criteria yet, that does not mean you are unsuitable for treatment. The licensed criteria are broader, and a prescriber can assess you privately. Many people who meet them choose regulated private treatment rather than waiting.
07 · Eligibility and BMI
What BMI qualifies, and what counts as obese?
For the licence, the threshold is a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples of those conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.111 Treatment must be used alongside a reduced-calorie diet and increased physical activity.
People often ask what weight is considered obese on the NHS. It is a BMI question rather than a single weight, because the same weight means different things at different heights. Under the licence, a BMI of 30 or above is the starting point for treatment on its own. The NHS criteria for Mounjaro are tighter: NICE recommends it for a BMI of at least 35 plus at least one weight-related condition, and the first NHS groups are higher still.4
Ethnic background matters too. People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk of weight-related health problems at a lower BMI, so clinical thresholds are usually reduced by 2.5 kg/m² for these groups.1 If that applies to you, mention it in your consultation or GP appointment.
A worked example helps. Suppose your BMI is 28 and you have high blood pressure. You fall in the 27 to 30 range with a weight-related condition, so you may meet the licensed criteria, though not the current NHS ones. Suppose your BMI is 31 with no other conditions. You meet the licensed threshold on BMI alone. In both cases, a prescriber still assesses your BMI, medical history and current medicines before deciding whether treatment is suitable.
BMI is a screening measure, not a verdict. A prescriber looks at the wider picture, including other medicines, previous conditions and what you have already tried.
08 · Safety and side effects
Safety, side effects and who should not take it
Mounjaro is not suitable for everyone, and an honest review of it has to say so. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.1 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.
On thyroid safety: in rodent studies tirzepatide caused thyroid C-cell tumours. It is not known whether this applies to humans, and no increase has been shown in human trials. As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. Routine thyroid monitoring is not required for most people.1
Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13 Your prescriber can talk you through what to expect as the dose goes up, and our team offers side-effect support during monthly reviews.
If you experience something new, speak to your prescriber. You can also report any suspected side effect through the MHRA Yellow Card scheme.19 Tell your prescriber about any new or lasting symptom.
If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar; your prescriber will advise. And Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed, so ask about contraception during your consultation.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

09 · Year cost planning
Planning the cost of a year on private treatment
If you are comparing NHS access with private treatment, it helps to see the full-year picture rather than a single pen price. Mounjaro is dosed weekly, and one pen covers four weeks, so a year is 13 pens. You begin at 2.5mg and step up, so early pens cost less than later ones.

Not everyone needs the top dose. The dose you settle on is decided with your prescriber, based on how you respond and tolerate treatment. In SURMOUNT-1, average results were reported at 5mg, 10mg and 15mg, so there is no single right dose for everyone.2
For context on pricing, Eli Lilly announced a UK list-price increase for Mounjaro on 14 August 2025, effective 1 September 2025. Pharmacy sell prices vary because of confidential discounts. Across the UK pharmacies we track, the cheapest 2.5mg starting pen is £143.96 and the dearest is £179.99 (checked 11 October 2026).

10 · Ozempic and alternatives
How Mounjaro compares with Ozempic and Wegovy
Ozempic and Wegovy both contain semaglutide, a single-action GLP-1 medicine, whereas Mounjaro's tirzepatide activates both GIP and GLP-1 receptors.1 That is the main mechanistic difference.
Can Ozempic be used for weight loss? Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg). A regulated UK pharmacy will not supply Ozempic for weight loss.
Wegovy's dose ladder is 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly. It steps up after at least 4 weeks and reaches the 2.4mg maintenance dose from week 17.18 Check availability at the time of order. Mounjaro's pens, by contrast, are in six strengths from 2.5mg to 15mg.
Is there a head-to-head comparison? Yes: SURMOUNT-5 was the first large head-to-head trial of tirzepatide and semaglutide for weight loss.15 Ask your prescriber which option fits your health history, tolerance and budget.
What about the cheapest alternative to Ozempic? Low-cost supplements are available cheaply without assessment, but they come with no clinical oversight and are not an equivalent of a licensed prescription medicine. Liraglutide (Saxenda and similar products) is a daily injection supplied at 6mg per millilitre, unlike once-weekly Mounjaro and Wegovy.

11 · Using and storing pens
Using and storing your Mounjaro pen
Mounjaro is a once-weekly injection from a pre-filled KwikPen, and each pen covers four weekly doses. Needles, a sharps bin and injection guidance are included with every order, so you do not need to source anything separately. Follow the pen's instructions for the injection itself, and ask our team if anything is unclear.
Storage matters, particularly if you read reviews that mention a pen that was left out. Unopened pens should be stored in the fridge until first use, and they keep until the printed expiry date. For any question about an unopened pen that has been out of the fridge, check the patient information leaflet and ask the pharmacy rather than guessing.13 Once in use, UK KwikPens are licensed for up to 30 days (not above 30°C) out of the fridge.1 Beyond that, sterility and potency are no longer assured, so the pen should not be used.
