Neither dose is better in general; each suits a different point in your treatment. Everyone starts on 2.5mg, moves to 5mg after at least 4 weeks, and can then step up by 2.5mg at a minimum of 4-week intervals, up to a maximum of 15mg, depending on response and tolerance.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.11

03 · How to decide

The factors that should decide it

Four things should guide the decision: how hungry you are between injections, the trend in your weight, how you tolerate side effects, and how long you have been on your current dose. A single weigh-in is not a reason to change dose.

If appetite is controlled and you are losing steadily, staying put is a legitimate plan. If side effects are troublesome, a higher dose is the wrong move and your prescriber can hold your dose longer or step down within the licence.1

04 · How the dose ladder works

How the Mounjaro dose ladder works

The dose ladder is a planned escalation, and 7.5mg and 5mg are simply two rungs on it. Mounjaro (tirzepatide) is a once-weekly injection made by Eli Lilly, supplied in a KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Everyone starts on 2.5mg once a week. That starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg. From there it can step up by 2.5mg at a minimum of 4-week intervals, passing through 7.5mg, 10mg and 12.5mg, to a maximum of 15mg weekly.1

Notice the wording: a minimum of four weeks. It is not a target. The licence sets the earliest point at which you may step up, and your prescriber decides the actual pace based on how you respond and tolerate treatment.

Why the ladder is gradual

The steps exist so your body can adjust. Each pen holds four weekly doses, so a typical pace gives you one full pen at each strength before any change.1 Skipping rungs is not part of the licensed approach.

Maintenance doses

Many people settle at 5mg, 10mg or 15mg as a maintenance dose.11 That is worth stating plainly: stopping at 5mg is not a failure or an incomplete course. It can be the correct dose for you.

Doses also scale in a simple way. A larger amount does not mean a proportionally larger result.

The ladder is also not a one-way street. If side effects are the concern, your prescriber can hold a dose longer or step down within the licence.1 What you should not do is alter doses yourself, split pens or stretch them; that sits outside the licence.

05 · What the trial evidence shows

What the trial evidence says about dose and weight loss

Higher doses produced higher average weight loss in the main trial, but individual results vary widely. In SURMOUNT-1, which included 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

Those are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Beyond the average

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 So the large majority of people on 5mg reached that threshold. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

To make this concrete: for someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg. That is straight arithmetic on trial averages, not a forecast for you.

Trial averages do not predict your own result

Two people on the same dose can lose very different amounts. Appetite, diet, activity, sleep, medication and tolerance all play a part. A dose that works well for one person is not a template for another.

Other medicines

Dose is not the only comparison people make. In the head-to-head SURMOUNT-5 trial, adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).15 Both are effective, licensed treatments; which suits you depends on your health profile, tolerance and prescriber advice. It is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply Ozempic for weight loss.

06 · When to go from 5mg to 7.5mg

When to go from 5mg to 7.5mg

The earliest you can move is after at least four weeks on 5mg, and whether you should depends on your response rather than the calendar.1 A sensible conversation with your prescriber usually covers the following.

  • Appetite: Is hunger returning well before your next injection, or is food noise as quiet as it was?
  • Trend: Is your weight moving in the right direction over several weeks, judged by trend, not a single weigh-in?
  • Tolerance: Are side effects settled, or still troublesome?
  • Time on dose: Have you completed at least four weeks, ideally a full pen?

Reasons to stay on 5mg

If your appetite is controlled and weight is trending down, you may not need to move. Many people settle at 5mg as their maintenance dose.11 The licence allows escalation depending on response and tolerance; it does not require it.

Reasons to wait

If side effects are still bothering you, a bigger dose is rarely the answer. Your prescriber can hold your current dose longer. Equally, if you have only just reached 5mg, remember that most measurable progress builds from 5mg onward, month by month.

The review rule

Your response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Note the wording: highest tolerated dose, not highest available dose.

What not to do

Do not move up early because you feel impatient, and do not take extra doses. Taking more or spreading doses differently from the pen's design is not licensed and not endorsed by any UK regulator.1 If you want a change, ask your prescriber.

