01 · The core answer
Should you syringe the last dose of Mounjaro?
No. Mounjaro is not meant to be drawn out of the pen with a syringe. The Mounjaro KwikPen is a sealed, pre-filled device designed to deliver its doses through the dial and needle, not through a syringe. Taking liquid out another way is outside the licence and outside what the pen was tested to do.
In plain terms: when the dial stops, the pen has done its job. A small amount of liquid usually remains in the cartridge, but that is not a spare dose you are owed. It is not a measured amount, and nobody can tell you how much active medicine a particular syringe draw would contain.

03 · If the dial sticks
What to do if the pen will not dial
If the pen will not turn to your dose, stop and treat it as finished or faulty rather than forcing it. Forcing the dial or taking the pen apart can damage it and risk the dose you do receive. 04 · Why not syringe
Why syringing leftover Mounjaro is not safe
The honest reason is simple: nobody has tested it. The pen is licensed and studied as a device that delivers set doses through its dial. Anything done outside that design means unknown dosing accuracy and no safety data.1
Several practical risks follow from this.
- Wrong amount. Without the dial, you cannot measure what you draw. You could take too little to have any effect or too much; if you use more Mounjaro than you should, talk to a doctor immediately.
- Contamination. Opening the pen or using an outside needle on the cartridge can introduce bacteria into a medicine you then inject under the skin.
- Damaged pens. Breaking a pen to reach the cartridge is unsafe and unlicensed: the leftover volume is not quality-assured, and manual extraction risks dosing errors and contamination.
None of these risks is worth a small amount of leftover liquid, particularly because your prescriber can help you plan supply in safer ways.
05 · What the leftover liquid is
What the liquid left in the pen actually is
The remaining liquid is not quality-assured and cannot be delivered as a full, accurate dose. It is not a hidden extra dose.
Think of it like the last bit of toothpaste in the tube. You can see it, but squeezing it out by cutting the tube open does not make it a measured portion, and in a medicine the stakes are higher. Online posts that talk about a so-called golden dose treat this residue as something to be harvested. We do not endorse this: the leftover volume is not quality-assured, and no regulated UK pharmacy or prescriber can endorse it.

06 · Doses, titration and timing
How doses, timing and your pen fit together
Knowing how the licensed schedule works makes it easier to see why the pen is built the way it is. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.111
Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Rotate the site with each dose. Each KwikPen contains four weekly doses.1
Because each pen covers a fixed run of weekly doses, the point at which the dial stops is predictable. Counting your injections on a calendar, or using a tracker, tells you well ahead when your pen is nearly done. 07 · Missed or late doses
Missed doses and changing your injection day
If your last pen runs out before your next arrives, the missed dose rule applies. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.113
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That flexibility is useful if a delivery arrives a little later than planned: you can usually realign your day rather than reach for any workaround.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
08 · Storage and the 30-day limit
Storage and the 30-day in-use limit
Storage matters for the pen you are using now and the next one waiting. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.113
After the in-use window, sterility and potency are no longer assured, and a pen that has passed it should go in the sharps bin. Unopened pens keep in the fridge until the expiry date printed on the box.
Travelling? You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.1

On the NHS, Mounjaro is available, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46
