What happens on the first day of Mounjaro

On the day, you inject one weekly dose of 2.5mg and then carry on as normal. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.¹

Mounjaro is injected under the skin, once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Many people expect to feel something dramatic. Usually you will not. The first day is often an ordinary day with one small task in it.

First Mounjaro injection: what to expect

The first injection takes only a short time, and the main thing is to be calm and follow the pen's instructions. The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

That feature is a useful safeguard on your first day, because it removes guesswork.

Before you start, read the patient information leaflet in the box from start to finish. Wash your hands, check the pen label matches your prescription, choose a site, and use a fresh needle. Afterwards, dispose of the needle safely in the sharps bin that comes with your order. Do not recap and reuse needles.

Many people feel a quick pinch or nothing at all. If anything at the injection site worries you, or you feel unwell in a way the leaflet does not describe, contact your prescriber rather than waiting to see what happens. For severe, persistent stomach pain, swelling of the face or throat, or difficulty breathing, get medical help straight away.

05 · Choosing your injection day

Choosing your weekly injection day

There is no best time of day, and what matters is the same day each week. Mounjaro works the same morning or evening, with or without food.¹ Pick a day and time you will remember, then stick to it.

Many people choose a weekend morning so that early side effects fall on days off. That is a sensible habit, not a rule. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹ So the choice you make on the first day is not permanent, but changing it often makes it harder to read how you respond.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.¹

06 · What is normal early on

How you may feel after the first dose

What you feel after the first injection varies from person to person, and mild effects are the usual picture. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹³

This fits how the medicine works. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹

Practical habits that many people find helpful in the early days include eating smaller meals, stopping when you feel full, and drinking water through the day. These are general comfort measures rather than treatment instructions. Your pharmacist or prescriber can advise if effects are bothering you.

Some people feel nothing at all after the first injection. That is also normal. A quiet first week does not mean the medicine is not working, and it does not predict how you will respond later.

07 · Weight loss in week one

Can you lose weight in the first week on 2.5mg?

You can, but many people do not, and both outcomes are normal. Some people notice appetite change and early loss in week one, others feel little until a higher dose. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.¹

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.² Trials measured results at 72 weeks, so think in months, not days.

For context, in the SURMOUNT-1 trial people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.² They are not a forecast for week one.

Weigh at the same time of day, on the same scales, and look at the direction over several weeks rather than one reading.

08 · After the first month

What happens after the first injection: the next weeks

After the first injection, you continue weekly on 2.5mg, then step up as your prescriber advises. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.¹

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.¹ Every dose change is a new adjustment, and mild early effects can reappear for a few days after a step up.

Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.²

Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.⁴

09 · What to avoid

What to avoid in the first weeks

Avoid altering the dose or the schedule, because the licensed ladder already starts low for tolerability.

Altering doses outside the licence means unknown dosing accuracy and no safety data. If the first dose feels too much, speak to your prescriber instead of splitting the pen or skipping ahead.

Also avoid treating the medicine as a substitute for habits. Mounjaro is used alongside a reduced-calorie diet and increased physical activity. Protect protein and activity, and keep an eye on the basics such as sleep and alcohol.¹ Plateaus of a few weeks are normal later on, and a plateau is not a reason to double doses or add unlicensed products.

10 · Storing your pen

Storing your pen after the first dose

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.

Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹ If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
