A low dose GLP-1 is the starting step of a licensed dose ladder, not a separate treatment. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1

The words "low dose" can mean several different things, and they carry very different levels of safety.

02 · Your decision

Do you need a lower dose than the licence gives you?

Most people do not need to invent a lower dose, because the licence already starts low and moves slowly. After at least 4 weeks the Mounjaro dose increases to 5mg, and can then step up by 2.5mg at minimum four-week intervals up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. 03 · Licensed dose ladder

The licensed ladder already starts low

The lowest Mounjaro step is 2.5mg once weekly, and it is designed as an adjustment phase. The first weeks are not expected to produce your biggest results, and that is by design rather than a sign the medicine is failing.1

Mounjaro comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Each KwikPen contains four weekly doses.1

Wegovy follows a similar approach with its own ladder: 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 The 2.4mg dose is the licensed maintenance dose for weight management.

04 · Why microdosing is unlicensed

Why microdosing is not something we support

Microdosing is not licensed, not supported by the pen's design and not endorsed by any UK regulator. It means taking smaller or more spread-out doses than the pen is designed to deliver.1

The licensed ladder already starts low for tolerability, and your prescriber can hold a dose longer or step down within the licence if side effects are the concern. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

The pen itself explains the practical problem. You set a Mounjaro KwikPen by turning the dose knob until the dose indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks. The pen will not let you dial past the medicine left inside.1

If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.1 In other words, the device is built to deliver a full labelled dose, not a fraction of one.

05 · Inflammation and menopause

Low dose GLP-1 for inflammation or menopause

Mounjaro is licensed for weight management in adults and for type 2 diabetes, so inflammation and menopause symptoms are not licensed uses.1 We do not supply it for those purposes, and we will not suggest a dose for them.

The same caution applies to menopause. If you are struggling with menopausal symptoms, your GP or a menopause specialist can discuss licensed options with you. If you are also living with excess weight, a licensed GLP-1 prescribed for weight management is a separate conversation with your prescriber.

If you notice a new symptom while on any GLP-1 medicine, discuss it with your prescriber and report it through the Yellow Card scheme.19 A symptom that is not on the leaflet list is not a listed side effect, so a clear report matters.

06 · Low dose pills

Low dose GLP-1 pills in the UK

There is no tablet form of Mounjaro, because tirzepatide is injection-only in the UK. Two GLP-1 weight-loss tablets are now licensed, and they are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results. Both are prescription-only.1

Wegovy tablets contain semaglutide, the same active ingredient as the Wegovy injection, taken as one tablet once a day. Treatment starts at 1.5mg once daily and steps up to 4mg, 9mg and then the 25mg maintenance dose, with at least one month at each dose; the dose can stay at an earlier step if needed.20

The tablet must be taken on an empty stomach after fasting for at least 8 hours, swallowed whole with a sip of water (up to half a glass, about 120ml), and you then wait at least 30 minutes before eating, drinking or taking other tablets.20

Foundayo contains orforglipron, a once-daily GLP-1 tablet made by Eli Lilly, the maker of Mounjaro. Treatment starts at 0.8mg once daily, then steps up to 2.5mg and 5.5mg, and then to 9mg, 14.5mg or 17.2mg depending on response and tolerability, with at least 30 days at each dose; 17.2mg once daily is the maximum.21

Unlike Wegovy tablets, Foundayo can be taken at any time of day, with or without food and without water restrictions. It may also make oral contraceptive pills less effective: switch to a non-oral method or add a barrier method for 30 days after starting and for 30 days after each dose increase.21 With Mounjaro, the pill can be absorbed less well because it slows stomach emptying: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after starting and for 4 weeks after each dose increase.

For results, the OASIS 4 trial of Wegovy tablets and the ATTAIN-1 trial of orforglipron each have their own figures.2223 No trial has compared Mounjaro directly with either tablet, so those results should not be set against each other as if they were head to head. Individual results vary.

The pills sold online as "natural GLP-1", such as berberine, are not medicines, are not regulated for this use and have no comparable trial evidence. 07 · Benefits of starting low

The real benefits of starting low

The benefit of a low starting dose is tolerability, and the licence builds that in for everyone. The first weeks on 2.5mg are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose. Both are normal.1

Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1

A gradual ladder also gives your prescriber time to judge how you respond. Higher is not automatically better, and some people do well at a middle maintenance dose. That is a choice made with your prescriber, based on how you respond and tolerate each step.

Being realistic matters too. Weight loss on Mounjaro is gradual by design, and most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days.1 If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

08 · How to get a low dose

How to get a low dose GLP-1 safely

The safe route is a prescription, and the first dose you receive will be the licensed starting dose. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity.1

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46

09 · Storage and handling

Keeping a low dose pen safe

Whatever dose you start on, storage rules are the same. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1

Keep the pen cap on to protect it from light, and never use a pen that has been frozen. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.1

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Rotate the site with each dose.1 Needles, a sharps bin and injection guidance are included with every order.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. 10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 If you are pregnant or planning a pregnancy, do not use Mounjaro: it must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
