People often ask this question because they have seen injection advice for other medicines, such as vaccines, which are commonly given into muscle. Those instructions do not apply here. 03 · Subcutaneous vs intramuscular
Subcutaneous versus intramuscular injections
A subcutaneous injection delivers medicine into the layer of tissue just beneath the skin. An intramuscular injection goes deeper, into the muscle itself. Mounjaro belongs in the first category: it is injected under the skin (subcutaneously).1 That is how the pen was designed, how the medicine was studied for licensing, and how the patient leaflet tells you to use it.13
Your pen comes with its own guidance and your pharmacy can walk you through it.
It also helps to understand why route matters at all. Medicines are developed and tested by a particular route, at particular doses, with a particular device. The licensed information for Mounjaro describes the dose ladder, the weekly schedule and the sites together as one package. Moving away from the route changes the assumptions behind that package.
Is there any situation where an intramuscular route would be used on purpose? Not for Mounjaro. Neither your prescriber nor our team would ever ask you to inject it into muscle. If a seller, forum post or social media account suggests it, treat that as a warning sign rather than a tip.

04 · Licensed injection sites
Where to inject Mounjaro
There are three licensed injection sites: the stomach area, the front of the thigh, and the back of the upper arm if someone else gives the injection.1 For the stomach, stay at least 5cm from the belly button. Rotate the site with each dose.
Many people find the stomach area easy to see and reach. Many people alternate sides week to week so that no patch of skin is used twice in a row. The front of the thigh is a good alternative, particularly if your stomach feels tender or you want to give the abdomen a rest. The back of the upper arm is the awkward one: it is licensed only when another person gives the injection, because it is hard to reach safely yourself.
Keep a simple pattern, such as moving between sides and between sites in a fixed order. The free GLPTrackr companion app lets you log each dose, which makes it easier to remember where you injected last time.
What about other areas people ask about, such as the hips or so-called love handles? They are not listed as licensed sites in the product information. Likewise, the buttock is a commonly cited intramuscular site for other medicines but is not a licensed Mounjaro site, so it does not belong in your rotation.
Practical preparation is straightforward. Wash your hands, choose the site, make sure the skin is clean and dry, and set the dose using the pen's window. Then follow the pen's instructions for the injection itself. Keep the cap on between uses, because the cap protects the pen from light.1
If you notice persistent soreness, lumps, redness or anything else at a site, contact your clinical team if a reaction is spreading, hot, very painful, or lasts more than a week, and report suspected side effects through the Yellow Card scheme.19 Do not self-diagnose from online photos. A prescriber can look at what you describe and advise on whether it is a normal reaction or needs attention.
Finally, injections are once a week at any time of day, with or without food, so you can fit the site choice around your routine. Some people prefer the stomach in the evening on a quiet day; others prefer the thigh seated at a desk. Either is fine within the licence.
05 · The pen and the dose window
How the KwikPen works
The pen is pre-filled and multi-dose: you do not draw anything up, measure anything, or choose a needle length from a syringe drawer. The device handles the dose; you handle the site and the timing. This is also why syringe-based guidance for intramuscular injections is the wrong frame of reference.
The pen strengths are 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Everyone starts on 2.5mg. A pen of any strength contains four weekly doses of that strength, so a month's supply is one pen.
You also do not need to count clicks. The number in the window is the only confirmation that matters.

06 · If you injected differently
What to do if you think you injected incorrectly
If you think you injected differently from the instructions, contact your prescriber or pharmacy, do not take an extra dose to make up for it, and report any unexpected symptoms through the Yellow Card scheme.19
First, stay calm and note the details: which site, what day and time, and anything unusual about the injection. Second, watch for symptoms in the following days and keep them in mind for your next review. Third, get in touch with a professional rather than relying on forums. Your prescribing team can ask the right questions and, if needed, direct you to urgent care. If you feel seriously unwell, seek medical help straight away in the usual way.
Fourth, do not try to correct the situation by injecting again. The missed dose rule shows how carefully the weekly schedule is protected. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1 A worry about technique is not a reason to double up.
Fifth, next week, go back to the plan. Use a licensed site, rotate it, set the dose by the number in the window, and follow the pen's instructions. If you are uncertain about the angle, see the correct injection angle.
Reporting is worth doing even when the effect seems minor. The Yellow Card scheme, run by the MHRA, collects reports of suspected side effects and is how regulators learn about real-world safety.19 You do not need to be sure the medicine caused the symptom; a suspicion is enough.

07 · Dosing and your weekly routine
Dose steps and your weekly routine
Everyone starts Mounjaro on 2.5mg once a week, and the starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.
Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace. Whatever strength you are on, it is the same subcutaneous injection, the same sites and the same weekly rhythm.
There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off.1
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 This is useful for holidays, shift changes or simply finding a day that suits you better. Make the change once, note it in the GLPTrackr app, and then keep the new day.
The first weeks deserve patience. The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1
Prescriber review is built into the process. 08 · Storage and travel
Storing and travelling with your pen
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them.1 Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.
The rules are short and worth memorising: fridge until first use, then up to 30 days not above 30°C once you have started the pen.
For unopened pens, the position is equally simple. Store them in the fridge until first use and they keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing. Ideally, use a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1

09 · How Mounjaro works and what to expect
How Mounjaro works and what results look like
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.
Results are gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. For someone starting at 100 kg, 20.9% is about 21 kg.

10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Getting Mounjaro: NHS and private routes
Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
