A quick overview

What to know
before you start.

  • Nausea is the most common Mounjaro side effect, and 5mg is the first step-up dose, so it is a typical point for it to appear or return.
  • Most people find it peaks early after the injection day and settles within days to a few weeks as the body adjusts.
  • Smaller, slower, plainer meals, steady fluids and staying upright after eating are the core fixes.
  • If you cannot keep fluids down, get medical help straight away; if you have severe stomach pain spreading to your back, get medical help straight away.
In this guide

01 · The short answer

Is nausea on Mounjaro 5mg normal?

Yes, nausea on 5mg is a recognised and very common effect of the medicine, not a sign that something has gone wrong. The most common side effects are digestive: those affecting more than 1 in 10 people are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.1

Side effects are most likely when starting treatment or moving up a dose. For most people they ease within days to a few weeks as the body adjusts.1 That is why 5mg is a classic moment for queasiness: it is the first time your dose rises after the starting pen.

Nausea in the trials was also tied to the dose changes themselves: it was more common while the dose was being increased and became less frequent over time.1 So a rough first stretch on 5mg does not predict how you will feel on it long term.

02 · Know the difference

Normal adjustment or time to call someone?

Most nausea is mild to moderate and manageable with food and routine changes.

What you noticeUsually meansWhat to do
Queasy for a stretch after the injection, easing as the week goes onTypical adjustment to the new doseSmaller, plainer meals; sip fluids; keep your usual injection day
Feeling full or off food after small portionsSlower stomach emptying, as intendedStop at the first feeling of fullness; eat slowly
Occasional vomiting that settles and you can drink afterwardsCommon effect while the dose is changingRehydrate gradually; tell your clinical team if it repeats
Nausea or vomiting that persists, or you cannot keep fluids downRisk of dehydrationContact your clinical team promptly
Severe, persistent stomach pain that may spread to your back, with or without vomitingPossible pancreatitisSeek immediate medical help
Upper right abdominal pain, fever, or yellowing of skin or eyesPossible gallbladder problemSeek immediate medical help

The last two rows are serious but uncommon effects.113 If you are unsure which row you are in, ask rather than wait.

03 · Your next step

What to do next if 5mg is making you feel sick

For a wider overview, see our guide to Mounjaro nausea in general, or the step-by-step guide to managing nausea on Mounjaro.

If you take Mounjaro through us, ongoing monthly reviews and side-effect support are part of the service.

04 · Why 5mg causes nausea

Why does Mounjaro 5mg cause nausea?

Mounjaro causes nausea mainly because of how it works on the gut. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals. The same slower emptying means food sits in the stomach for longer. If you eat a normal-sized or rich meal while your stomach is emptying slowly, queasiness is a likely result.

Everyone starts on 2.5mg once a week, and that starting dose is about letting your body adjust, not about weight loss itself.1 If you felt fine on 2.5mg, that does not promise you will on 5mg, and if you felt sick on 2.5mg it does not mean 5mg will be worse. Our guide to nausea on the 2.5mg starting dose covers that earlier stage.

05 · Trial rates at 5mg

What the trials show about 5mg nausea

In the trials supporting Mounjaro's weight-management licence, nausea was reported by 24.6% of people on 5mg, 29.0% on 10mg and 28.0% on 15mg, compared with 8.5% on placebo.1

Put plainly, a sizeable minority of people on 5mg reported nausea, a clear majority did not, and nausea also occurred in some people taking placebo.

Diarrhoea followed a similar pattern. Diarrhoea was reported by 18.7% of people on 5mg, 20.8% on 10mg and 22.5% on 15mg, compared with 7.8% on placebo.1 Nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.1

Few people stopped for this reason. 1.9% of people on 5mg, 3.3% on 10mg and 4.3% on 15mg stopped treatment permanently because of gastrointestinal side effects, compared with 0.5% on placebo.1 In SURMOUNT-1, 4.3% of people on 5mg, 7.1% on 10mg and 6.2% on 15mg stopped treatment because of side effects, compared with 2.6% on placebo.2

The takeaway is balanced: nausea is common, most people manage it, and a small minority find it intolerable enough to stop. If you are in that minority, that is a reason to talk to your prescriber early, not to suffer silently.

