Protein matters because some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance.4 In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

Losing that much weight is a good outcome. The point of eating enough protein is to make sure what you lose is mostly fat. Protein also tends to be filling, which helps if you are managing hunger between meals, although Mounjaro already does much of that work.

This matters more, not less, as the dose rises and appetite falls.4 A person on a high dose who barely eats may find that protein falls well below what they need without any obvious warning sign. How to get enough protein when you are not hungry

The simplest method is to make protein the first thing you eat at each meal. The medicine slows how quickly your stomach empties, so large portions sit heavily, and smaller, slower meals work better.4 If you are full after a few mouthfuls, you want those mouthfuls to be protein.

Aim to include a protein source at every meal, which is harder when appetite is low.4 These habits help many people:

  • Eat the protein part of the meal first, then vegetables, then starchy food if you still have room.
  • Choose protein foods that are easy to eat in small amounts: eggs, fish, poultry, yoghurt, cottage cheese, tofu, beans and lentils.
  • Eat little and often if large meals are uncomfortable, rather than forcing a full plate.
  • Keep a ready-to-eat protein option available for days when cooking feels like too much.
  • Avoid very fatty, fried or very sugary options when you feel nauseous, since many people feel worse after them.

Protein works best with resistance exercise

Protein alone is only half of the muscle-protection plan. Eating protein without giving your muscles a reason to keep themselves is less effective.

Increased physical activity is part of the licence, not an optional extra. Nothing special is required. The evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle. Start where you are and build up. On injection days with early side effects, lighter movement still counts. Exercise also protects against regain if you later stop treatment.4

Resistance work does not need a gym. Bodyweight exercises, resistance bands or light weights at home all count. A personal trainer or physiotherapist can help if you have joint problems or have not exercised for a long time.

06 · Hydration and kidneys

Drink enough, especially when eating more protein

Drinking enough matters more on Mounjaro than usual, because reduced appetite means less fluid from food. Vomiting or diarrhoea can also dehydrate you quickly, which is the main route to the rare kidney problems reported with this class of medicine.13 This matters if you are raising your protein, since you do not want a change in diet to coincide with poor fluid intake.

Keep water within reach through the day. If you cannot keep fluids down, contact your clinical team, as dehydration needs medical help straight away.13 Coffee and tea are fine, as there is no interaction between Mounjaro and caffeine. Strong coffee on an empty, slower-emptying stomach can aggravate nausea or reflux for some people, and high-calorie coffee drinks are an easy place for the calorie deficit to leak.13

Alcohol is a related point. There is no direct interaction, but drinking on a reduced appetite and an emptier stomach can hit harder than usual, and alcohol adds calories that work against your goals.13

07 · Protein shakes and supplements

Protein shakes, vitamins and supplements

Protein shakes are a convenience option, not a requirement. They can help on days when solid food is unappealing, because a drink is often easier to manage than a plate. No supplement is required with Mounjaro. Eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net while losing weight. Discuss anything beyond that with a pharmacist.13 Supplements sold as natural GLP-1 boosters are not equivalents and are not substitutes for prescribed treatment. Oral supplements, like other oral products, may be absorbed more slowly when you start treatment.13

Signs of too much or too little protein

There is no reliable checklist of symptoms that prove you are eating too much or too little protein. Tiredness, feeling weaker or losing strength can have many causes, including eating too little overall, poor hydration or a side effect of treatment. Do not self-diagnose from a list online.

Protein is not a free food. It contains calories, and treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.4 If you add protein shakes, bars or large portions on top of your usual eating, total calories can rise and weight can stall.

Other common explanations are fluid shifts, strength training adding muscle while fat falls, or simply a normal fluctuation from day to day. Judge progress over weeks, not days. If your weight rises steadily over a long period despite taking your medicine as prescribed, speak to your prescriber, who can review your dose and diet.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
