01 · The core answer
Can Mounjaro affect your gallbladder?
Yes, it can, though serious problems are not the usual experience. The product information lists gallstones and gallbladder inflammation among the possible side effects of tirzepatide.1 Gallstones (cholelithiasis) are a common side effect in the weight-management trials, affecting between 1 in 100 and 1 in 10 people.1 Inflammation of the gallbladder (cholecystitis) was uncommon in the weight-management trials, affecting between 1 in 1,000 and 1 in 100 people.1
The same source notes that acute gallbladder problems were linked to the amount of weight lost.1 So the effect is tied to the weight-loss process, and it is a reason your prescriber looks at your history before treatment and keeps reviewing you afterwards.
| Gallbladder-related effect | How the label describes it | What it means for you |
|---|---|---|
| Gallstones (cholelithiasis) | Common in the weight-management trials | Get medical help straight away for pain in the upper right abdomen, fever or yellowing of the skin or eyes |
| Gallbladder inflammation (cholecystitis) | Uncommon in the weight-management trials | Get medical help straight away for pain in the upper right abdomen, fever or yellowing of the skin or eyes |
| Pancreatitis | Listed as a serious effect needing urgent attention | Severe, persistent stomach pain that may spread to the back: stop using Mounjaro and get medical help straight away |
02 · Your next step
Is this relevant to you, and what do you do next?
It is relevant if you have ever had gallstones, gallbladder pain, or a gallbladder operation, or if you notice new pain in the upper right abdomen while on treatment. Prescribers take extra care, or may recommend against treatment, if you have a history of gallbladder problems, so share your full history during your consultation.13
If you are already on treatment and feel well, nothing needs to change. If you develop pain in the upper right abdomen, fever or yellowing of the skin or eyes, get medical help straight away rather than waiting for your next review.
03 · Warning signs to act on
First signs of a gallbladder problem on Mounjaro
The signs that need urgent action are specific. Seek immediate medical help for pain in the upper right abdomen, fever or yellowing of the skin or eyes, as these may indicate gallbladder problems.13 Do not wait to see whether it passes.
Ordinary digestive upset is common on Mounjaro and is not the same thing. Nausea, loose stools, constipation and mild abdominal discomfort are the most frequent effects, especially when you start or step up a dose, and they usually ease as your body adjusts.1 General queasiness that comes and goes after injection day is more typical of normal adjustment.
- Get medical help straight away: pain in the upper right abdomen, fever, yellowing of the skin or eyes.
- Get medical help straight away: persistent nausea or vomiting, or if you cannot keep fluids down.
- Usually manageable at home: mild nausea, bloating or reflux in the days after a dose change.
If you are unsure which category you are in, contact your prescriber or NHS urgent care. It is better to be checked than to guess. Our guide to gastro side effects of Mounjaro covers the everyday digestive effects in more detail.
04 · Gallbladder or pancreatitis?
Gallbladder pain versus pancreatitis symptoms
The two can look alike, which is why both are treated as emergencies. The product information advises seeking immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, as this could be pancreatitis.13
Pain in the upper right of the abdomen, fever or yellowing of the skin or eyes could be a gallbladder problem — get medical help straight away.13 You do not need to work out which one you have. Both need immediate medical help.
| Feature | Possible gallbladder problem | Possible pancreatitis |
|---|---|---|
| Pain location | Upper right abdomen: get medical help straight away | Severe, persistent stomach pain that may spread to the back: get medical help straight away |
| Other signs | Fever, yellowing of skin or eyes: get medical help straight away | With or without vomiting |
| Action | Immediate medical help | Immediate medical help |
People with a history of pancreatitis should tell their prescriber before starting, because that history calls for extra care.1 If you suspect either condition, get medical help straight away. Seek immediate medical help for signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing. Your clinician will advise on treatment afterwards. Our overview of rare side effects of Mounjaro covers other uncommon problems.
05 · Why weight loss matters
Why the amount of weight lost matters
Acute gallbladder problems were linked to the amount of weight lost.1 Mounjaro works by activating the GIP and GLP-1 receptors, which helps regulate appetite and slows how quickly the stomach empties, so most people feel fuller sooner and eat less.1
Your prescriber guides the pace of your dose ladder, and if a new dose is hard to tolerate they can hold the current dose longer before stepping up.1 If you feel you are losing weight very quickly and have any gallbladder history, mention it at your review.
06 · Who needs extra care
Who needs extra care before starting
Mounjaro is not suitable for everyone, and gallbladder history is one of the things your prescriber weighs. Extra care, or a recommendation against treatment, may apply if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy or gallbladder problems.13
That does not automatically rule you out. It means the decision is individual and made by a clinician who has seen your full picture. Tell them about previous gallstones, any biliary pain, a gallbladder operation, and every medicine you take.
Mounjaro is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber. At EverydayMeds the consultation is free, and an unsuitable order is refunded automatically. Our potential side effects guide lists what else to discuss.
07 · After gallbladder removal
Mounjaro after gallbladder removal
Whether you can start or restart Mounjaro after gallbladder surgery is a prescriber decision. The label asks you to share gallbladder problems in your history, and the assessment takes your surgery, your recovery and any current symptoms into account.13
Nausea, constipation and reflux still apply, and pancreatitis warning signs still matter.13
We cover this in detail in separate guides: Mounjaro after gallbladder removal, taking Mounjaro with no gallbladder and using Mounjaro without a gallbladder.
On wider questions, such as whether weight loss is harder after gallbladder removal, which foods to avoid, or life expectancy, your surgeon, GP or dietitian is the right person to ask.
08 · Reducing digestive strain
Sensible steps while on treatment
There is no proven way to prevent gallstones, pancreatitis or gallbladder problems while taking Mounjaro.
- To ease nausea, eat smaller meals slowly and stop at the first feeling of fullness.13
- Favour plain lower-fat foods while your body adjusts, and stay well hydrated.13
- Avoid lying down straight after eating.13
- Take your dose increases at the pace your prescriber advises rather than pushing through severe symptoms.1
If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention. Side effects are most likely when starting or moving up a dose and for most people ease within days to a few weeks.1

09 · Reporting and support
Reporting a side effect and getting support
You can report any suspected side effect through the MHRA Yellow Card scheme, and doing so helps regulators monitor medicines.19 Tell your prescriber about any new symptom, whether or not it is a listed effect.
If you order with us, ongoing monthly reviews, dose-titration and side-effect support from the prescribing team are part of the service, and repeat orders re-check suitability with no auto-shipping. Orders ship from official LillyDirect supply with free next-day tracked delivery, and same-day dispatch applies when you order by 3pm. Support is available Monday to Friday, 9am to 5pm, by email at support@everydaymeds.co.uk or on 0333 207 0413. For emergencies, use NHS urgent care or call emergency services, not email.
This page is information, not a substitute for medical advice. Results vary, and Mounjaro is used alongside a reduced-calorie diet and increased physical activity.1 Our guides to reported side effects and psychological side effects cover other questions.