07 · What to expect on 7.5mg

What to expect from Mounjaro 7.5mg

Expect a gradual continuation of progress, not a sudden change. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase, and most measurable progress builds from 5mg onward, month by month.1 By the time you reach 7.5mg you are already well into that phase.

Appetite

Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. Your response to a step up varies by person.

Side effects after a step up

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 A new dose can bring them back for a short time. Many people choose a weekend morning to inject so early side effects fall on days off.

Judge progress by trend, not individual weigh-ins. Early loss includes water weight, so the pattern across weeks matters more than any single number.1 Trials measured results at 72 weeks, so think in months, not days.

There is no fixed time. The licence sets a minimum of four weeks between steps, not a maximum.1 Some people stay on 7.5mg for months because it works; others move on after a pen or two. Your prescriber guides the pace.

Practical routine

Why am I not losing weight on Mounjaro 7.5mg?

The first thing to check is whether the trend is truly flat, because day-to-day weight moves for reasons unrelated to fat. Early loss includes water weight, and the trend over weeks matters more than any single weigh-in.1

Questions worth asking

  • Is it a trend or a blip? Look at several weeks, not one.
  • How long have you been on this dose? A few weeks into a new dose is early.
  • Is the medicine alongside diet and activity? Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
  • Are doses being taken on schedule? Missed or irregular injections change the picture.

Trial averages are only averages

In SURMOUNT-1, people on 5mg lost on average 15% over 72 weeks, and individual results vary.2 Some lose more, some less. Trial averages do not predict your own result.

What your prescriber looks at

If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That is a structured review, and it is a better route than guessing.

What not to try

Taking smaller or more spread-out doses than the pen is designed to deliver is not licensed, not supported by the pen's design and not endorsed by any UK regulator.1 It means unknown dosing accuracy and no safety data.

What is the difference between 7.5mg and 10mg?

The difference is one 2.5mg step on the ladder, and 10mg is the next rung after 7.5mg. The dose can step up by 2.5mg at a minimum of 4-week intervals, so the path runs 7.5mg, 10mg, 12.5mg and 15mg.1

What was measured

In SURMOUNT-1, the average loss was 19.5% on 10mg compared with 15% on 5mg over 72 weeks.2 Half of participants on 10mg lost at least 20% of their body weight.2

Why people stop at one dose

Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.11 The licensed question is not how high can I go, but what is the lowest dose that does the job for me with tolerable side effects.

Planning rather than chasing

A common mistake is to treat each step up as the solution to a plateau. A plateau on 7.5mg may reflect diet, sleep, activity, timing or simply normal fluctuation. Your prescriber will weigh these before agreeing to a step.

Costs, NHS access and how ordering works

At standard prices, a 5mg pen is £218.99 per month and a 7.5mg pen is £279.99 per month; you can check both on the Mounjaro pen page. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year (52 weeks, 13 pens) costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg. A new customer's first order is £40 less, and every later order is £25 less.

NHS or private

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. 11 · Injecting, missed doses and storage

Injecting, missed doses and storage at 5mg and 7.5mg

The routine is the same at every dose: one injection under the skin, once a week, on the same day.1 Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.

Timing

There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week, so pick a day and time you will remember.

Missed a dose?

Take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.13

Changing your day

You can change your injection day if at least 3 days (72 hours) have passed since your last dose.1

Using the pen

Turn the dose knob until the indicator shows the correct dose in the window; confirm by the number, never by counting clicks. The pen will not let you dial past the medicine left inside. If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.13 Follow the pen's instructions for the injection itself.

Storage and travel

Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Keep the cap on to protect it from light.1 You can fly with Mounjaro: carry pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription. For an unopened pen that has been out of the fridge, check with the pharmacy before using it.

Every two weeks?

Mounjaro is licensed once weekly. Stretching to a longer gap is not part of the licence, and the safe route if you struggle with side effects is to speak to your prescriber, who can hold or reduce the dose within the licence.1

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
