No: hoarseness does not appear as a listed side effect in the Mounjaro SmPC or the patient information leaflet.113

Mounjaro is the brand name for tirzepatide, a once-weekly injection licensed in the UK for weight management in adults and for type 2 diabetes.1 It works by activating the GIP and GLP-1 receptors, which helps regulate appetite and slows how quickly the stomach empties.1

What to do if your voice has changed

If your voice has been hoarse for only a short time and you otherwise feel well, the sensible first step is to rest your voice, stay hydrated and watch how it develops. Many spells of hoarseness settle by themselves. If it does not settle, or you are unsure, speak to a clinician rather than waiting indefinitely.

What the UK product information says

The Mounjaro SmPC and patient information leaflet are the authoritative lists of recognised side effects, and hoarseness is not among them.113

The leaflet is written for people already using the pen. It tells you which symptoms to report, and it explains that you should speak to your doctor, pharmacist or nurse about any side effect, including ones not listed.13

The leaflet and SmPC are also what the Yellow Card scheme builds on. If you think a symptom could be linked to your medicine, you can report it to the MHRA yourself, and your report helps regulators spot patterns that individual patients and prescribers cannot see.19

04 · Everyday causes of hoarseness

What commonly causes a hoarse voice?

Hoarseness is a symptom, not a diagnosis, and it has many possible causes that have nothing to do with medicines. Throat and chest infections, a cold, shouting or singing, dry air, smoking and irritation from reflux are all familiar reasons for a rough or weak voice. Allergies and a lingering cough can also strain the voice box.

It is common to ask why the voice is hoarse when the throat does not hurt. Pain is not required for the voice box to be irritated or inflamed, and a hoarse voice without a sore throat is not unusual. It also means that absence of pain should not reassure you if the change persists.

A cough from a recent cold can leave the voice rough for some time afterwards. If you have had no illness and the change is unexplained, that is a stronger reason to have it checked.

A GP can listen to your history, examine your throat and neck, and refer you to an ear, nose and throat clinic if needed.

05 · Red flags to act on

Red flags for a hoarse voice

The red flags are a voice change that persists or keeps getting worse, a lump or swelling in the neck, difficulty or pain when swallowing, noisy breathing, coughing up blood and unexplained weight loss that is not explained by treatment. If you have breathlessness or difficulty breathing, get medical help straight away. Any of these should prompt a medical assessment rather than watchful waiting.

People who smoke, or who have in the past, and people who have had previous neck or throat problems, have extra reason to get a lasting voice change looked at. The same applies if you use a voice heavily for work and the change does not recover.

If you are taking Mounjaro and notice a neck lump or trouble swallowing alongside a voice change, tell your prescriber and arrange a GP review; if swallowing or breathing is badly affected, get medical help straight away. 06 · What helps hoarseness settle

What helps hoarseness go away?

The most useful steps are resting your voice, staying well hydrated, avoiding smoking and smoky rooms, and avoiding whispering or shouting, which both strain the voice. Steam and throat lozenges may feel soothing, though they treat comfort rather than the cause.

Eating and drinking patterns can matter when you are on a medicine that reduces appetite. If you are eating and drinking less than usual, a dry throat can make a rough voice feel worse, so keep fluids going through the day. Mounjaro slows how quickly the stomach empties, and most people feel fuller sooner,1 so small, regular drinks are often easier than large ones.

Avoid taking over-the-counter remedies or supplements for an unexplained voice change without asking a pharmacist, because they can interact with other medicines you use. Our pharmacy team and your GP can both advise.

If the hoarseness has not eased after a reasonable period, treat that as the cue to get it examined. Waiting longer rarely helps a problem that is not resolving on its own.

07 · Drugs and vitamins that affect the voice

Which drugs and deficiencies can affect the voice?

Whether a given medicine affects the voice is specific to that product, so the only reliable answer for your own medicines is the leaflet that came with each. If you use an inhaler and notice a rough voice, ask your pharmacist or GP; do not stop it without advice.

The same applies to the question of whether Ozempic makes the voice hoarse. Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. Check Ozempic's own product information.

Vitamin deficiencies are sometimes mentioned online as a cause of a hoarse voice. Do not treat an unexplained voice change with supplements instead of getting it checked. A GP can decide whether any tests are needed.

If you take several medicines, bring the list to your appointment, including Mounjaro, any inhalers, and anything bought over the counter. That gives the clinician the full picture.

How Mounjaro treatment fits in

Mounjaro is a prescription-only medicine used alongside a reduced-calorie diet and increased physical activity, and individual results vary.1

A new symptom is not a reason to quit by itself, nor a reason to ignore it. The decision about whether to continue, pause or change dose belongs with your prescriber, who can weigh the symptom against your overall health and goals.11 If treatment is stopped, tirzepatide has a half-life of about five days, so it takes roughly four to five weeks to clear the system, and nothing needs doing while it clears.1

Reporting a symptom and keeping records

Reporting a symptom through the MHRA Yellow Card scheme is quick and open to patients as well as professionals.19 You do not need proof that the medicine caused it; a suspicion is enough, and the leaflet encourages you to report side effects you notice.13

Before you speak to a clinician, jot down when the voice change began, whether it is constant or comes and goes, whether anything else changed at the same time, and which dose you are on. Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Tell your prescriber about anything new or troubling.
Which vitamin deficiency can cause a hoarse voice?
Do not self-treat an unexplained voice change with supplements. A GP can assess your symptoms, decide whether any tests are needed and look for other causes of the change.
Should I stop Mounjaro if my voice is hoarse?
Do not stop or change your dose on your own. Speak to your prescriber, who can weigh the symptom against your health and treatment. If you stop, tirzepatide takes roughly four to five weeks to clear the system, and nothing needs doing while it clears. Stopping should always be planned with your prescriber: in SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year.
