What is the recommended dose of Mounjaro?
The recommended dose of Mounjaro is a gradual climb, not a single fixed number. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1
After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1

Which dose question are you actually facing?
- You have not started yet. Your dose is already decided: 2.5mg. The only real decisions are whether treatment suits you and who will review you along the way.
- You are partway up the ladder. The question is whether to step up. That depends on tolerability and response, and it is a conversation with your prescriber rather than a calendar rule.
- You are on a maintenance dose. The question is whether it is still right. A higher dose is not automatically better, so staying put can be the correct clinical choice.
A step-up is permitted from four weeks, not required at four weeks. If side effects are still noticeable, holding the current dose longer is within the licence, and your prescriber can advise on it.1

04 · Why you start low
Why everyone starts on 2.5mg
The starting dose exists for tolerability. Tirzepatide acts on the gut-hormone system, and your body needs time to adjust to that, so a lower dose is used first and then increased gradually.1 This is why 2.5mg is not meant to be judged as a weight-loss dose.
The first weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal.1 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 That framing helps if you are tempted to rush up the ladder because the early weeks feel quiet.
Mechanically, tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1
05 · Stepping up safely
How and when the dose increases
The dose increases in defined steps, with a minimum gap between each. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
Notice the wording: a minimum interval, not a schedule. Many people do not move at the earliest possible point. A step-up is more comfortable when the previous dose feels settled, with appetite effects steady and side effects mild or gone.
Each pen is a different strength, so stepping up means moving to a new pen rather than dialling a higher number on the old one. Each KwikPen contains four weekly doses.1 A full pen therefore covers about a month of treatment at a given strength.

06 · Maintenance doses
Maintenance: 5mg, 10mg or 15mg
There is no single maintenance dose for everyone. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 The right dose is the one that gives you a useful response while remaining comfortable to live with.
The SURMOUNT-1 trial shows how results differed by dose, but it also shows why averages do not decide your own dose. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
Read these as a picture of what the medicine can do across a group, not a prediction for you. Trial averages do not predict your own result, and a lower dose that you tolerate well and can keep taking may suit you better than a higher one that disrupts daily life.
The 12.5mg and 7.5mg strengths are steps within the ladder. Some people settle on these doses, and that is a legitimate outcome rather than a failure to reach the top.
07 · Review and response
When to review your dose with a prescriber
Response is reviewed, not assumed. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1
Note that this refers to the highest tolerated dose. It does not mean everyone must reach 15mg. If you cannot tolerate more than a certain dose, that becomes your highest tolerated dose, and the review is based on it.
Judge progress by trend, not individual weigh-ins. Trials measured results at 72 weeks — think in months, not days.1 A slow week on the scales, or a week where you hold a dose, is not a reason to change anything by yourself.
Things worth raising at a review include persistent nausea, a feeling of being too full to eat enough, tiredness that does not settle, and any new symptom you are unsure about. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If they do not settle, tell your prescriber; the licence allows a dose to be held longer or stepped down. Any suspected side effect can also be reported through the MHRA Yellow Card scheme.19

08 · Injecting each dose
How to take each weekly dose
Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.13
There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember (many people choose a weekend morning so early side effects fall on days off) and stick to it.1

09 · Missed doses and day changes
Missed doses and changing your injection day
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.13
Skipping is the correct response after the window, not a problem to fix by doubling up later. The dose you take next stays at the strength you were on.
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.13 This is useful when work, travel or a new routine makes your usual day awkward.
10 · Storage and travel
Storing pens at each dose
Storage rules are the same whichever strength you are on. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13
You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch.13
Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.13
11 · Off-label dosing
Why not to alter the dose yourself
Changing a dose outside the licence is unsafe, however attractive it sounds online.
Altering doses outside the licence means unknown dosing accuracy and no safety data.1
If the recommended step feels too much, the answer is to say so. A prescriber can adjust within the licence; a home-made schedule cannot be monitored.
Wegovy, for reference, is titrated from 0.25mg through 0.5mg, 1mg and 1.7mg to a 2.4mg maintenance dose, stepping up after at least four weeks and reaching 2.4mg from week 17.18 Its ladder is a different product with different strengths, so numbers should never be swapped between the two.
12 · NHS and private routes
Does the dose differ on the NHS or privately?
The licensed dose ladder is the same whichever route you use; what differs is access. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.4

13 · Stopping and clearance
What happens when you stop
If you stop, tirzepatide does not vanish immediately. After a final dose it takes roughly four to five weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
Because of this, side effects from a final dose can linger a little, and appetite changes do not reverse overnight. If you plan to stop or pause, discuss it with your prescriber first, and in particular ask what dose to restart at if you later resume, since that is not something to guess. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

14 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
