01 · The short answer

What does prepping a Mounjaro pen involve?

Prepping a Mounjaro pen means getting a pen ready for the weekly injection: checking it is safe to use, fitting a new needle and setting the correct dose. The Mounjaro KwikPen is a pre-filled multi-dose pen. Each KwikPen contains four weekly doses.1 That design shapes everything about prep, because you are preparing the same pen again every week, not opening a fresh single-use device each time.

The exact order of steps, including anything to do with the needle and any flow check, is written in the patient information leaflet and the pen's instructions for use.13

If you are new to the pen, read the leaflet through once before your first injection day, with the pen in front of you. Then keep it somewhere you will see it each week.

02 · What to do next

Do you need to prime the pen every time?

Whether and when to prime is a leaflet question, not a guess. Follow the instructions for use that come with your pen exactly, and repeat them the same way each week.13

If the leaflet and something you have read online disagree, trust the leaflet. Online advice often comes from other countries' labels or from different devices. 03 · Before you start

What to do before the first Mounjaro injection

Before your first injection, read the leaflet, check you have everything, and choose your injection day. Everyone begins on the starting strength, 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 That context helps: you are not trying to achieve anything dramatic in the early weeks, only learn a calm routine.

A sensible checklist before the day:

  • Read the patient information leaflet in full, including the instructions for use.13
  • Check the pen is the strength your prescriber chose for you, and that the box and pen label match.
  • Check the expiry date printed on the box.
  • Make sure you have a new needle, a sharps bin and somewhere clean and well lit to sit.
  • Decide which injection site you will use, and which you will use next time.
  • Pick a day and time you will remember every week.

Do not rush the first time. Many people find the pen less intimidating once they have simply handled it, turned the dose knob with the needle off the table, and looked at the window. Treat the first session as a rehearsal with a real injection at the end.

04 · Checking the pen

How do you check a Mounjaro pen is fit to use?

You check the label, the expiry date, the condition of the pen and the medicine you can see through it, using the criteria in your leaflet.13 Two storage rules matter most for the pen itself. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Do not use a pen that has been above 30°C or has been frozen. If you suspect a pen was frozen, for instance it sat against the back of a fridge or in a cold bag, do not use it. Contact the pharmacy.

The same applies to a pen that is cracked, damaged or looks different from what your leaflet describes. If it does not look right, do not inject and ask. A replacement is a far smaller problem than an unreliable dose.

A few habits make checking easy:

  • Do the check in good light, with the cap still on, before you reach for a needle.
  • Look at the label on every pen, every week. Mix-ups happen when someone has more than one strength at home, for example after a dose change.
  • Keep the pen cap on between doses, because the cap protects it from light.
  • Keep a note of the date you began using each pen, so you can see when the in-use window is closing.

Should you keep your pen in the fridge after first use? It can go back in the fridge, which is the normal place for storage. Once in use, a pen can also be kept out of the fridge at a temperature not above 30°C for up to 30 days.

05 · Needle and dose

Attaching the needle and setting the dose

You attach a fresh needle for the injection and set the dose by turning the dose knob. The needle step is described in your leaflet, which also explains how to remove and dispose of the needle safely afterwards. Use a new needle each time and put used needles straight into the sharps bin supplied with your order.13

You set the dose by turning the dose knob until the correct dose shows in the window. The number in the window is your confirmation.

If the dose cannot be set in full, stop. Do not inject the small amount left and do not top up from another pen. Contact the pharmacy or prescriber for guidance on the next dose.

06 · Injection site and day

Choosing your injection site, day and time

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 Part of prepping is therefore deciding your routine, so prep becomes a habit rather than a decision.

The licensed sites are: the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 A simple way to rotate is to move between sites in a set order each week and write it down, or note it in the free GLPTrackr companion app if you already use it to follow your treatment.

What is the best time of day to inject Mounjaro? There is no "best" time of day — Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.1

If you ever need to move your day, you can: you can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

07 · Storage and travel

Storage, the 30-day rule and travelling

Pens are kept in the fridge until first use, and an in-use pen has a limited life out of it. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. That answers why you cannot use Mounjaro beyond 30 days, or above 30°C once a pen is in use: the licensed window ends, so the pen is discarded even if medicine appears to remain. Using the same pen for longer than that is outside what the label supports.

For unopened pens the guidance is simpler. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.

Can you travel with Mounjaro without refrigeration? For most trips, yes, within the in-use limit. You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch, and check destination rules for prescription injectables before you fly.

Prepping on holiday follows the same rules as at home: check the pen, use a new needle, confirm the dose in the window. The only new decision is where to keep the pen between doses.

08 · Common prep mistakes

What happens if you do not prep a pen properly?

Skipping or improvising prep steps can mean a dose that is not what was intended, and that is why the leaflet's order matters.13 The practical rule is: if you realise you missed or changed a step, do not try to correct it by injecting again. Stop and ask the pharmacy or your prescriber what to do.

The mistakes we would most like you to avoid:

  • Injecting without checking the dose in the window.
  • Using a frozen or damaged pen. A pen that has been frozen should never be used.
  • Keeping a pen past its in-use window. Throw it away after 30 days, or sooner if it has been above 30°C.
  • Sharing a pen. A pen is prescribed for one person. See our note on sharing a Mounjaro pen.
  • Taking smaller or spread-out doses.

On that last point, our position is clear. Altering doses outside the licence means unknown dosing accuracy and no safety data.

If a dose goes wrong in a way that worries you, talk to your prescriber. If you have severe, persistent stomach pain, chest pain, swelling of the face, lips, tongue or throat, or difficulty breathing, get medical help straight away. You can also report suspected side effects through the MHRA Yellow Card scheme.19

09 · Missed doses and pacing

What if you prep late or miss a dose?

If you miss your injection day, the rule depends on how late you are. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 Prepping the pen late is therefore fine within that window, and it is the same prep as always.

Sticking to the same day each week matters more than perfection on any single week. If your usual day stops working for you, you can move it, provided there are at least 3 days between two doses.1

Prepping carefully also supports the bigger picture. Weight loss on Mounjaro is gradual by design, and the first weeks on 2.5mg are an adjustment phase and losses are usually modest.1 You are building a routine that has to last months. 10 · When to ask for help

Who to ask when prep does not go to plan

Ask the pharmacy or prescriber whenever the pen, the needle or the dose does not behave as the leaflet describes. You do not need to work it out alone, and you should not experiment on your own body to find out.

Good reasons to contact us before injecting:

  • The pen looks damaged, or you think it may have been frozen.
  • The label does not match what you were expecting for your dose.
  • The dose knob will not turn as the leaflet describes.
  • You are unsure how long an unopened pen was out of the fridge.
  • You are unclear which step comes next in your instructions.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
