This phase is about adjustment rather than results, and experiences differ widely between people.¹ Some people notice an appetite change and early loss in week one. Others feel almost nothing until a higher dose. Neither is a sign that the medicine is working better or worse for you.

Week one. You inject once, on the same day each week. In the first days after the injection, mild nausea, altered appetite and tiredness are the commonest early effects, and they usually settle.¹ Some people already feel fuller sooner and less hungry between meals. Any loss in this first week includes water weight, so it can look more dramatic or less dramatic than the true trend.

Weeks two and three. Appetite changes may become more noticeable, or they may stay subtle. This is where many people start to compare themselves with online stories. It is worth resisting that, because the stories that get shared are the extremes. A quiet start is as normal as a fast one.

Week four. You reach the end of the first pen. This is a natural point to talk to your prescriber at your monthly review about how you have felt, whether side effects have settled, and whether a step up in dose is appropriate. Whether and when you move up is a prescriber decision, not something to do on your own.

It also helps to know what a month is not. It is not a test you pass or fail. Trial results were measured at 72 weeks, and the trial participants took tirzepatide alongside diet and exercise, not instead of them.² A single month tells you how your body tolerates the medicine and whether your appetite shifts, which is useful, but it is a poor guide to where you will end up.

05 · Why trial figures differ

Why trial headline figures do not describe month one

The big percentages quoted for Mounjaro come from trials lasting 72 weeks, so they do not describe a single month. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages, and individual results vary.

To make the scale concrete, take someone starting at 100 kg. A 20.9% loss is about 21 kg, but that was the average at the 15mg dose after 72 weeks, not after one month. Most people also do not start at 15mg. The dose ladder begins at 2.5mg for exactly the reason that the body needs time to adapt.

The trial also reported how many people reached certain thresholds at 72 weeks. 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.² Again, these are long-term figures. They show what is possible over a full course on the right dose, not what a first pen delivers.

So what should you expect at the three-month mark, the next natural checkpoint? Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. In SURMOUNT-1, average weight loss built month by month, with the full averages measured at 72 weeks. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.

The more useful question is whether you are tolerating the medicine, whether your appetite is changing, and whether your trend is heading the right way over several weeks.

06 · How it works

How Mounjaro works, and why appetite changes first

Mounjaro works by activating two natural gut-hormone receptors at once, which is why appetite is often the first thing to change. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

Tirzepatide is the first treatment of its kind to activate both GIP and GLP-1 receptors. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹ This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).

Slower stomach emptying also explains some of the early effects. Feeling full for longer is useful for eating less, and side effects tend to be most noticeable in the first days after an injection and then ease.

There is one practical point linked to this mechanism that matters from month one. Because the medicine slows stomach emptying, it can reduce absorption of oral contraceptives. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ If you use an oral contraceptive, discuss this with your prescriber at your consultation, and see the MHRA guidance on contraception with GLP-1 and GIP medicines.⁷

Why does this matter for expectations? It changes how hungry you feel and how quickly you feel full, and you still need to use that change. Treatment is used alongside a reduced-calorie diet and increased physical activity, not instead of them.²

Finally, a note on how long the medicine stays in your system. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear. There is no way to speed this up, and nothing needs doing while it clears.¹

07 · Early side effects

Side effects in the first month and what is normal

The commonest side effects in the first month are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.¹ Most people find the pattern repeats weekly for a short while and then eases as the body adapts to the 2.5mg dose.

It is worth separating three things: what is expected, what is worth mentioning, and what needs prompt attention.

Expected and usually manageable. Mild nausea, a changed appetite and some tiredness around injection day. Eating smaller meals and stopping when you feel full fits with how the medicine works, since it helps you feel fuller sooner. Worth mentioning at your monthly review. Effects that are more bothersome than mild, or that do not settle, are worth raising with the prescribing team. Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight. If your first month looks flat, or your second month stalls after a good start, that does not mean treatment has failed.

People often ask how to speed things up. Within the licence there are sensible responses, and they are all about the basics rather than shortcuts:

• Review food intake honestly. Portions drift back up over time, even when appetite is lower.
• Protect protein and activity. Treatment works alongside a reduced-calorie diet and increased physical activity.²
• Check the basics. Sleep and alcohol both affect appetite and progress.
• Ask about your dose. Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.

What a plateau is not is a reason to double doses or add unlicensed products. If a dramatic claim promises quick results, treat it with caution.

One detail matters for expectations: in the first month you are on the lowest dose by design. Asking how to speed up weight loss in month one often really means asking when the dose will rise. That is a prescriber decision, made at your monthly review once you are tolerating the current dose.

If you ask whether you can stay on 2.5mg for longer than four weeks, the answer is that your prescriber decides, based on how you are tolerating treatment and responding. Raise it at your review.

There is also a formal checkpoint. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.¹ That is a long way past month one, which is another reason not to judge treatment by its first four weeks.

09 · Who can start

Who can start Mounjaro, and NHS versus private

Mounjaro is licensed for adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹¹¹ A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.

This matters for month-one results because the decision to start is clinical, not just a purchase. Whether you are suitable, which dose is right, and how you will be monitored are all part of the consultation. On the NHS side, Mounjaro is available, but access is being phased in over several years. After month one the next steps are your monthly review, a decision on your dose, and a focus on trend rather than single weigh-ins. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.² These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.

If the scales barely move, other signs may still tell you something: how full you feel after meals, how often you think about food, how your clothes fit, or how your energy is. 11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
