Is there a Mounjaro cartridge?

No: Mounjaro is a pre-filled multi-dose pen (KwikPen) in six strengths, and the medicine is already inside it.1 Tirzepatide is the active ingredient and Mounjaro is the brand name, so both names refer to the same product. Mounjaro is a once-weekly injectable prescription medicine made by Eli Lilly, licensed in the UK for weight management in adults and for type 2 diabetes.1

In practice this means three things. You do not buy refills for a reusable pen. You do not draw medicine from a vial into a syringe. You choose the strength your prescriber has agreed with you, and you receive a pen at that strength. When you move up a dose, you receive a different pen, not a different cartridge for the old one.

If someone offers you tirzepatide in cartridges, vials or powder, treat it as a warning sign. Unlicensed products offered outside a regulated pharmacy carry risks that no label or leaflet covers. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting problems.7

05 · What the KwikPen is

What the KwikPen is and why there is no cartridge

The KwikPen is the form in which Mounjaro is supplied: a pre-filled multi-dose pen in six strengths.1 The word cartridge usually describes a replaceable medicine container that slots into a reusable injector. That is not how this product is presented, and the patient information leaflet describes the pen as a whole.13

It helps to think of the pen as a sealed unit. The medicine is already inside, the dose is set by the pen's mechanism, and you attach a new needle for each injection.

Each pen is built for a month of weekly use. If your dose changes, the next pen simply has a different strength printed on it.

Several other injectable medicines, including some insulins, are sold in cartridge form, so the term carries across. Liraglutide products such as Saxenda are a daily injection supplied at a concentration of 6mg per millilitre, which is a different regimen from the once-weekly Mounjaro pen. Mixing the formats up can lead to wrong assumptions about dosing, so it is worth being clear about which medicine you hold.

Check three things on any pen you receive: the name Mounjaro, the strength, and the expiry date. If any of these looks wrong, or the packaging seems altered, contact the pharmacy that supplied it before you use it. If you bought from a seller offering to supply without a consultation or prescription, do not inject; speak to a pharmacist or your GP. Suspected counterfeit medicines and side effects can be reported through the Yellow Card scheme.19

Finally, note that tirzepatide is the generic name and Mounjaro the brand. In the UK, if a pharmacy sells tirzepatide, it should be the Mounjaro KwikPen from the licensed manufacturer, supplied against a prescription.

06 · Strengths and dose steps

Strengths and how the doses step up

Mounjaro comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Treatment starts low and moves up in steps, so most people will use several different pens over time.

Each pen is designed to deliver one set strength.

What does this cost? The standard prices per pen are £169.99 at 2.5mg, £218.99 at 5mg, £279.99 at 7.5mg, £290.99 at 10mg, £319.99 at 12.5mg and £329.99 at 15mg. Those are before discounts. A new customer's first order is £40 less with code FIRST40, and every later order is £25 off the standard price.

Planning ahead is easier with the totals. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. For other maintenance doses, use that dose's monthly price rather than a year figure. Discounts reduce each of these totals.

Your prescriber decides when to step up, hold or step down. Moving up is not automatic and is not a target. Some people stay on a lower strength because it works for them and they tolerate it well. Our monthly reviews exist so that decisions about strength are made with a clinician, based on how you are responding and how you feel.

If you are switching to us from another provider at a higher dose, a new-customer discount still applies to your first order at any dose. Your first order at 15mg, for example, is £289.99. That is a first-order price, so it is not a monthly figure. Repeat orders after that are £25 off the standard price of the dose you are on.

07 · How tirzepatide works

How tirzepatide works inside the pen

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). It does not remove the need for the diet and activity changes your prescriber discusses with you. The medicine makes it easier to eat less; it does not do the eating for you.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.

Your leaflet and prescriber advise what to do if you miss a dose, and we do not improvise extra injections from a pen. The pen's design is built around a regular weekly schedule.

Because Mounjaro slows stomach emptying, it can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you use oral contraception, discuss it with your prescriber before starting. If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, so tell the prescriber about every medicine you take.

The surrounding paperwork, such as the European assessment documents, describes the same substance and the same weekly use. If a seller describes a product as tirzepatide but not Mounjaro, ask what licence it holds and who made it. A genuine UK supply chain can answer that question; a counterfeit one cannot.

08 · Storage and pen life

Storing your pen: fridge, room temperature and the 30-day rule

Store unopened pens in the fridge at 2–8°C until first use; they keep until the printed expiry date.1 Once you start using a pen, the UK KwikPen is licensed for up to 30 days (not above 30°C) out of the fridge.13 After that, discard it, even if some medicine seems to remain.

