Mounjaro bum is the everyday label for buttocks that look flatter, looser or smaller after weight loss on treatment. It is shorthand for a change in appearance, not a clinical condition, and you will not find it in the product information.

Mounjaro is the brand name for tirzepatide, a once-weekly injection licensed in the UK for weight management in adults and for type 2 diabetes.1 It helps people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 When people eat less and lose weight, the whole body changes shape. The buttocks are one place where that can show.

03 · Why it happens

Why does your bum go flat on Mounjaro?

The buttocks flatten because the body is losing weight overall, and the medicine is the reason weight is falling. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which regulate appetite and slow how quickly the stomach empties.1 Most people feel fuller sooner, eat less, and lose weight.

In the SURMOUNT-1 trial, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.

Losing that much weight changes how clothes fit and how your body looks. It would be unusual for the buttocks to stay exactly the same while the rest of the body changes.

04 · What you can do

What you can do about a flatter bum

The practical steps are general healthy habits rather than a treatment, and they work alongside your medicine. Treatment is used with a reduced-calorie diet and increased physical activity.2 Think about the quality of both, not only the amount.

  • Eat enough protein. Appetite is lower on treatment, so it helps to plan meals around protein rather than leaving it to chance.
  • Add resistance training. Squats, lunges, hip thrusts and similar lower-body exercises build the muscle that gives the buttocks shape. Start gently and build up.
  • Keep moving. Walking and everyday activity support the physical-activity part of treatment.
  • Review your pace. If you feel you are losing too fast or eating too little, say so at your monthly review. Do not change your dose yourself.
  • Be patient with skin. Appearance often changes over a long period, not overnight.

05 · Injecting in the bum

Can you put Mounjaro in your bum?

Mounjaro is injected under the skin once a week, and the licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. The buttock is not one of these sites, and you should rotate the site with each dose.13

People often ask about the buttock because they want to rotate sites or avoid sore areas. If that is your reason, read the leaflet's guidance on rotating, and ask your prescriber or pharmacist if you are unsure. Where not to inject, and what if it goes wrong

Do not inject into skin that is bruised, sore, hard or damaged, and do not use any site the leaflet does not list. If you think you have injected incorrectly, do not take an extra dose to make up for it. Contact your prescriber or pharmacist and describe what happened.

Report any side effect, including injection-site problems, through the MHRA Yellow Card scheme.19

"Bum wee", bleeding and other symptoms

These are not terms in the product information, so describe them plainly to your prescriber as symptoms. If you have bowel or bladder changes, pain, or any bleeding, tell your prescriber. Do not assume it is related to Mounjaro, and do not assume it is not.

Will stopping bring your shape back?

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system.1 There is no way to speed this up. Stopping should always be planned with your prescriber. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

If you use oral contraceptives, Mounjaro slows stomach emptying, which can reduce how well the pill is absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Tell your prescriber about any new symptoms.
Is Mounjaro bum a real side effect?
