Mounjaro Missed Dose Side Effects: What Happens and What to Do (October 2026)

Missing a Mounjaro dose is not dangerous in itself, but appetite control and blood sugar control can slip, and stomach side effects may return if you restart after a long gap. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 Current as of October 2026.

A quick overview

What to know
before you start.

  • A missed dose is usually a manageable event: you either take it late inside the licensed window or skip it and carry on.
  • The most likely effects are returning hunger and, if you have type 2 diabetes, less steady blood sugar.
  • Stomach side effects are most likely when starting or moving up a dose, so a long gap is worth discussing before you restart.
  • Never take two doses within 3 days of each other to make up for a missed one.
  • Severe stomach pain, signs of allergic reaction or persistent vomiting need medical help straight away, whether or not you missed a dose.
In this guide

01 · The core answer

What to do if you miss a dose of Mounjaro

The rule depends on how late you are. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.113

There is no catch-up dosing, no half dose and no need to change your treatment plan because of one late injection. If you are unsure which side of the line you are on, count from your scheduled injection day and ask your prescriber before you inject.

Your situationWhat to doWhat you may notice
Inside the licensed window after your scheduled injection dayTake the dose as soon as you can, then return to your usual day.Little or nothing for most people; a short period of stronger hunger is possible before you inject.
Beyond the licensed windowSkip the missed dose entirely and take the next one on your normal day.Appetite may feel louder for a while, and blood sugar can run higher if you treat type 2 diabetes.
Several weeks without a doseContact your clinical team before restarting; do not simply guess at a dose.Stomach side effects are most likely when starting or moving up a dose. Your prescriber advises on the dose to resume.
Unsure how long it has beenHold off, check the date, and ask your prescriber or pharmacist. No harm from a short pause while you check.

If you are on a repeat plan with us, our prescribing team reviews dose and follow-up with you, so you can ask about a missed dose as part of your monthly review. You can see current options on the Mounjaro pen page, and our weight loss clinic guide covers the wider treatment pathway.

02 · What to expect

Which side effects can a missed dose cause?

A missed dose does not create a new, special set of side effects. What changes is how much tirzepatide is working in your body, and the effects you notice follow from that.

Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 When a dose is late or skipped, those effects soften gradually, so the realistic consequences are:

  • Returning hunger. Most people feel fuller sooner and less hungry between meals on treatment, so you may notice food noise creeping back.
  • Less steady blood sugar. If you use Mounjaro for type 2 diabetes, tell your prescriber about any gap so they can advise on monitoring.
  • Digestive effects on restarting. Nausea, diarrhoea, vomiting, constipation and abdominal discomfort are the usual stomach side effects, and they are most likely when starting or moving up a dose.1

A short delay inside the licensed window rarely causes anything you would notice.

03 · Appetite and blood sugar

Appetite and blood sugar after a missed dose

Appetite is usually the first thing people notice after a missed Mounjaro dose. Because the medicine helps you feel fuller sooner and less hungry between meals, a late injection can mean those signals feel weaker in the days before your next dose.

This is not a sign that the treatment has stopped working. It reflects falling levels of the medicine in the body, which then recover once you inject again. Planning helps: keep balanced, protein-rich meals and snacks to hand on the days you are waiting, and avoid the temptation to compensate by eating very little afterwards.

Blood sugar is a separate point. Mounjaro is licensed for type 2 diabetes as well as weight management, and it improves the way the body responds to insulin.1 If you take it for diabetes, a missed dose can mean less steady blood glucose levels, so follow the monitoring plan your prescriber has given you and ask for advice if readings drift.

If you also take insulin or a sulfonylurea, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.1

04 · Restarting and stomach effects

Restarting: why stomach side effects can return

Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.1 That pattern is why Mounjaro uses a gradual dose ladder. Everyone starts Mounjaro on 2.5mg once a week.1

After a missed dose inside the licensed window, you simply carry on with the dose you were already tolerating. After a long gap, the position is less clear-cut. Your prescriber decides how to resume. They can hold your current dose longer before stepping up if a dose is hard to tolerate.1

In the weight-management trials, nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.1 The most common stomach effects are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and they affect more than 1 in 10 people.1

Do not push through severe symptoms or pick your own restart dose. Contact your clinical team, tell them how long you have been without a dose, and let them set the pace. If you are weighing up a longer break, our guides on missing a week of Mounjaro and missing three weeks go into the detail.

05 · Is it withdrawal?

Is a missed dose a withdrawal reaction?

No. What you feel is not a withdrawal reaction.

