Exercise with Mounjaro is sometimes treated as optional because weight loss can happen without a structured exercise programme. However, exercise still has an important role during weight loss, particularly when it comes to fitness, strength and maintaining muscle.

Most Mounjaro exercise advice online stops at "start gently and listen to your body". There is more to consider. This guide looks at what can happen to muscle during weight loss with tirzepatide, what research on GLP 1 medicines and exercise has found, how much activity is recommended in the UK, and how to approach exercise when your appetite has reduced.

  • Some of the weight lost during tirzepatide treatment can come from lean tissue as well as fat. This makes strength training worth including in a weight loss plan.
  • Resistance training can help maintain or improve muscle strength, while cardio supports heart and lung fitness and overall health.

Mounjaro Exercise: Why It Matters More, Not Less

Weight loss can involve losing both body fat and lean tissue. This is not unique to tirzepatide and can also happen with other forms of weight loss.

In the SURMOUNT 1 body composition substudy, participants taking tirzepatide lost 21.3% of their total body weight over 72 weeks. Fat mass fell by 33.9% and lean mass fell by 10.9%. The researchers estimated that around 25% of the total weight lost was lean mass and around 75% was fat mass.

This does not mean that tirzepatide specifically causes harmful muscle loss. Lean mass can include more than skeletal muscle, and some reduction in lean mass is expected during substantial weight loss.

The practical point is that maintaining strength and physical function matters while you are losing weight. Resistance training, adequate protein and regular physical activity can all form part of that approach.

Exercise does not guarantee that you will maintain all your muscle mass, but strength training gives your muscles a reason to keep adapting while your body weight changes.

What the Trial Evidence Shows About Combining Exercise and a GLP 1

There is useful randomised trial evidence looking at exercise alongside a GLP 1 medicine, although it is important to understand which medicine was studied.

Researchers enrolled adults with obesity who had first completed an 8 week low calorie diet. They were then randomly assigned to exercise plus placebo, liraglutide plus usual activity, exercise plus liraglutide, or placebo plus usual activity.

The exercise programme involved at least 150 minutes of moderate intensity activity each week or 75 minutes of vigorous activity.

The combination of exercise and liraglutide was associated with improvements in weight related outcomes and body composition. A secondary analysis also looked at physical fitness and muscle strength.

Secondary analysis of a randomised controlled trial using liraglutide, a GLP 1 receptor agonist. Tirzepatide is a dual GIP and GLP 1 receptor agonist, so these findings should not be treated as direct evidence for tirzepatide.

Compared with liraglutide alone, the combined approach improved stair climb performance and peak oxygen consumption. Exercise alone also improved physical fitness, while liraglutide alone did not show the same improvement.

The useful takeaway is that weight loss and physical fitness are different things. A medicine can help with weight management without replacing the benefits of exercise.

There is an important limitation. This trial used liraglutide rather than tirzepatide. Direct evidence on specific exercise programmes during tirzepatide treatment is still developing. The results therefore support the general value of exercise alongside GLP 1 treatment, but they should not be presented as proof that the same results will occur with Mounjaro.

How Much Exercise: The UK Guidance

There is no separate Mounjaro exercise target. The UK Chief Medical Officers' physical activity guidance applies to adults, including people using GLP 1 medicines. The updated 2026 guidance continues to recommend regular activity and strength work.

General population guidance. If you have a heart condition, joint problems, diabetes or another medical concern, speak to your GP or healthcare professional before starting or substantially increasing exercise.

You do not have to reach 150 minutes immediately. The latest UK guidance emphasises that some activity is better than none, and people who are currently inactive can build up gradually.

Strength training is particularly useful to include because the UK guidance recommends muscle strengthening activities on at least 2 days a week.

Mounjaro Strength Training and Muscle Preservation

Resistance training is a useful part of a weight loss programme because it challenges your muscles and helps maintain or improve strength.

Cardio still has an important role, but it does not replace strength training when your goal includes maintaining muscle strength.

You do not need a gym to get started. Bodyweight exercises, resistance bands, weights and some everyday activities can all count as muscle strengthening exercises. The UK guidance recommends working the major muscle groups, including the upper and lower body.

  • Focus on basic movement patterns such as a squat or sit to stand, a push, a pull, a hip hinge or bridge, and exercises for your core.
  • Start at a level that feels manageable and increase the difficulty gradually.
  • You can progress by adding repetitions, resistance, range of movement or better control.
  • Two full body sessions a week can be a simple starting point for many people.
  • Give the muscles you have trained time to recover before working them hard again.

You do not need to train to complete exhaustion. The aim is to challenge your muscles consistently while allowing enough recovery between sessions.

Swimming and other water based activities are useful forms of aerobic exercise, but they should not automatically be treated as a replacement for resistance training if maintaining muscle strength is one of your goals.

Cardio and Mounjaro

Cardiovascular exercise is still worth including. It can improve cardiovascular fitness, support general health and help you stay active during and after weight loss.

Walking is a practical option, particularly if you are new to exercise or are dealing with nausea or low energy. You can divide your activity into shorter sessions and build up gradually.

For example, several brisk 20 minute walks during the week can contribute towards your weekly activity target. You do not have to complete all your exercise in one long session.

If you enjoy higher intensity exercise, you can continue with it if you are already accustomed to that level of activity and feel well enough to train. If you are new to exercise, increase the intensity gradually rather than trying to do too much at once.

If nausea, vomiting, diarrhoea, dizziness or significant fatigue is affecting you, it may be better to reduce the intensity or take a break until you feel better.

Fuelling Training When Your Appetite Has Gone

Reduced appetite can make eating enough food more difficult when you are taking tirzepatide. This can also make planning meals around exercise more challenging.

Protein is particularly important because it provides amino acids needed for normal muscle maintenance and other functions in the body.

The UK reference nutrient intake for protein for adults is 0.75g per kilogram of body weight a day. However, people who are losing weight and doing resistance training may be advised to consume more than this. Dietitians and other healthcare professionals may use higher protein targets depending on factors such as age, body size, activity level and health. There is no single protein target that is right for everyone.

  • Include a source of protein at meals where possible, such as eggs, fish, lean meat, dairy products, beans, lentils or tofu.
  • If your appetite is low, smaller meals or snacks may be easier to manage than large portions.
  • A protein shake can be useful if you struggle to get enough protein from food, but it is not essential for everyone.
  • Drink enough fluid throughout the day, particularly if you are exercising or losing fluid through vomiting, diarrhoea or sweating.
  • If you regularly cannot eat enough food or feel too weak to exercise, speak to your prescriber or another healthcare professional rather than trying to push through it.

There is no need to exercise on an empty stomach unless that is simply what you prefer and you feel well doing it.

Many people can remain active while taking tirzepatide, but certain situations need extra care.

Exercise While Taking Mounjaro: Training Around Injection Day

There is no general requirement to avoid exercise on the day you take your Mounjaro injection.

Tirzepatide has a half life of approximately 5 days and reaches steady state after around 4 weeks of once weekly dosing. There is therefore no simple injection day peak that everyone needs to plan their workouts around.

However, some people notice more nausea, stomach discomfort or other side effects after an injection or following a dose increase. If you notice a clear pattern, you can plan easier activity around the days when you feel less well.

This is about comfort and how you respond to treatment rather than a fixed medical rule. If you feel well, there is no general reason to avoid your usual exercise routine because you have taken your weekly dose.

Building a Mounjaro Workout Plan

A workout plan is most useful when it fits your current fitness level and is realistic enough to maintain.

If you are starting from very little activity, doing too much in the first week can leave you exhausted or sore and make it harder to continue. A gradual increase is often a more practical approach.
