01 · The honest verdict
Is one month of Mounjaro worth it?
One month is worth it only if your goal is to find out how you tolerate the medicine; it is not worth it if you expect a meaningful result. Mounjaro is a once-weekly injection, and the first weeks on 2.5mg are an adjustment phase where losses are usually modest.2
Experiences differ widely. Some people notice an appetite change and early loss in week one; others feel little until a higher dose, and both are normal.13 Early loss also includes water weight, so a single weigh-in tells you very little.
The 2.5mg pen is a starter dose. 03 · Deciding
When a one-month trial makes sense, and when it does not
A one-month start makes sense if you want to see how you tolerate the medicine before committing to a longer plan. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 A month gives you a real first impression of those.
It does not make sense if your reason is a short-term target such as a holiday or event. 04 · First month expectations
What to expect in your first month
Your first month is the 2.5mg adjustment phase, and the scale may move only a little.
In SURMOUNT-1, a trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2 For someone starting at 100 kg, 20.9% is about 21 kg, but that figure belongs to 72 weeks on the 15mg dose, not to one month.
Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties. In practice, most people feel fuller sooner and less hungry between meals.1 That appetite effect is what you are testing in month one.

05 · How quickly it works
How quickly does Mounjaro work?
Mounjaro works gradually by design, and the pace builds over months rather than days. Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at that stage are modest compared with the headline figures.
In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.
Your prescriber reviews response at set points. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.11
Beyond the scales, weight loss on tirzepatide in trials came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises, and they too take time to appear.2
06 · Stopping after one month
Can you stop Mounjaro after one month?
You can stop at any time, and your prescriber is the right person to talk to first. Nothing needs doing to clear the medicine: tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system, and there is no way to speed this up.1
That clearance time is why one month of treatment does not leave lasting change on its own. The appetite effect fades as the medicine leaves, and the habits you built in the meantime are what carry forward.
If you want to come off without losing ground, the supportable steps are the ones that apply on treatment too: keep to a reduced-calorie diet, keep your activity up, protect protein, and track your trend. Speak to your prescriber before you stop rather than after, so the plan is agreed in advance.
If you have a gap in treatment, such as a three-week break, do not guess at restarting. Check the patient information leaflet and contact your prescriber or pharmacy before your next injection, because the right restart dose depends on your situation.13 If you notice any new symptom, tell your prescriber and report it through the Yellow Card scheme.19 Serious side effects are uncommon but need urgent attention. Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
07 · Eligibility and who should not
Who should not take Mounjaro?
Mounjaro is only suitable for some people, and a prescriber decides that after reviewing your history. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation. It is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1
It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable, so someone outside these criteria, or with a history that makes it unsuitable, will not be prescribed it.10 Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
On the NHS, Mounjaro is available but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
08 · Plateaus and add-ons
Plateaus, add-ons and what not to do
A plateau of a few weeks is normal and expected, because weight loss on Mounjaro is not linear.1 A flat month is not a reason to give up or to speed things up.
Within the licence, the responses are to review food intake honestly, since portions drift back up, to protect protein and activity, to check the basics such as sleep and alcohol, and to discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.