Tirzepatide has a long half-life, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

If you are pausing deliberately, for example around surgery or a medical procedure, tell your prescriber and the team involved so the timing can be planned.

06 · Changing your injection day

Changing your injection day instead of skipping

If your usual injection day keeps clashing with work or travel, moving it is better than repeatedly missing it. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 That flexibility means a late-evening or morning slot works equally well, so choose a time that fits your routine and set a recurring phone reminder.

The same minimum gap is the reason you must never take two doses within 3 days of each other. If you are not sure whether enough time has passed, wait and ask rather than injecting early.

Once you have settled on a new day, keep to it. A regular weekly schedule keeps your dose steady and makes it easier to spot when something has gone wrong.

07 · Managing symptoms

Managing symptoms after a missed or restarted dose

Most symptoms after a gap respond to the same practical steps that help when you first start. Eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated, and avoid lying down straight after eating.1

If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.1

Constipation is another frequent complaint because food moves through the gut more slowly and many people eat and drink less. Fluid comes first, then fibre from vegetables and whole grains, then daily movement. If it persists, a pharmacist can recommend a suitable laxative, and you should contact your clinical team for pain, bloating or more than a few days without a bowel movement.1

Tiredness can also show up around dose changes. It usually reflects the body adjusting and simply eating less, so keep meals balanced with enough protein, stay hydrated and keep gentle activity going. Tell your clinical team if it is severe or not settling.

Injection-site reactions such as mild redness, itching or a small bruise are common and normally settle within a few days. Rotate the site with every dose, and contact your team if a reaction is spreading, hot or very painful.1 Licensed sites are the stomach area, the front of the thigh, or the back of the upper arm if someone else gives the injection.

08 · When to seek help

When to seek medical help

Serious side effects are uncommon, but they need urgent attention whether or not you have missed a dose. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis.1

Get medical help straight away for pain in the upper right abdomen, fever or yellowing of the skin or eyes, which could signal gallbladder problems. Seek immediate medical help for signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing.1

For symptoms that are not serious but are new or worrying, speak to your prescriber. You can also report any suspected side effect through the MHRA Yellow Card scheme.19

If your mood drops or you have distressing thoughts during treatment, seek help promptly. Regulators have looked at this closely and the EMA's safety committee concluded that the evidence does not support a causal link, but big changes in weight, appetite and routine can affect how you feel. Our guide to psychological side effects covers this in more depth.17

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

09 · Avoiding missed doses

How to avoid missing doses in the first place

Most missed doses come from routine, not forgetfulness, so build the injection into something fixed in your week. Pick a day, set a repeating reminder, and keep the pen where you will see it, remembering that it must be stored in the fridge.

Travel is the other common cause. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded.1 Keep the cap on to protect it from light, and never use a pen that has been frozen.

If supply is the problem, plan repeat orders ahead; we re-check suitability on every repeat order, and there is no auto-shipping.

Our free companion app, GLPTrackr, can help you log injections and keep your journey on track. Reminders and a consistent injection day help prevent gaps.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
What happens if I miss a dose of Mounjaro?
Nothing dangerous happens in itself. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose and take the next one on your normal day. You may notice stronger hunger for a while.
Can missing a dose of Mounjaro cause side effects?
A late dose does not create new side effects, but you may notice returning appetite and, if you have type 2 diabetes, less steady blood sugar. Stomach effects such as nausea or diarrhoea are most likely when starting or moving up a dose, so they are more of a concern after a long gap than a short delay.
Can I take two doses to make up for a missed one?
No. Never take two doses within 3 days of each other. If you are inside the licensed window, take the missed dose and return to your usual day. If you are beyond it, skip the missed dose entirely and inject on your normal day. If you use more Mounjaro than you should, talk to a doctor immediately.
Will I get withdrawal symptoms if I miss Mounjaro?
What you feel is not a withdrawal reaction. What you feel is the gradual fading of the medicine's effects. Tirzepatide has a long half-life, and after a final dose it takes roughly four to five weeks to clear the system, so levels fall slowly. Nothing needs doing while it clears.
Do I need to restart at a lower dose after missing Mounjaro?
After a long gap this is a prescriber decision. After a short delay inside the licensed window you simply carry on with your current dose. After an extended break, contact your clinical team, tell them how long it has been, and let them advise on the dose to resume.
Can I change my injection day instead of missing a dose?
Yes. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose. Mounjaro can be injected at any time of day, with or without food, so pick a day and time that fits your routine and keep to it.
When should I get urgent help after a missed or restarted dose?
Seek immediate medical help for severe, persistent stomach pain that may spread to your back, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.
