Why the price rises with each dose

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ The strength you are prescribed is the single biggest driver of what you pay each month.

A higher-strength pen costs more, so anyone asking what Mounjaro is going to cost needs to ask which dose they will end up on, not only what the starting pen costs.

The 2.5mg pen is a starting dose. It is not the dose most people stay on, and the starting price should not be read as a long-term monthly cost. Your prescriber decides when, and whether, to move you up, based on how you are responding and tolerating treatment. Some people settle at a lower strength, and others move up through the range. Each step means a new pen at that strength's price.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹ This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide.

06 · First-year budgeting

Budgeting for the first year

A realistic first-year budget is £2,399.88 if you settle at 5mg and up to £3,279.88 if you settle at 15mg, at standard prices before discounts. A year here means 52 weeks, which is 13 pens. Both figures assume you start at 2.5mg and step up through the doses, at least 4 weeks on each, before reaching your maintenance strength.

If you step up after 4 weeks on each dose, your first four pens (2.5mg, 5mg, 7.5mg and 10mg) cost £879.96 at standard prices. Everything after that depends on where you settle.

Do not forget what happens after year one. Your cost does not stop after 52 weeks if you stay on treatment, and stopping has its own considerations. 07 · NHS versus private

NHS or private: the honest picture

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

That makes the NHS the lowest-cost route for people who fit the current rollout stage, and the NHS has its own guidance on the medicine.⁵ If you think you may qualify, ask your GP where your area stands. The NHS prescription charge in England is £9.90 per item, though whether that charge applies to your own prescription depends on your circumstances, and your GP or pharmacist can confirm it.

Private treatment is the route for people who do not yet fall into an NHS group, or who prefer not to wait.

These are not rival choices to pick between forever. Some people use private treatment while they wait for NHS access, and others start on the NHS and later move to private. If you move between routes, tell both clinical teams what you are taking and at what dose, so your records and your dose plan stay consistent.

08 · Who it is licensed for

Who Mounjaro is licensed for

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹ A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.

Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Eligibility matters to cost because paying for a medicine that is not suitable would waste money. That is the reason the consultation comes before payment. Insulin and sulfonylurea users are a common question. Doses of those medicines may need adjusting because of the risk of low blood sugar. This is something to discuss with your prescriber before starting, and not something to change yourself.

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹

There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar. New symptoms should be discussed with your prescriber and can be reported through the Yellow Card scheme.

09 · What you pay for: results

What the price is buying: trial results

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages. Individual results vary, and trial averages do not predict your own result.

For a worked example only: for someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg. That is straight arithmetic on the trial percentages, not a forecast for you, and it says nothing about how quickly any of it happens.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.²

The trial ran for 72 weeks, longer than a 52-week year of pens, so treat the trial duration and the pen count separately. Also keep in mind that treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Are you trying to decide whether Mounjaro is better than Wegovy? The first large head-to-head trial (SURMOUNT-5) compared tirzepatide with semaglutide, and which suits you depends on your health profile, tolerance and prescriber advice. Both are effective licensed treatments. Mounjaro is a once-weekly injection that activates GIP and GLP-1 receptors, while semaglutide is a single-action GLP-1 medicine. The honest summary: the money buys a licensed medicine plus clinical oversight, with results that average well in trials but differ person to person. Weigh that against your budget and your health needs, not against a headline percentage.

10 · Cutting cost safely

What not to do is chase a bargain from an unverified seller.

Wegovy is the other injectable many people consider on cost. Its dose ladder runs 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, with 4 weeks per step, reaching the 2.4mg maintenance dose from week 17. Neither medicine's price should be your only criterion, since suitability comes first.

11 · Counterfeits and genuine pens

Cheap Mounjaro and counterfeit risk

An unusually cheap Mounjaro pen is a warning sign, not a bargain. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷ Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.¹² Genuine treatment always involves a prescription and a clinical consultation, so anyone offering to supply without a consultation or prescription should be avoided.

Mounjaro is manufactured by Eli Lilly. Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked. When your pen arrives, check it. A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.

For storage, keep unopened pens in the fridge until first use; they keep until the printed expiry date. For anything about unopened pens out of the fridge, check the patient information leaflet and ask the pharmacy.¹³ Once a pen is in use, the licensed window out of the fridge is up to 30 days (not above 30°C).

The cost of a fake can be far higher than the saving. A pen of unknown content can cause harm and still leave you without the treatment you were paying for. If the price seems too good, ask where the pen came from, whether a prescriber reviewed you, and whether the pharmacy is on the register.

Serious side effects are uncommon but need urgent attention: for severe, persistent stomach pain, pain in the upper right abdomen with fever, yellowing of the skin or eyes, or signs of a severe allergic reaction, get medical help straight away. What has already happened is clear: What we can state is what has already happened: Eli Lilly raised the UK list price effective 1 September 2025, from £92–£122 per month to £133–£330 per month depending on dose.¹⁰

This matters for timing. Waiting for a price drop carries a cost of its own, because you spend those weeks without treatment you might benefit from. If you are suitable for treatment and it fits your budget today, there is no basis for assuming a lower price is coming.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