The UK figure is the one that applies to you. Some online material quotes a shorter in-use window; that comes from a different market's label. Follow the UK patient information leaflet in your box.

What about an unopened pen left out of the fridge by accident, for instance during a delayed delivery or a warm day? If an unopened pen has been out of the fridge, check the pharmacy before using it rather than relying on a remembered day limit. Practical habits that help:

  • Put the new pen in the fridge as soon as it arrives, unless you are about to start it.
  • Note the date you first use a pen, and count the in-use period from that date.
  • Keep pens in their original packaging, away from direct heat and light.
  • Do not use a pen if it has passed its printed expiry date or if it looks damaged.
  • Do not use a pen that has been frozen.

Travelling with a pen? Keep it in your hand luggage rather than the hold, protect it from heat, and carry the dispensing label. A small insulated pouch is a low-cost accessory that some people find useful; it does not replace the leaflet's storage rules.

Finally, always dispose of used needles in a sharps bin. One is included with every order.

09 · Using the pen safely

Using the pen safely: needles, clicks and the injection routine

You attach a new needle for every injection, and the patient information leaflet explains each step, including the setup check before the first dose.13 If you are unsure, read the leaflet or ask our team.

The question about clicks is understandable, since people used to other pens expect to hear a count. Use the pen exactly as described, and contact the pharmacy if the pen does not behave as the leaflet says.

A routine that many people find manageable:

  • Choose the same day each week so doses are evenly spaced.
  • Wash your hands and check the label shows Mounjaro and the strength you expect.
  • Attach a new needle and follow the leaflet's pen check.
  • Inject into the sites the leaflet lists, and rotate the site each week.
  • Remove the needle, dispose of it in a sharps bin and note the date.

What is the best time of day? There is no best time of day: Mounjaro works the same morning or evening, with or without food, so pick a time you can keep up each week. Consistency matters more than the hour, and your prescriber can help you fit it around your routine and any side effects you notice.

Tell your prescriber about any new or lasting symptom. 10 · What the trials show

What the trials show and the 2.5mg question

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example only: for someone starting at 100 kg, 20.9% is about 21 kg. Your own start weight, dose and response will differ, and trial averages do not predict your own result.

Can you lose weight on 2.5mg? The 2.5mg pen is the starting strength, about letting your body adjust rather than weight loss itself. Do not judge the medicine by the first pen alone.

How can you get the most from treatment? The trial context is a reduced-calorie diet and increased physical activity. Speaking with your prescriber about meal patterns, protein, hydration and activity is a better route than hunting for shortcuts. The medicine does not work by itself, which is why our reviews cover lifestyle as well as dose.

What not to eat? No food is banned on Mounjaro. In practice, people often find that large, rich or very fatty meals sit uncomfortably because stomach emptying is slower, so smaller meals can feel easier. Ask your prescriber about your own situation, especially if you have other health conditions.

What about the age question, such as the hardest age to lose weight? Eligibility is about BMI and weight-related conditions, assessed by a prescriber, not age alone.

In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 If you plan to stop, discuss it with your prescriber first.

11 · NHS or private supply

NHS or private: how access works

Mounjaro is available on the NHS, but access is being phased in over several years.6 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Where can you find the cheapest Mounjaro? Compare the dose you need across providers, and look at the full cost of the plan rather than the first pen alone. A very low price from a seller that skips the consultation is a risk, not a saving. Make sure the pharmacy is on the GPhC register, which you can check publicly.12

12 · Spotting unsafe supply

Spotting unsafe supply and counterfeit pens

The safest source of a Mounjaro pen is a GPhC-registered pharmacy that supplies it against a prescription after a consultation reviewed by a prescriber. The GPhC register is public, and you can search for a pharmacy's registration number.12 Ours is 9012878. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for suspected fakes and side effects.7

Warning signs include:

  • A seller offering to supply without a consultation or prescription.
  • Products described as Mounjaro or tirzepatide in formats other than the KwikPen.
  • Prices that look far below what other regulated pharmacies charge for the same strength.
  • Dramatic weight-loss claims or promises of specific results.
  • No registration number, or one you cannot find on the GPhC register.
  • Pens that arrive without a leaflet, with altered packaging or with an unclear expiry date.

The medicine goes into your body by injection, so quality and sterility matter far more than they do for most purchases. A fake pen can contain the wrong amount of active ingredient, or none at all, and nothing about its look will tell you. If you have already used a product you now doubt, speak to a pharmacist or your GP and report it through the Yellow Card scheme.19

13 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
