Are problems with Mounjaro normal?
Some problems are normal, particularly digestive ones, and they tend to show up when you start or move up a dose. The medicine works partly by slowing how quickly the stomach empties and helping you feel fuller sooner.2 Feeling full, queasy or a little off your food is the same mechanism doing its job, sometimes too strongly.
The patient information leaflet lists every known side effect and how often each occurs.13 Read it before your first injection and keep it with the pen.
Not every problem is a side effect. Some are about technique, storage, diet, expectations or supply. Sorting yours into the right group is the fastest route to fixing it.

What should I do if I am having problems?

03 · Digestive problems
Why Mounjaro causes digestive problems
Digestive symptoms are the best known problems because of how the medicine works. Tirzepatide activates the GIP and GLP-1 receptors, which help regulate appetite and slow how quickly the stomach empties.2 When food sits in the stomach longer, nausea, bloating, reflux-type discomfort and changes in bowel habit are understandable consequences.
The SmPC and leaflet list the digestive effects people may notice.113 They tend to be most noticeable early on and around dose increases, because your body is adjusting to a stronger effect.
Practical points that prescribers commonly discuss include eating smaller portions, eating slowly, avoiding very rich or greasy meals if they upset you, staying hydrated and not lying down straight after eating. These are general habits, not a replacement for individual advice. If symptoms stop you eating or drinking normally, tell your prescriber rather than pushing on.
Dehydration deserves particular attention. Persistent vomiting or diarrhoea can leave you short of fluid, which can make you feel dizzy and tired and can strain the kidneys. If you cannot keep fluids down, get medical help straight away.
04 · Warning signs
Warning signs that need urgent help
Some symptoms should never be treated as routine side effects. Serious reactions are uncommon, but they need urgent attention.13
Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis), or for pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems). Get medical help straight away for signs of a serious allergic reaction such as swelling of the face or throat or difficulty breathing, or if you are unable to keep any fluid down.
Whatever you experience, you can report a suspected side effect yourself through the MHRA Yellow Card scheme.19 Reports from patients and professionals help the regulator spot patterns.

05 · Sinus and other symptoms
Tirzepatide and sinus problems, and other unlisted symptoms
Sinus problems are not a listed side effect in the UK patient leaflet.13 Blocked noses, congestion and sinus pressure are common for many reasons, including colds, allergies and changes in weather, and they can coincide with starting treatment by chance.
The same applies to any symptom that is not on the leaflet list. Not being listed does not mean you should ignore it. Raise new symptoms with your prescriber, who can check whether something else is going on and whether any change is needed.

06 · Interactions
Medicine interactions that cause problems
Because Mounjaro slows stomach emptying, it can reduce the absorption of oral contraceptives.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1
If you take insulin or a sulfonylurea for diabetes, doses may need adjusting because of the risk of low blood sugar.1 This does not mean the medicines cannot be used together. It means your prescriber or diabetes team should plan the change with you.
Always tell the prescriber about everything you take, including over-the-counter products and supplements. Our consultation asks about this, and repeat orders re-check suitability rather than shipping automatically.
07 · Pen and storage problems
Pen, storage and injection problems
Many practical problems come from handling rather than the medicine. Store unopened pens in the fridge at 2–8°C until first use, and they keep until the printed expiry date.13 Once you start a pen, it can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or frozen.1 For anything else about unopened pens left out of the fridge, check the patient information leaflet and contact the pharmacy.
If a pen looks damaged, leaks, will not dial or does not seem to deliver, do not improvise. Follow the pen's own instructions and contact us. A faulty, damaged or wrong item should be reported promptly; our returns policy covers those cases.

08 · Results and expectations
When the problem is slow results
Disappointment with results is one of the commonest complaints, and it is not a side effect. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result, so a slower start is not necessarily a sign that something is wrong.
If progress stalls, bring it to your monthly review. The conversation usually covers eating pattern, protein and fluid, activity, sleep, other medicines and whether your dose is right for you. Please do not raise your dose or change the injection schedule on your own.
09 · Safety comparisons
Which is safer: Mounjaro, Wegovy or Ozempic?
No honest answer ranks one licensed medicine as simply safer than another. Mounjaro and Wegovy are both licensed UK treatments, each with its own SmPC listing side effects and warnings.118 Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply Ozempic for weight loss. Digestive effects are a feature of this whole medicine class, which is why they feature on all of these labels.

10 · Long-term use
Long-term effects and stopping treatment
People understandably ask about long-term side effects. The key trial behind weight-management results ran for 72 weeks.2 Longer-term experience continues to build, which is why ongoing monitoring matters.
If you stop, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.2 Talk to your prescriber before stopping so you can plan eating and follow-up. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.
People stop treatment for many reasons: side effects, cost, reaching a goal, pregnancy planning or feeling it is not for them. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. All are legitimate, and none requires you to push through problems you are unhappy with.
11 · Supply and safe sourcing
Supply problems and counterfeit risk
Where you get the pen matters. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber.12 Be wary of anyone offering to supply without a consultation or prescription, or selling unbranded vials. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme.7

On NHS access, Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first.4 Many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
