NHS or a private Mounjaro bundle: which route fits you?

If you meet the NHS criteria and can wait for local access, the NHS route is worth pursuing; if you meet the licensed criteria but do not qualify yet, or do not want to wait, a private route is the alternative. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions. Full rollout is expected to take years.⁶ NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition.⁴

The word bundle does not describe an NHS item. On the NHS, your prescriber decides what you are supplied and when you are reviewed. 02 · Cost

What do the first four pens cost privately?

If you step up after at least 4 weeks on each of 2.5mg, 5mg, 7.5mg and 10mg, your first four pens cost £879.96 at standard prices. Your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99). After that, each later order is £25 off the standard price, and the amount you pay depends on your dose: £189.99 for 5mg, £244.99 for 7.5mg and £274.99 for 10mg at standard prices.

A first year costs £2,399.88 if you stay on 5mg after your 2.5mg starting pen (13 pens), up to £3,279.88 if you step up through every dose to 15mg. 03 · How ordering works

How ordering a private bundle works

Ordering takes three steps; payment is taken when you place the order, and if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. First, complete the free online consultation, a short health questionnaire with no charge and no obligation, via our free consultation. Second, a GPhC-registered prescriber reviews it; unsuitable orders are refunded automatically. Third, our registered pharmacy dispenses and dispatches, with same-day dispatch when you order by 3pm and free next-day tracked delivery in temperature-controlled packaging. 04 · Is Mounjaro on the NHS?

Is Mounjaro available on the NHS?

Yes, Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.⁴ The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially people with a BMI of 40 or above and four or more qualifying conditions.⁶

That phasing is the reason many people look at bundles and private options. The licence allows a wider group than the first NHS stage, so some people meet the licensed criteria without yet being in the group an NHS service is prioritising. Full rollout is expected to take years.

What this means in practice depends on where you live. Local NHS bodies manage how the rollout is delivered, so the pace can differ. Your GP surgery is the right first contact for current local arrangements, and our guide to Mounjaro through your GP explains what to ask.

Be wary of any claim of a fixed date, a fixed list of rollout stages, or a bundle you can obtain on the NHS. Neither a bundle nor a multi-month pack is an NHS prescribing concept. If you are prescribed Mounjaro by the NHS in England, the standard NHS prescription charge of £9.90 per item applies unless you are exempt from charges, and your NHS prescriber decides how and when you are supplied.

If you are not in a prioritised group, there are three sensible choices: ask your GP to record your interest and review your eligibility, continue with lifestyle support while you wait, or consider regulated private treatment. None of these is a wrong choice, and you can ask your GP about the NHS route while also exploring private options. Treatment is only appropriate after a clinician has reviewed your medical history, and it is used alongside a reduced-calorie diet and increased physical activity.¹

05 · NHS eligibility criteria

Who is eligible for Mounjaro on the NHS?

NICE recommends Mounjaro on the NHS for adults with a BMI of at least 35 plus at least one weight-related condition.⁴ In the first stage of the primary-care rollout, which began in June 2025, access was focused on people with a BMI of 40 or above and four or more qualifying conditions.⁶

In short, NHS access depends on a BMI threshold, at least one weight-related condition, and a rollout that starts with the highest-need groups and widens over time. Your GP or local NHS service will tell you which stage applies to you.

The NHS thresholds are higher than the licence. The medicine is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.¹ That gap between the NHS criteria and the licence is why private prescribing exists alongside the NHS.

Ethnic background matters too. People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk of weight-related health problems at a lower BMI, so clinical thresholds are usually reduced by 2.5 kg/m² for these groups.⁴ Mention your background to whoever assesses you, whether that is your GP or a private prescriber.

How do you get Mounjaro on the NHS? There is no application route that bypasses clinical assessment. Book an appointment with your GP, bring your height, weight and details of any weight-related conditions, and ask whether you meet the current local criteria. If you do, your NHS prescriber will tell you how supply works in your area. If you are in England the standard NHS prescription charge of £9.90 applies unless you are exempt, so free is not assured for everyone; exemptions depend on your circumstances.

Before an appointment, it helps to note any conditions that may count as weight-related: high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. A clinician decides which conditions qualify, but having your own list makes the conversation faster.

