01 · Eligibility

Can you get Mounjaro with a BMI of 26?

Usually not for weight management, because the licence starts higher. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 A BMI of 26 is below both entry points.

Prescribers work within the licence, and the medicine must be used alongside a reduced-calorie diet and increased physical activity.1

02 · Exceptions

Is there any situation where BMI matters differently?

Yes, ethnic background can change the picture. People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk of weight- related health problems at a lower BMI, so clinical thresholds are usually reduced by 2.5 kg/m² for these groups.4

That does not mean everyone from these groups qualifies at this BMI. It means a prescriber may apply a lower threshold when they review you, alongside any weight-related conditions. Tell them your background in the consultation.

04 · Is it safe at this BMI?

Is Mounjaro safe if your BMI is 26?

Mounjaro is only assessed as suitable within its licence, and a BMI of 26 falls outside it for most people. The medicine is not risk-free. The most common effects are digestive: nausea, diarrhoea, vomiting, constipation and abdominal discomfort affect more than 1 in 10 people.13 Taking Mounjaro when you do not clinically need weight reduction means accepting its side effects without the offsetting health benefit.

This is why we do not supply it outside the licensed criteria. A prescriber weighs your BMI, medical history and current medicines before deciding whether it is suitable.1 Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

05 · Who should not take it

Who should not take Mounjaro at any BMI?

Some people should avoid it whatever their BMI. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.1

Always share your full medical history and medicine list. Tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2.1

06 · NHS and private routes

What about the NHS?

NHS access is stricter than the licence. NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first. Full rollout is expected to take years.46 A BMI of 26 is therefore well short of the NHS criteria as well.

How does this compare with other BMIs?

The threshold story changes quickly as BMI rises. Below the licensed range, the answer is generally no. From 27 with a weight-related condition, a prescriber can consider treatment. From 30, a condition is no longer required.

Should you be concerned about a BMI of 26?

Not usually on its own. BMI is a rough screening measure and does not show where you carry weight, your muscle mass or your fitness. A BMI of 26 is in the overweight band, which is worth keeping an eye on, but it is not a medical emergency.

What matters more is the wider picture: blood pressure, cholesterol, blood sugar and family history. Your GP can check these. If any are raised, that can change how a prescriber views your weight, particularly if your BMI is rising towards the licensed range.

What is the lowest BMI requirement for Mounjaro maintenance?
The licensed criteria apply when treatment starts, and your prescriber decides whether to continue at each review. Reviews look at your response, side effects and overall health, and our aftercare includes ongoing monthly reviews.
At what BMI does the body start shutting down?
There is no single BMI figure, because it depends on the individual. A very low BMI is a medical concern in its own right and needs a GP. It is one reason Mounjaro is not prescribed to people who do not clinically need weight reduction.
Do you gain weight back after stopping Mounjaro?
Yes, it is possible. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5%. Appetite effects fade as the medicine clears, which takes roughly four to five weeks, and nothing needs doing while it does. Talk to your prescriber about a plan before you stop.
What should I avoid while taking Mounjaro?
Do not take it if you are allergic to tirzepatide. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. Always give your prescriber your full medicine list and history, including pancreatitis, gallbladder or digestive problems. Read the patient leaflet, and report any side effects through the MHRA Yellow Card scheme. Seek immediate medical help for severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or swelling of the face or throat with difficulty breathing.
Can I stop Mounjaro abruptly?
Mounjaro clears from the body over roughly four to five weeks, and nothing needs doing while it does. Even so, speak to your prescriber before stopping so you can plan diet, activity and follow-up. Contact them if you have new symptoms. If you have severe, persistent stomach pain, get medical help straight away.
