Eli Lilly makes it, and in the UK it is licensed for weight management in adults and for type 2 diabetes.1

It comes as a pre-filled multi-dose pen, called a KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 The strength is the amount of tirzepatide in your dose, so a higher strength means more of the same active molecule, not a different formula.

The pen also contains other, inactive ingredients that keep the solution stable and suitable for injection. The patient information leaflet lists them in full, and if you have an allergy or sensitivity you should read that section and speak to your prescriber before starting.13

05 · How it works

How the molecule works in the body

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). Semaglutide acts on the GLP-1 receptor; tirzepatide acts on GLP-1 and GIP.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

What does fullness and slower stomach emptying feel like in daily life? Most people describe smaller appetite and earlier satisfaction at meals. Those are the intended effects, but they are also linked to common side effects such as nausea or digestive upset, which your prescriber and the patient information leaflet cover in detail.13 Report any suspected side effect through the Yellow Card scheme as well as telling your prescriber.19

A word of caution on interpretation. Knowing that tirzepatide hits two receptors does not mean that more is always better, or that you can adjust the dose yourself. 06 · Trial evidence

What the evidence shows for tirzepatide

The best-known evidence for tirzepatide in weight management is the SURMOUNT-1 trial, and the headline result is a trial average, not a promise. In SURMOUNT-1 of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 The molecule does the pharmacological work of reducing appetite, but the trial participants were also receiving lifestyle support, and that is the context for every figure.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Notice how the numbers rise with strength. Higher is not automatically better, and your prescriber will guide the right pace. Your prescriber balances benefit against tolerability, and many people settle at a lower strength.

For someone starting at 100 kg, 20.9% is about 21 kg. Your starting weight will differ, and so will your result, because the percentage is an average across a large group of people with different responses.

It would be wrong to compare these figures side by side with results from other medicines' trials as if they were head to head, because the trials had different designs and populations. A direct comparison between tirzepatide and semaglutide exists in the SURMOUNT-5 trial, the first large head-to-head trial.15

If a website promises specific results from a supplement or a "formula", treat it with suspicion. The honest position is the trial position: sizeable average effects, wide individual variation, and a medicine that works alongside lifestyle change. 07 · Versus semaglutide

How Mounjaro differs from Ozempic and Wegovy

Mounjaro differs from Ozempic and Wegovy in its active ingredient: tirzepatide versus semaglutide. Tirzepatide activates both GIP and GLP-1 receptors, while semaglutide is a single-action GLP-1 medicine.1 Different molecules mean different formulas, different dose ladders and different licences.

The dose ladders show the practical difference. Wegovy pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg weekly; the dose steps up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17.18 You cannot read across from one scale to the other, because a milligram of one molecule is not equivalent to a milligram of the other.

Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. The licensed use of Mounjaro covers weight management in adults and type 2 diabetes.1 If you want a fair, evidence-led comparison for weight loss, look at the Wegovy pen alongside the Mounjaro pen and speak to a prescriber about which suits you.

Liraglutide is another GLP-1 medicine you may see mentioned. It is supplied at a concentration of 6mg/ml, and unlike weekly Mounjaro and Wegovy it is a daily injection.

Tablets are a growing part of the picture too. Wegovy tablets and Foundayo are oral options with their own product information, and they are not the same medicine as Mounjaro.

What this means for your decision: the formula is the starting point, but the sensible comparison is about tolerability, how you respond, how often you inject, what your prescriber recommends and what you can sustain. Many people find it useful to talk it through in the consultation rather than trying to settle it from a chemistry diagram.

Finally, never switch between these medicines on your own or combine them. Different molecules have different dosing rules, and any change should be made with a prescriber who has your history. That safety position is the same whichever medicine you are considering.

08 · Natural alternatives

Natural alternatives, dupes and copies

There is no natural alternative that shares Mounjaro's formula or has been shown to match its effect. Supplements marketed as "nature's Ozempic" or similar are different substances sold without a prescription, and their makers often rely on dramatic weight-loss claims that the products' evidence does not support. They are low-cost supplements with no clinical oversight, and dosing is not standardised.

The comparison is not close. Tirzepatide is a licensed prescription medicine that activates two receptors and has been studied in large trials.2 A plant extract or fibre product does not have that evidence base, and it does not reproduce the molecule. If a product claims to work "just like Mounjaro", ask what the active ingredient is, who regulates it, and whether a clinician reviews who takes it. Usually the answers are unsatisfactory.

Healthy habits are the legitimate natural route, and they are also what Mounjaro is designed to support. The medicine is used alongside a reduced-calorie diet and increased physical activity.1 Eating more protein and fibre, sleeping well, moving regularly and cutting back on ultra-processed food will not replace the medicine's effect, but they matter whether or not you use it. 09 · NHS and private

NHS and private access to tirzepatide

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

The molecule is the same whether it comes through the NHS or a private pharmacy. What differs is the pathway: NHS eligibility follows the phased criteria, while private supply depends on a consultation and a prescriber's judgement against the licence.

10 · Food and cravings

Sugar, potatoes and chocolate cravings on tirzepatide

Mounjaro is used alongside a reduced-calorie diet and increased physical activity, so food choices still matter.1

Start with how the medicine affects eating. It helps regulate appetite and slows how quickly the stomach empties, and most people feel fuller sooner and less hungry between meals.1 Rich, sugary or fatty food can sit heavily when digestion is slower, so some people find that large portions or very sweet foods leave them feeling unwell. If that happens, smaller portions and slower eating are sensible, and you should tell your prescriber if symptoms persist. The patient information leaflet lists the side effects to watch for.13

Can you eat potatoes? Yes, there is no instruction against them. A reduced-calorie diet is about overall energy intake and balance, so how potatoes are cooked and how much you eat matters more than the vegetable itself. Boiled or baked with the skin on is a different meal from deep-fried chips.

Dark chocolate and sugar are similar. They are not prohibited, but they are calorie-dense and can work against a reduced-calorie plan if eaten freely. Because appetite tends to be lower on treatment, some people find a small portion is enough. If sweet food makes you feel sick, that is a reason to cut back, not a sign of danger in itself.

Why might you still crave chocolate? Cravings have many causes, including habit, stress, tiredness, hormones and routine, and treatment may not remove all of them. It is not reliable to assume a vitamin deficiency from craving sugar, so if you are concerned about your nutrition, ask your GP or pharmacist about testing rather than guessing or buying supplements.

For swaps, think about what you actually want. If it is something sweet, fruit with yoghurt, a small square of dark chocolate eaten slowly, or a hot drink can all help. If it is comfort or boredom, a short walk or a different routine may do more. A dietitian can tailor advice to you.

If cravings feel out of control, or you are skipping meals, losing weight too quickly or feeling unwell, speak to your prescriber. 11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