06 · How long it lasts

How long does nausea last on 5mg?

For most people, nausea on 5mg eases within days to a few weeks. Side effects follow a pattern: they appear in the first days after starting or increasing a dose, peak early and settle as your body adapts.1

In practice, that often looks like a rhythm across each week. You may feel worst in the days after the injection and better as the next dose approaches. Over several weeks on the same dose, that wave usually flattens. Keeping your injection on the same day each week helps you see the pattern clearly and plan meals around the rougher days.

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 Pick what suits your routine and stay consistent.

If you want a longer view of how symptoms usually unfold, our Mounjaro nausea week by week guide and the page on how long Mounjaro nausea lasts go into the timeline in more detail.

If nausea is getting steadily worse rather than easing, speak to your clinical team.

07 · Eating to ease nausea

What to eat and how to eat on 5mg

The practical steps that help most people are to eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated, and avoid lying down straight after eating.1

Smaller portions, eaten slowly. Your stomach is emptying more slowly, so a plate that felt normal before can now feel like too much. Serve less than you think you need and add more only if you are still hungry later.

Stop at the first feeling of fullness. This is the habit that matters most. Eating past that point can make nausea and reflux more likely. Fullness can arrive sooner than you expect, partway through a meal.

Plain, lower-fat foods. While you adjust, simple foods such as toast, rice, plain crackers, soups and lean protein tend to sit more comfortably than greasy, fried, spicy or very rich dishes, which take longer to digest.

Eat little and often if that suits you. Some people feel better with several small snacks than with large meals. Others prefer regular meal times. Notice which pattern leaves you least queasy.

Protein and balance. Eating less overall makes it easy to under-eat protein. Even when appetite is low, keep meals balanced.

Many people find a short note of what preceded a bad spell helpful. Our Mounjaro nausea tips page has more ideas for eating well while you adjust.

08 · Fluids and position

Hydration, posture and what stops nausea quickly

Staying hydrated and upright are two of the simplest steps. Stay well hydrated, and avoid lying down straight after eating.1

Is it better to lie down when nauseous? After a meal, no. Lying flat soon after eating can make queasiness and reflux worse, so sit or stand upright and give your stomach time. Resting in a comfortable, slightly raised position away from strong smells is fine if you simply feel unwell, but avoid lying flat straight after food.

Sip rather than gulp. Large volumes of fluid taken fast can make you feel sicker. Small, frequent sips of water through the day are easier to tolerate. Many people also find that drinking between meals, rather than with a big meal, leaves less pressure on a slow-emptying stomach.

What stops nausea immediately? There is no instant fix with this medicine. Fresh air, sitting upright, slow sips of cool water and putting the meal down all help in the moment. The longer-term fixes are portion size, food choice and time.

Many people ask about remedies they can buy. Speak to a pharmacist or your clinical team, who can suggest something suitable for you, especially if you take other medicines.

If vomiting means you cannot keep fluids down, do not try to ride it out at home. Contact your clinical team, as dehydration needs prompt attention.

09 · Cramps, vomiting and bowels

Stomach cramps, vomiting, diarrhoea and constipation on 5mg

Nausea rarely travels alone, and the other digestive effects are covered by the same advice. Abdominal discomfort, vomiting, diarrhoea and constipation are all in the group of most common effects.1

Can Mounjaro cause stomach cramps? Yes. Abdominal discomfort is a very common effect, and mild cramping can come with the same slower digestion that causes nausea. Smaller, plainer meals and steady fluids usually help. The distinction that matters is severity. Severe, persistent stomach pain that may spread to your back, with or without vomiting, could be pancreatitis: stop using Mounjaro and get medical help straight away.113

Vomiting. Occasional vomiting around a dose change is common and tends to improve as the dose settles. Repeated vomiting, or vomiting that stops you drinking, needs a call to your clinical team.

Diarrhoea and constipation. Both occur, sometimes in the same person at different points. Constipation happens because food moves through the gut more slowly and many people eat and drink less. Regular fluids come first, then fibre from vegetables and whole grains, and daily movement. If it persists, a pharmacist can recommend a suitable laxative.1

Because several digestive effects can overlap, a small diary of meals, symptoms and injection day is worth keeping for a few weeks. It makes the conversation with your prescriber far more useful, and many people use a free tracking app, such as our GLPTrackr companion app, for exactly this.

