03 · What Mounjaro is licensed for

What Mounjaro is licensed for

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.13

Rheumatoid arthritis does not appear among those licensed uses. That is why Mounjaro is not a treatment for RA.

Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary.

04 · How it works

How Mounjaro works, and why it matters with RA medicines

Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals.

This mechanism is about appetite, digestion and blood sugar. It is not described in the product information as acting on the immune system, so it should not be thought of as an anti-inflammatory or disease-modifying medicine.

Slower stomach emptying is the practical point for anyone on regular medicines. The same slowing can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Tell your prescriber and your rheumatology team about everything you take, including tablets, injections and anything bought over the counter. If you use insulin or sulfonylureas for diabetes, doses may need adjusting because of the risk of low blood sugar.

05 · Can it cause RA?

Can Mounjaro cause rheumatoid arthritis?

Rheumatoid arthritis is not a listed side effect of Mounjaro in the product information (SmPC) or the patient leaflet.113

RA can begin at any time in adult life, so it may appear while someone happens to be using any medicine. That timing alone does not show a cause. Still, new joint pain, stiffness, swelling or morning symptoms deserve a proper assessment by your GP rather than guesswork.

If you notice a new symptom after starting Mounjaro, discuss it with your prescriber. You can also report it through the MHRA Yellow Card scheme, which helps regulators monitor medicines.19 Do not stop other prescribed treatment without medical advice.

06 · Weight, joints and expectations

Weight, joints and realistic expectations

Mounjaro's evidence is about weight, not joint disease. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Those figures say nothing about rheumatoid arthritis symptoms, flares or disease activity. Whether weight loss on Mounjaro would reduce your joint pain is a question for your GP or rheumatology team.

07 · Talking to your rheumatologist

Talking to your rheumatology team

Tell your rheumatology team before you start Mounjaro, and bring a full medicine list. They know how your disease is behaving and which of your treatments matter most.

Can your rheumatologist prescribe Mounjaro? That depends on the service and local arrangements. Weight-management prescribing normally sits with GPs, specialist weight-management services or a regulated pharmacy prescriber. Rheumatology clinics focus on RA treatment.

Useful questions to ask:

  • Are there any of my current medicines you want to monitor more closely if my appetite and eating change?
  • Would you want to see me sooner if my symptoms change after I start?
  • Should my blood tests or monitoring schedule be adjusted?
  • Is there any reason my condition makes weight-management medicine unsuitable for me?

Take their answers into your online consultation. A prescriber can then make a safer decision with the full picture.

08 · Common RA questions

Common questions about managing RA

Common medicines and the most successful drug. There is no single most successful drug for everyone. Treatment is chosen by your rheumatology team to suit your disease, other conditions and response. Mounjaro is not one of these treatments.

Controlling RA and managing flare-ups. This is a matter for your specialist team, who can give you a personal plan. Follow it rather than relying on a weight-management medicine to influence your joints.

Disability and life questions. Whether RA counts as a disability, and how it affects work or relationships, depends on your circumstances. Ask your rheumatology nurse or specialist team for advice, or an advice service you trust.

09 · NHS and private access

NHS and private access to Mounjaro

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

  • Do not stop or change RA medicines on your own because you have started a weight-management medicine.

  • Do not treat Mounjaro as a joint-pain remedy.
  • Do not ignore new symptoms. Contact your prescriber or GP.

Mounjaro is used alongside a reduced-calorie diet and increased physical activity. Tell the rheumatology team about any change to your exercise plan, particularly if joints are painful.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
