No, you should not try to half dose Mounjaro, and the pen gives you no reliable way to do it. The KwikPen is a multi-dose pen that is set to a single labelled strength. You turn the dose knob until the indicator shows the correct dose in the window.¹³ There is no marking for half of that dose, and nothing in the instructions describes a partial injection.

Some online threads suggest stopping an injection part way or counting clicks to reach a smaller amount. Do not try this: it is outside the licence and nobody can verify how much medicine you would actually receive. The pen's own guidance is clear on the point: you confirm the dose by the number in the window, never by counting clicks.¹³

It helps to separate the question from the reason behind it. Most people who ask about a half dose want one of two things: fewer side effects or a slower climb up the dose ladder. Each of those has a proper route through your prescriber, and none needs the pen to be modified or misused.

The Mounjaro KwikPen is a pre-filled multi-dose pen, and understanding that design explains why a half dose is not possible in practice. Mounjaro comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ Each pen is made for one strength only. A 5mg pen is a 5mg pen, and it is meant to be used for 5mg doses.

Each KwikPen contains four weekly doses.¹³ That arrangement is what makes the pen convenient: one pen covers a month of weekly injections at a set strength. It also means the medicine inside has been measured out to support exactly that number of full doses. Take less than the intended amount from each injection and you do not gain extra doses in any controlled way; you simply create an unpredictable pen.

People sometimes assume a multi-dose pen is like a vial and syringe, where you can draw up whatever volume you like. It is not. A vial and syringe lets you read a volume against a scale. The KwikPen has a dial that only offers the dose the pen was made to give, and the window shows that single number. There is no half-way graduation to read.

This is also the reason that pharmacies and prescribers do not offer half-strength supply. The licensed range already includes six strengths, with the lowest at 2.5mg. If you need a lower dose than the pen in your hand, the correct answer is a different pen, not a modified one.

It is worth remembering what the medicine does, too. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1, which together help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.¹ Because it works through these signals, the amount you receive each week matters, and an uncertain amount makes your response harder for you and your prescriber to read. Dial, window and clicks: what the pen really tells you

The number in the window is the only reliable measure of your dose, and the clicks are not. The Mounjaro KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window — you confirm the dose by the number in the window, never by counting clicks.¹³ That sentence is the main reason click-counting advice should be ignored.

Clicks are a mechanical by-product of turning the knob. They are not a calibrated measuring scale that you can safely reinterpret. When someone online says a certain number of clicks equals half a dose, they are describing something the manufacturer does not support, and they cannot know how your individual pen behaves. Even if a rule of thumb happened to work once, you would have no way of confirming that it worked again.

Another important design feature is that the pen will not let you dial past the medicine left inside. Each pen contains four weekly doses; the pen will not let you dial past the medicine left inside.¹³ This is a safety feature. It protects you from drawing more than the pen holds.

It also answers a common question from people near the end of a pen: what if the window will not reach a full dose? 07 · Microdosing and the law

Microdosing and altered doses: the safety position

Splitting doses sits inside a wider trend people call microdosing, and the safety position on it is straightforward. "Microdosing" — taking smaller or more spread-out doses than the pen is designed to deliver — is not licensed, not supported by the pen's design and not endorsed by any UK regulator.¹

The licensed dose ladder already starts low (2.5mg) for tolerability, and your prescriber can hold a dose longer or step down within the licence if side effects are the concern.¹ In other words, the lower-dose strategy that people hope to achieve by microdosing is already built in, with measured pens and clinical oversight.

Altering doses outside the licence means unknown dosing accuracy and no safety data.¹ It is worth dwelling on those two phrases. Unknown dosing accuracy means that you could receive more or less than you intend, and not know which. No safety data means that nobody has studied what happens when the medicine is used this way, so nobody can tell you how it will behave in your body.

There is a further practical issue: your prescriber makes decisions based on what you tell them about your dose. If you have been taking an unmeasured partial amount, any review of your tolerance or progress is built on a false picture. You could end up being stepped up on the basis of a dose you never really took, or told the medicine is not working when it was never given as intended.

Be careful about sellers and social media accounts that promote shortcuts. Offers to supply without a consultation or prescription, or videos promising dramatic weight-loss claims from tiny doses, should be treated with suspicion. Counterfeit and unregulated products are a real concern, and the MHRA publishes alerts about them.⁷ Genuine Mounjaro should come from a registered pharmacy; you can check ours on the GPhC register (registration 9012878).

08 · When side effects are the problem

When side effects are the problem: licensed ways to go gentler

If you want to halve your dose because you feel unwell, the licensed fix is to tell your prescriber, who has several options. The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal.¹ Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

The key phrase is "usually settle". Many people feel worst in the first days after an injection, then better as the week goes on. That pattern is a reason to be patient and to plan your injection day sensibly. There is no "best" time of day — Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.

If effects are more than mild, or do not ease, your prescriber can adjust the plan without anyone altering a pen. The dose can be held at its current level for longer before the next step. After at least 4 weeks the dose increases to 5mg, and further steps come at a minimum of 4-week intervals; that is a minimum, not a deadline.¹ Slower is allowed.

The prescriber can also step you back to a lower strength within the licence if a higher dose is not agreeing with you. This gives you a true lower dose from a correctly labelled pen, with accurate delivery. It is the right tool for the job that a half dose is trying, unreliably, to do.

Remember that higher is not automatically better. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.¹ A comfortable dose you can stay on is worth more than a high dose you cannot tolerate.

09 · Missed doses and timing

Missed doses, injection day and why a half dose is not a fix

A missed or badly timed injection has its own clear rules, so there is no need to split a dose to cope with it. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.¹

That last rule matters for anyone tempted to inject "half now, half later". Spreading a dose across two injections moves away from the licensed schedule of one full dose once a week. The label gives a simple pattern: one full dose, once a week, on the same day.

Life does sometimes require a change of routine. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹ Switching to a day that suits you better, for example one that places the first uncomfortable days on a weekend, is a licensed way to manage how the medicine fits around work and family.

Where you inject also matters, and rotation is part of a good routine. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.¹³

If you are ever unsure whether you injected a full dose, for instance if the pen was removed too soon or you were interrupted, do not top up. Contact your prescriber or the pharmacy for advice and do not take another injection within the minimum gap. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.

Storage, travel and leftover pens

Many half dose questions come from worry about waste, so it is worth being clear on how long a pen stays usable. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹³

Trying to make a pen last longer by splitting doses is not licensed, and a pen must be thrown away 30 days after first use, or sooner if it has been above 30°C.

Travel does not require special workarounds either. You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch.¹³ Check destination rules for travelling with prescription injectables before you fly.

For unopened pens, the rule is simple. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.¹³ That caution applies directly to US-based advice about shorter windows or pens that behave differently, because the UK product and its label are not the same.

11 · Realistic expectations

Realistic expectations: a lower dose is not a half-strength result

A smaller dose is not simply a smaller version of the same result, and the trial evidence is worth knowing before you decide. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.¹

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.²

Those figures come from people taking the labelled doses, delivered by pens made for them. They say nothing about what might happen with partial or unmeasured injections, because that was not studied. That is the central point of the safety position: altered doses mean no safety data and unknown accuracy.

The same trial reports that at 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.² Notice that a lower licensed dose such as 5mg still sits in the evidence base.

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.¹

Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins. 12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹

13 · NHS, private and cost

NHS, private treatment and the cost question

Cost is a common reason people ask about half doses. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.⁴