10 · Holding the dose

Should you stay on 5mg longer or step up?

If 5mg is hard to tolerate, you do not have to move up on schedule. If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. Speak to your clinical team rather than pushing through severe symptoms.1

This is built into how the medicine is licensed. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Those intervals are minimums, not targets.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better, so your prescriber will guide the right pace.1 For some people 5mg is where they stay, particularly if it controls appetite well and the side effects have settled.

Do not adjust the dose on your own or take doses closer together. Dose changes should go through a prescriber. If you are thinking ahead, our page on nausea at the 7.5mg step explains what the next increase can feel like.

Going slower is a legitimate clinical decision, not a failure.

11 · When to get help

When nausea needs medical attention

If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team: dehydration needs prompt attention.1

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for:

  • severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis);
  • pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems);
  • signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.113

You can also report any suspected side effect through the MHRA Yellow Card scheme.19

Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber, and used alongside a reduced-calorie diet and increased physical activity. If you want to compare approaches to easing symptoms, see how nausea relief options compare, or our guide to helping Mounjaro nausea.

12 · Injecting and storing well

Does how you inject affect nausea?

The injection routine does not usually cause nausea, but consistency helps you read your symptoms. Mounjaro is injected under the skin once a week. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

Stick to the same day each week. If you ever need to move it, you can change your weekly injection day as long as at least 3 days (72 hours) have passed since your last dose.1 Do not double up to make up for a rough week.

Store pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 Follow the pen's own instructions when injecting, and keep the cap on to protect it from light.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
Is nausea normal on Mounjaro 5mg?
Yes. Nausea is among the most common Mounjaro side effects, and the step-up from 2.5mg to 5mg is a typical time for it to appear or return. In the weight-management trials, nausea was reported by 24.6% of people on 5mg, compared with 8.5% on placebo. For most people it eases within days to a few weeks.
How long does nausea last after increasing to 5mg?
For most people, nausea eases within days to a few weeks. Side effects usually appear in the first days after a dose increase, peak early and settle as your body adapts. It often feels worst shortly after the weekly injection and improves as the week goes on. If it is not improving, speak to your clinical team.
How can I treat Mounjaro nausea?
Eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods, stay well hydrated and avoid lying down straight after eating. If these are not enough, ask a pharmacist or your clinical team what is suitable for you. If you cannot keep fluids down, get medical help straight away.
Can Mounjaro cause stomach cramps?
Yes, abdominal discomfort is one of the most common effects, linked to slower digestion. Mild cramping often improves with smaller, plainer meals and regular fluids. However, severe, persistent stomach pain that may spread to your back, with or without vomiting, could signal pancreatitis and needs immediate medical help.
What to expect when taking Mounjaro 5mg?
Expect to feel full sooner and less hungry between meals, and possibly some nausea, diarrhoea or constipation in the days after the dose increase. These effects usually settle as your body adjusts. Many people settle at 5mg as a maintenance dose, but your prescriber will guide the right pace, and results vary between individuals.
Is it better to lie down when nauseous?
Not straight after eating. Guidance is to avoid lying down straight after meals, as lying flat can worsen queasiness and reflux. Sitting or standing upright, taking slow sips of water and getting some fresh air tend to feel better. If nausea is persistent or you cannot keep fluids down, get medical help straight away.
What stops nausea immediately?
Nothing works instantly for Mounjaro nausea. In the moment, sit upright, sip cool water slowly and stop eating if you feel full. Longer term, smaller plainer meals and time on the dose help. A pharmacist or your clinical team can advise on anything further that suits you.
Will the nausea from Ozempic go away?
Ozempic is semaglutide licensed for type 2 diabetes only; it is not a UK weight-loss medicine. With Mounjaro, side effects most often ease within days to a few weeks, as the body adjusts, and nausea becomes less frequent over time in trials. Anyone on another GLP-1 medicine should ask their own prescriber about what to expect.
Should I stay on 5mg if the nausea is bad?
If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. The licensed intervals are minimums, not targets. Speak to your clinical team rather than pushing through severe symptoms, and never change your dose or injection timing on your own.
