The most plausible explanations are ordinary and linked to how the medicine feels in your body, not to a direct effect on sexual function. Tirzepatide slows how quickly the stomach empties and helps you feel fuller sooner, which supports a reduced-calorie diet.1 That change in appetite and eating pattern can touch many parts of daily life.

Digestive effects are the most common. Nausea, diarrhoea, vomiting, constipation and abdominal discomfort are the usual ones, and they are most likely when you start or move up a dose.13 Feeling bloated, queasy or worried about your stomach is not a recipe for feeling romantic, and that is entirely understandable.

Fatigue is also recognised. It is most noticeable in the first weeks and after dose increases, and usually reflects the body adjusting plus simply eating less.13 Low energy and low desire often travel together.

04 · Side-effect timing

How long do these effects last?

Most side effects follow a pattern: they appear in the first days after starting or increasing a dose, peak early, and settle as your body adapts.13

For fatigue specifically, energy returns to normal for most people within a few weeks. Tell your clinical team if tiredness is severe or not settling.13

If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. Speak to your clinical team rather than pushing through severe symptoms.13 So if your desire dipped after an increase and you feel generally unwell, ask whether staying on your present dose for longer makes sense.

Because the medicine takes roughly four to five weeks to clear after a final dose, any effect that is truly drug-related would not vanish overnight if you stopped, and there is no way to speed this up.1 Never stop or change dose without speaking to your prescriber first.

The practical steps that help most people with the digestive and energy effects are the same ones that may help you feel more like yourself.

  • Eat smaller meals slowly, and stop at the first feeling of fullness.
  • Favour plain lower-fat foods while you adjust.
  • Stay well hydrated and avoid lying down straight after eating.
  • Keep meals balanced, with protein especially, and keep gentle activity going.

If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.

Beyond that, talk to your partner. A drop in desire during a period of physical change is common to discuss and easier to handle when it is out in the open. Closeness that does not depend on sex can keep a relationship connected while you find your feet.

Mood and desire are closely connected, so it is worth being clear about what regulators have found. The EMA's safety committee reviewed whether these medicines are linked to low mood or suicidal thoughts and concluded in 2024 that the evidence does not support a causal link, and the MHRA keeps this under continuous review.17

That said, any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17