06 · NHS vs private compared

NHS vs private Mounjaro: how the routes differ

The main differences are who sets eligibility, how fast you can start, and what you pay. Both routes involve a clinician reviewing your suitability, and both use the same licensed medicine. Neither route replaces a reduced-calorie diet and increased physical activity.¹

On the NHS, NICE sets the recommendation (BMI of at least 35 plus a weight-related condition) and the rollout decides who is reached first.⁴ That has been phased in from June 2025 starting with the highest-need groups, and full rollout is expected to take years.⁶ The benefit is that the cost to you is limited to the standard prescription charge of £9.90 in England unless exempt, and care sits within the NHS.

Privately, the licensed criteria apply, which are wider. A GPhC-registered prescriber reviews your consultation, and if approved you choose the pens in your order. You pay the pen prices, which we show live on the Mounjaro pen page.

Which suits you? If you meet NHS criteria and your area is supplying, the NHS route is a reasonable first call. If you meet the licence but are not yet in a prioritised group, private treatment is the alternative. 07 · Private eligibility and safety

Private eligibility and who should not take Mounjaro

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.¹ It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.

Eligibility is only the first test; safety is the second. Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.¹³ Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.

On thyroid safety: in rodent studies tirzepatide caused thyroid C-cell tumours. It is not known whether this applies to humans, and no increase has been shown in human trials. As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. Routine thyroid monitoring is not required for most people.¹

The consultation is where you share your full medical history and medicine list. Be complete: omissions are the most avoidable safety risk. If you take other medicines, mention them all. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

Why this matters for bundles: ordering several pens at once does not change the assessment. Each repeat order re-checks suitability, and the prescribing team can adjust your dose or pause treatment as needed. A multi-pen order should never be treated as permission to keep going if you develop symptoms that worry you; contact your prescriber, and report suspected side effects through the Yellow Card scheme.¹⁹ Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

The same applies to NHS prescribing: the NICE recommendation is for specific groups, and a clinician makes the decision.⁴ Mounjaro (tirzepatide) is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. 08 · How Mounjaro works

How Mounjaro works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). The first large head-to-head trial of tirzepatide and semaglutide is SURMOUNT-5.¹⁵

Why does that matter for bundles? Because the pens are stepped up gradually. You start on 2.5mg and your prescriber moves you up through 5mg, 7.5mg and 10mg, and on to 12.5mg and 15mg if that is appropriate.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹

09 · What results to expect

What results to expect, and how fast

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.²

To make a percentage tangible: for someone starting at 100 kg, 20.9% is about 21 kg, and 15% is about 15 kg. That is straight arithmetic on the trial averages, and it describes a 72-week average, not a prediction for you.

People often ask how much they will lose in three months, or whether very rapid loss is normal. If you lose weight faster than you expected, or feel unwell, tell your prescriber rather than comparing yourself with others.

This is also why a bundle should be sized to your plan and not to a target. Your prescriber reviews progress during monthly check-ins, decides on dose changes, and can pause or stop treatment.

Beware of pages or sellers that promise specific outcomes, or dramatic weight-loss claims within a short window. The trial reports averages across a group, with lifestyle support included, and does not promise a particular outcome for any one person. A practical view: use the trial figures to understand the size of effect that was seen over 72 weeks, then agree your own goals with your prescriber, including non-weight goals such as blood pressure or activity levels.

10 · Staying on treatment and ordering safely

How long can you stay on Mounjaro, and how to buy safely

There is no fixed number of bundles you must buy, and the right length of treatment is a decision for you and your prescriber. Our aftercare includes ongoing monthly reviews, dose-titration and follow-up assessments, with side-effect support.

When buying, the safest route is a regulated pharmacy. Check that any pharmacy appears on the GPhC register; ours is GPhC-registered (9012878), and you can verify it on the GPhC register.¹² Mounjaro is prescription-only. Be cautious of any seller offering to supply without a consultation or prescription, or at prices that look implausibly low.

The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected side effects or suspect products.⁷ We supply through official LillyDirect supply, sourced directly from Eli Lilly's UK distribution, with genuine KwikPens and all six doses stocked (2.5mg to 15mg).

Storage matters if you hold several pens. Store unopened pens in the fridge until first use; they keep until the printed expiry date. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. If you are unsure about an unopened pen that has been left out, check the patient information leaflet and contact the pharmacy rather than guessing.¹³ Buying more pens than you can store properly is a good reason not to over-order.
