03 · What Mounjaro is
What Mounjaro is and what it is licensed for
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 Those two licensed uses are the foundation of every honest answer about its health benefits.
The licence matters because it tells you where the safety and efficacy evidence has been reviewed by regulators. When a benefit sits inside the licence, it has been assessed against the standards of the product information. When a benefit sits outside, you are reading someone's interpretation.
Mounjaro comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 That range exists because treatment starts low and is adjusted. A prescriber decides which strength suits you, and the plan is reviewed rather than fixed on day one.
Because it is a prescription-only medicine, there is a gatekeeping step that protects you. A short health questionnaire is reviewed by a GPhC-registered prescriber, not a scoring script. 04 · How the dual action works
How the dual action produces its benefits
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. Almost every benefit that is genuinely established traces back to one of those three effects.
Start with appetite. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That is why people often describe feeling in control around food, and why "why do I feel so good on Mounjaro?" is such a common question. Quieter hunger can feel like relief if you have spent years fighting constant cravings. It is a plausible and understandable experience, but it is a description of appetite, not a claim that the medicine changes your mood directly.
Next, stomach emptying. Food leaves the stomach more slowly, which is part of why you feel full for longer. It is also why some people notice digestive side effects, and why you should read the patient information leaflet and talk to your prescriber about anything that troubles you.13 Slower emptying is also the reason the medicine can reduce absorption of oral contraceptives, which your prescriber will discuss with you if it applies.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1
Finally, insulin response. Improving how the body responds to insulin is why Mounjaro has a licence for type 2 diabetes. If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, so this is a conversation to have with your prescriber rather than something to manage alone.

One practical point on duration: tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1
05 · The weight-loss evidence
The weight-loss evidence from SURMOUNT-1
Weight reduction is the main established health benefit of Mounjaro, and the clearest data come from the SURMOUNT-1 trial. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 That sentence deserves to be read twice. An average is a midpoint, not a prediction for you. Some people lose more, some less, and the comparison group in the trial also changed their lifestyle, so the difference between the groups is what the medicine adds.
The trial also reports how many people reached particular milestones. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

A few honest cautions. The trial figures cover a defined adult population, not everyone.

06 · Beyond the scale
Beyond the scale: blood pressure, cholesterol and other conditions
In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises. If you have a heart, liver, kidney or breathing condition, the right step is to ask your own clinician what treatment may or may not mean for it.
The medicine is licensed for weight management and type 2 diabetes.1 Weight reduction and better insulin response are the direct effects. Anything further is something to discuss, not something to assume. Our related pages on side benefits people ask about take the same careful approach.
The same caution applies to questions such as "does Mounjaro help inflammation?" or "does Mounjaro help ADHD symptoms?" These are not established effects of the medicine. If you are living with ADHD, an inflammatory condition or any other long-term diagnosis, tell your prescriber at the consultation, because it can affect whether and how treatment is appropriate.
There is also a useful way to read marketing about "extra" benefits. Ask four things: who made the claim, what group of people it came from, whether it is in the product information, and whether the person making it is selling something. If the answer to the third is no, treat it as a question for your prescriber.
If you notice any new symptom while taking Mounjaro, whether or not you think it is connected, tell your prescriber. 07 · Mood and mental health
Mood, mental health and "feeling good"
Mounjaro is not licensed as a treatment for any mental health condition. Regulators have looked at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee concluded in 2024 that the evidence does not support a causal link, and the MHRA keeps this under continuous review.
Why, then, do so many people say they feel better? There are understandable, non-clinical reasons. Quieter appetite can ease the mental load of constant food thoughts. Seeing steady progress can lift confidence. Moving more can feel good in itself.
The reverse can also happen. Changing how you eat, losing weight and adjusting to a new routine can be emotionally unsettling for some people. If your mood drops or you have distressing thoughts during treatment, seek help promptly; if you have any thoughts of harming yourself, get help straight away. Do not wait for your next scheduled review.

Tell your prescriber about any change in your mood while taking Mounjaro.
Please also tell the prescriber about any existing mental health condition or medicines you take when you complete the consultation. It helps them judge whether treatment is appropriate and how closely you should be followed up. Finally, a note on expectations. Medicines for weight management are one part of looking after yourself. Sleep, stress, relationships and support all matter, and no injection replaces them. If emotional eating is a big part of your story, say so in your consultation. A prescriber can then think about the whole picture, and may suggest other support alongside treatment.
08 · Getting the most from it
How to get the most from Mounjaro
You get the most from Mounjaro by using it as licensed: alongside a reduced-calorie diet and increased physical activity, with regular prescriber reviews.2 The trial results describe what happens when treatment and lifestyle changes work together, not what the injection does alone.
On food, the question "what are the best foods to eat during Mounjaro?" is common. The general principle is consistent with the licence: eat a reduced-calorie diet, and because you may feel full sooner, choose food that is worth the space. Smaller meals, eating slowly, and paying attention to fullness suit many people. If a food makes you feel unwell, note it and mention it at your next review. On what to avoid, the safest answer is to follow the leaflet and your prescriber's advice, and not to take any other weight-loss product without checking first. In particular, do not combine Mounjaro with unlicensed products or try to alter the dose yourself. Dose stretching and other off-label approaches are not something we support. Activity matters too, and it does not have to mean the gym. Walking, swimming, cycling or anything you will actually keep doing counts. Building the habit while appetite is quieter makes it easier to keep going later.

09 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
10 · Long-term use and access
How long you can stay on it, and how to access it
The pivotal trial ran for 72 weeks, but that is a study length, not a rule about how long treatment may continue.2
Questions like "what are the long-term effects of Mounjaro?" deserve a careful reply. Treatment is monitored, which is one reason ongoing monthly clinical reviews matter.
On dose, "can you stay on 2.5mg and still lose weight?" is best answered by your prescriber.
If you stop, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system.1 Talk to your prescriber before stopping so you can plan what comes next.
Now access. Mounjaro is available on the NHS, but access is being phased in over several years.6 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
