If a site lets you add a pen to a basket with no health questions, treat that as a warning sign.

How to vet any online pharmacy

The safest way to vet an online pharmacy is to verify it independently rather than trusting the badges on its own site. Second, confirm that a prescriber reviews your answers. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber.1 A site that skips this step, or offers to supply without a consultation or prescription, is not operating as a regulated pharmacy. That applies to social media sellers, marketplace listings and anyone sending pens by courier with no assessment.

Third, be wary of counterfeit risk. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme, which lets you report suspected fake products and side effects.719 A price far below the rest of the market, no pharmacy details, or pens that look different from the manufacturer's packaging are all reasons to stop. 07 · What Mounjaro is

What Mounjaro is and how it works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). It is one reason people compare the two, though you should not read trial results across different studies as a head-to-head contest.

The pen is the same product whichever regulated pharmacy supplies it, so the meaningful differences between providers lie in price, service, clinical oversight and delivery, not in the medicine itself.

Doses are not chosen on day one by preference. Treatment starts at the lowest strength to let your body adjust, then increases in steps.

You do not choose your own pace. Your prescriber reviews how you are tolerating each step and whether you need to go higher, and some people stay at a lower strength. Moving up faster than prescribed is not something we support.

08 · Expected results

What results to expect

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

The same trial reported how many people reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

It is worth reading those numbers carefully. They describe groups of volunteers followed closely in a clinical trial, so trial averages do not predict your own result. Some people lose more, some less, and some stop for tolerability reasons. A pharmacy that promises specific results for you is overstating what the evidence supports, and we do not do that.

Higher is not automatically better, and your prescriber will guide the right pace. Your monthly review exists to make that judgement with you.

Finally, keep the context in view. These results describe people who also made dietary and activity changes. If the medicine reduces your hunger, the sensible response is to use that space to build habits you can keep, not to treat the pen as the whole plan. 09 · Stopping and maintenance

Stopping, maintenance and keeping weight off

Nothing needs doing while Mounjaro clears from your system. Tirzepatide has a half-life of about five days, so after a final dose it takes four to five weeks to clear the system, and there is no way to speed this up.1 Tell your prescriber if you plan to stop so that your records are right.

Can you take it indefinitely? There is no single answer that suits everyone. How long you continue is a decision for you and your prescriber, made at monthly reviews, based on how you are doing, how you tolerate the medicine, your health goals and your wishes. Some people reach a maintenance strength and stay on it, and others come off after reaching their goal.

Your prescriber reviews your response: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you. Whether to continue is a clinical judgement, not a cut-off number. Your prescriber looks at the whole picture, including whether the medicine is still helping and how well you tolerate it.

Keeping weight off afterwards comes down to what you built while the medicine was lowering your appetite. The product is used alongside a reduced-calorie diet and increased physical activity, so the habits that carried the benefit are the ones to keep: regular meals you can sustain, consistent activity and ongoing monitoring of your weight. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5% on average. 10 · Storage and 30 days

Storage and the 30-day pen rule

Once in use, a Mounjaro KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. That is the answer to why you cannot use a pen after 30 days (not above 30°C): the product information sets that in-use limit, and beyond it sterility and potency are no longer assured.113 Do not use a pen that has been above 30°C or has been frozen.

Unopened pens are different. Store them in the fridge (2–8°C) until first use, and they keep until the printed expiry date. If you are unsure about an unopened pen that has been out of the fridge, for example during a delayed delivery, read the patient information leaflet and contact the pharmacy rather than guessing. We would rather answer that question than have you use a pen you doubt.

Travel is a common concern. If you will be away, plan how you will keep the pen at the right temperature, and think about airline and hotel arrangements ahead of time. Keep pens out of direct sun, and never leave them in a car.

The pen is multi-dose, so you do not use it up in a single injection. Your supply is designed around the four-week pattern: a pen per month at your prescribed strength. Do not share pens or needles with anyone, and use the sharps bin we include for used needles.

11 · NHS or private

NHS or private: the honest picture

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you may qualify for NHS treatment, your GP is the right first conversation. Ask whether you fall within the groups being prioritised in your area. The NHS has its own guidance on the medicine, which is worth reading.5 A private route is not a replacement for that conversation, and nothing stops you asking about both.

The private route exists for people who do not currently qualify for NHS treatment or do not want to wait. In that case, you pay for the pen yourself, and the pharmacy you choose matters. 12 · Safety and side effects

Mounjaro side effects and safety

Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13 If you notice anything new or worrying after a dose, tell your prescriber, and report it through the Yellow Card scheme.19

Because Mounjaro slows how quickly the stomach empties, it can reduce absorption of oral contraceptives. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 MHRA guidance covers this, and your prescriber will talk through your options during the consultation.7 Tell us about every medicine you take, including contraception.

If you use insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. That is a matter for your prescriber and diabetes team, not something to adjust alone.

Tell your prescriber about any new or lasting symptom, and you can report suspected side effects through the MHRA Yellow Card scheme.

Safety also depends on how you buy. Buying from an unregulated seller means no assessment of whether the medicine suits you and no certainty about what is in the pen. Aftercare includes ongoing monthly reviews, dose titration, follow-up assessments and side-effect support.

Finally, remember who should not simply press ahead. Mounjaro should not be used during pregnancy and should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy or gallbladder problems. Answer the health questions fully and honestly, because the review is only as good as the information it receives.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Once a KwikPen is in use, it can be kept out of the fridge for up to 30 days (not above 30°C), and after that limit sterility and potency are no longer assured. Any medicine remaining should be discarded, even if some is left visible. Unopened pens should be stored in the fridge (2–8°C) until first use and keep until the printed expiry date. If you are unsure, read the patient information leaflet or ask the pharmacy.
How to keep the weight off after Mounjaro?
Mounjaro is used alongside a reduced-calorie diet and increased physical activity, so the habits you build during treatment are the ones to keep. As the medicine clears over four to five weeks, your appetite may return gradually, so plan regular meals, consistent activity and ongoing weight monitoring in advance. Talk to your prescriber before stopping, because they can help you plan the transition. Individual results vary, and ongoing support makes a difference.
