Because tirzepatide has a half-life of about five days, levels build and fall around each injection. Some people notice hunger or constant thoughts about food creeping back in the days before the next dose. That is a pattern to discuss, not a sign that the medicine has stopped working.

The weekly cycle and the half-life

Mounjaro is a once-weekly injection, so your body has a rhythm of higher and lower activity across the week. The label describes tirzepatide as having a half-life of about five days.1 A half-life is the time it takes the amount of medicine in the body to fall by half, so the effect eases gradually rather than stopping suddenly.

That is why some people feel steadier appetite control in the days after injecting and a slightly louder appetite later in the week. It is a pattern linked to how the medicine is dosed, and it does not mean the pen is faulty or the dose has stopped working.

Practical steps that fit within the licence include:

  • Inject on the same day each week, following the pen's instructions.
  • Plan your most structured, filling meals for the days you notice hunger most.
  • Keep regular mealtimes so that the end-of-week dip does not turn into grazing.
  • Record when hunger returns, so your prescriber can see a pattern.

04 · Dose and titration

Dose, titration and why early weeks feel different

Mounjaro comes in strengths of 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg, and treatment begins at the lowest strength.1 The early doses are an introduction for your body. They are not intended to deliver the strongest effect, so a modest appetite change at the start is normal.

Dose increases are made step by step, after at least 4 weeks on each dose, depending on how you respond and tolerate it.

After a dose increase, the effect of the new strength can take time to be felt. If you have recently stepped up and your appetite feels unchanged, give the dose time and raise it at your review.

Dose choices belong with your prescriber, who weighs the benefit against how well you tolerate each step.

05 · Tolerance and resistance

Do you build a tolerance or become resistant to Mounjaro?

Trial evidence shows average results over a long treatment period. In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

Weight loss is not linear, and plateaus of a few weeks are normal as the body adapts to a lower weight. Slower progress is not automatically a sign the medicine has stopped working. A medicine review is the right way to find out whether something else is going on.

For a worked example only: for someone starting at 100 kg, 20.9% is about 21 kg. Trial averages do not predict your own result.

06 · What not to do

What not to do when hunger returns

Do not change how you use the pen to chase a stronger effect. Mounjaro is licensed as a once-weekly injection, and any change to the schedule belongs with your prescriber.

Specifically, avoid the following:

  • Injecting more often or taking extra doses. This is unsafe and unlicensed.
  • Moving to a fortnightly or alternate-week pattern. Mounjaro is licensed weekly, and stretching or skipping to make supplies last is not something we or the label support.
  • Dropping your dose yourself. Questions such as going from 10mg to 5mg are a decision to make with your prescriber, based on your response and side effects.
  • Using a pen past its in-use life. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.113 After that, sterility and potency are no longer assured, so discard it even if medicine remains.

Keep unopened pens in the fridge until first use. They keep until the printed expiry date. For unopened-pen storage questions, check the patient information leaflet or contact the pharmacy.

07 · Habits that support fullness

Everyday habits that make the effect feel stronger

Mounjaro supports a reduced-calorie diet and increased physical activity rather than replacing them.2 If the medicine feels weaker, check the basics first.

  • Eat regular meals. Skipping meals can lead to stronger hunger later in the day.
  • Build meals around protein and fibre. Many people find they stay fuller for longer.
  • Eat slowly and stop when comfortably full. Because stomach emptying slows, large portions can cause discomfort.
  • Notice non-hunger eating. Stress, boredom and routine can drive eating even when the medicine is working.
  • Keep moving. Activity is part of the treatment plan and supports your wider health.

Non-drug tactics such as acupressure points for appetite suppression or acupuncture are not a replacement for a medicine review. Stimulant medicines such as those in our Concerta appetite suppression guide are used for entirely different reasons and are not options to combine with Mounjaro on your own.

08 · When to contact your prescriber

When to speak to your prescriber

Contact your prescriber if hunger has returned and stays strong across several weeks, or if your appetite changes suddenly. Ask for a review too if you are struggling with side effects, eating very little or feeling unwell, because a dose or schedule change may be needed.

Tell them about all other medicines you take. Insulin and sulfonylureas may need dose adjustments because of the risk of low blood sugar. Mounjaro can also reduce absorption of oral contraceptives because stomach emptying slows, so ask about contraception.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

If you notice a new or lasting symptom, tell your prescriber, and you can report suspected side effects through the MHRA Yellow Card scheme.19

09 · If you stop treatment

What happens to appetite if you stop

Stopping Mounjaro will not bring hunger back overnight. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

Do not stop because of one difficult week. Talk it through first, since the issue may be solved by a review rather than by stopping.

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first.46 Many people who meet the licensed criteria choose regulated private treatment instead of waiting.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Store unopened pens in the fridge until first use, and check the patient leaflet for storage details.
Can I go from 10mg to 5mg Mounjaro?
Dose reductions are possible, but they are a decision for your prescriber, based on your response and any side effects. Do not switch strengths on your own, and never combine pens to make up a dose. Contact the prescribing team, explain how you feel, and they can advise whether stepping down suits you.
Should I increase my dose if my appetite suppression is wearing off?
Only your prescriber should change your dose. Doses are increased in steps, after at least four weeks during titration, and the right strength depends on your response and how well you tolerate it.
