Feeling fuller sooner also changes how you eat. Smaller portions tend to feel like enough, and long gaps between meals are easier to manage. That is a useful effect, but it is a support for your plan, not a replacement for one.

05 · Starting dose and titration

What the starting dose and step-ups feel like

You begin on the 2.5mg pen, the lowest of six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

The starting dose exists so your body can adjust. It is not designed to be the dose you stay on.

Each pen contains four weekly doses, and once in use a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, then must be thrown away.13 Unopened pens are stored in the fridge between 2–8°C until first use, and keep until the printed expiry date.13

What the trials show about results

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

The same trial reported spread as well as averages. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

Read that spread carefully. Averages hide people who lost more and people who lost less.

How much can you lose in four weeks?

Weight loss on Mounjaro is gradual by design, so think in months, not days. Early weight change is also affected by water, food intake and the dose you are on, so a quiet first month does not mean the medicine is not working.

07 · Side effects in practice

Side effects: what to expect and when to act

Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13 Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.

Sun sensitivity is not listed as a side effect in the UK patient leaflet. Raise it with your prescriber anyway, since a new symptom deserves a proper conversation rather than a guess.

People ask about foods to avoid, fizzy drinks and chocolate more than almost anything else.

Tirzepatide slows how quickly the stomach empties and makes most people feel fuller sooner.1 Because of that, large or very rich meals may feel uncomfortable for some people, and smaller portions often feel like enough. Fizzy drinks and chocolate are not prohibited by the product information, but how they feel is individual, so notice your own response and adjust.

The medicine is used alongside a reduced-calorie diet, so whatever you eat still counts.2 If you have a condition such as diabetes, ask your prescriber or GP for dietary advice that fits your situation.

Can you speed up weight loss on Mounjaro?

There is no approved shortcut. Following your plan, staying on schedule with your injections and keeping up a reduced-calorie diet with more physical activity is what the trial evidence supports. Taking extra doses or shortening the interval is unsafe and unlicensed.

The most useful lessons are practical rather than dramatic.

  • The first pen is a starting point. You step up through the strengths gradually, so early impressions describe the lowest dose, not the whole treatment.
  • Storage matters. Keep pens in the fridge at 2–8°C and never freeze them; once in use, a pen can be kept at a temperature not above 30°C for up to 30 days after first use.13
  • Pick a regular injection day. It is a once-weekly medicine, so a consistent day helps you stay on schedule.
  • It is a tool, not a replacement for effort. Diet and activity remain part of the plan.

10 · Stopping and what happens next

Stopping Mounjaro: what to expect

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

That means appetite effects do not vanish overnight when you stop. They fade gradually as the medicine leaves your body. Stopping should always be planned with your prescriber. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. If you are planning a pregnancy, Mounjaro must not be used during pregnancy and should be stopped at least one month before trying to conceive.

Treatment is designed alongside lasting changes to how you eat and move, which is why the trial framed it as support for lifestyle change.2

11 · NHS or private route

NHS or private: how people get Mounjaro in the UK

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
