Mounjaro is not a fat-dissolving medicine. What it does is change appetite signalling and digestion. Tirzepatide activates GIP and GLP-1 receptors, helping regulate appetite, slow stomach emptying and improve the body's response to insulin.1

The practical sequence looks like this. You feel full sooner and less hungry between meals. You eat less, and over weeks your energy intake falls below what your body uses. Your body then makes up the shortfall from what it has stored, and that includes body fat. Nothing in this chain is a fat-dissolving step. It is the same basic route that any sustained calorie reduction takes, but with appetite made easier to manage.

Where does the fat go on Mounjaro?

What you can say with confidence is that the pace and pattern differ between people. Some notice changes in the face and waist first, others in the hips and thighs.

Mounjaro cannot target stomach fat specifically. Fat comes off the body as a whole, and where it leaves first is largely down to your own biology. Many people do see their waist shrink as overall weight falls, which is why the question comes up so often, but that is a result of general loss, not a targeted effect.

Do not confuse the stomach area with the injection site. Licensed injection sites include the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 Injecting into the belly does not make fat disappear from that spot, and injecting into the thigh does not make the belly any less likely to shrink.

06 · Eating fat

What happens when you eat fat on Mounjaro?

You can eat fat on Mounjaro, but rich and greasy food often feels heavier while you adjust. Mounjaro slows how quickly the stomach empties, so large portions can sit heavily and some people feel worse after very fatty or fried food.1 Nausea, diarrhoea, vomiting, constipation and abdominal discomfort are the most common side effects, and they are most likely when starting treatment or moving up a dose.1

The practical steps that help most people are to eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated, and avoid lying down straight after eating.13 This is about comfort, not a rule that fat is forbidden.

Good everyday choices include eggs, yoghurt, lean meat, fish, beans and vegetables, all of which carry protein without being heavy. Oily fish, nuts and olive oil can stay in your diet in modest amounts if they sit comfortably. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team, because dehydration needs medical help straight away.13

Tirzepatide's licensed uses are weight management in adults and type 2 diabetes. Liver fat is not a licensed indication for Mounjaro in the UK.1 If you have been told you have fatty liver or any liver condition, raise it with your prescriber or GP.

On "fat metabolism", the medicine works through hormone receptors. Tirzepatide improves the way the body responds to insulin, helps regulate appetite and slows stomach emptying.1 That is a metabolic influence, but it is not the same as a drug that speeds fat burning.

If you notice shakiness, sweating or confusion and you take insulin or a sulfonylurea alongside Mounjaro, that may be low blood sugar, and your prescriber may need to adjust those medicines.1

08 · Protecting muscle

Protecting muscle, face and skin as fat comes off

Losing weight on any method can reduce some lean tissue as well as fat, so the goal is to give your body reasons to keep muscle. The counter-measures are adequate protein, resistance exercise, and a pace of loss guided by your prescriber rather than maximum speed.13 Ask your prescriber or a dietitian what protein intake suits you.

"Ozempic face" is not a drug effect. It is what losing a significant amount of weight quickly can do to facial volume, on any method including diet or surgery. The same applies to loose skin, which commonly adapts gradually after weight stabilises.13

Hair shedding is a recognised side effect and is linked to rapid weight loss itself rather than a direct drug effect. For most people it settles as weight stabilises, and protein intake and overall nutrition matter.13 In the weight-management trials, hair loss was reported by 4.9% of people taking tirzepatide, compared with 1.0% on placebo.1

Fatigue is also common, most noticeable in the first weeks and after dose increases. It usually reflects a mix of the body adjusting and simply eating less, so keep meals balanced (protein especially), stay hydrated, and keep gentle activity going. Tell your clinical team if tiredness is severe or not settling.13

Signs your body is burning fat on Mounjaro

There is no reliable symptom that tells you fat is being burned. Feeling fuller sooner, thinking about food less and eating smaller portions are signs the medicine is acting on appetite, which is the mechanism it is licensed on.1

Better markers over time are practical ones. Clothes fit differently, your waist measurement changes, your portions shrink without feeling forced, and your prescriber reviews your progress. Do not read stomach noises, tiredness or mild nausea as evidence of fat loss. They are more likely side effects of the dose, and they tend to appear in the first days after starting or increasing a dose, peak early and settle as your body adapts.13 That is one reason Mounjaro uses a gradual dose ladder.

10 · Dose ladder and timing

Dose ladder, timing and what changes along the way

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least four weeks the dose increases to 5mg, and can then step up by 2.5mg at minimum four-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the right pace.

You can inject at any time of day, with or without food, as long as it is the same day each week.1 If you miss a dose, take it as soon as you can within four days (96 hours) of the scheduled time. If more than four days have passed, skip it and take the next one on your normal day. Never take two doses within three days of each other.1

Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.13 If a dose is hard to tolerate, prescribers can hold the current dose longer before stepping up, so speak to your clinical team rather than pushing through severe symptoms.13

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis), for pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems), or for signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13 You can report suspected side effects through the Yellow Card scheme.19

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you take insulin or a sulfonylurea, watch for low blood sugar, because those doses may need adjusting.13 Always tell your prescriber about every medicine you take, including anything bought over the counter.

On mood, regulators have looked at whether GLP-1 medicines are linked to low mood or suicidal thoughts. The EMA's safety committee concluded in 2024 that the evidence does not support a causal link, and the MHRA keeps this under continuous review. Even so, tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.
