01 · The short answer

Can you take 2.5mg out of a 5mg Mounjaro pen?

No. Each KwikPen contains four weekly doses of its stated strength, and the pen will not let you dial past the medicine left inside.13 Taking smaller or more spread-out doses than the pen is designed to deliver is called microdosing. It is not licensed, not supported by the pen's design and not endorsed by any UK regulator.1

The same applies whichever way round you ask: 2.5mg from a 7.5mg or 12.5mg pen, 5mg from a 7.5mg pen, or 7.5mg from a 5mg pen. The pen is set for the strength printed on it, and altering doses outside the licence means unknown dosing accuracy and no safety data.

02 · Safe options

What to do instead

Speak to your prescriber, because there are licensed ways to get a lower dose. The licensed ladder starts at 2.5mg, steps to 5mg after at least 4 weeks, and then moves up in 2.5mg steps at minimum 4-week intervals.1 Your prescriber can hold a dose longer or step you down within the licence if side effects are the concern.

First, complete a free online consultation. Second, your consultation is reviewed by a GPhC-registered prescriber; payment is taken when you place the order, and if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Third, we dispense and dispatch, with same-day dispatch when ordered by 3pm and free next-day tracked delivery. Needles, a sharps bin and injection guidance come with every order, and the six strengths are in stock.

04 · Why clicks don't work

Why counting clicks is not a safe method

The KwikPen is set by turning the dose knob until the indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks.13 No click count is part of the licensed instructions, so counting clicks is not a reliable way to set a dose.

05 · Syringes and the last drop

Extracting Mounjaro from the pen with a syringe is not a licensed way to use it. The pen is a sealed delivery device, and no instructions exist for converting its contents into millilitre or syringe measurements. Any such conversion would be guesswork, and dosing accuracy would be unknown.

The same goes for trying to get the last bit out of a pen. If the pen will not dial a full dose, it holds less than one dose and should not be used for a partial injection.

06 · Pen life and storage

The 30-day rule and storage

Keep pens in the fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.1 Keep the cap on to protect it from light.

A pen holds four weekly doses, so four weekly injections fit within that window. Using a pen beyond 30 days means sterility and potency are no longer assured.

07 · Dose ladder explained

How the licensed dose ladder works

Everyone starts on 2.5mg once a week. After at least 4 weeks the dose increases to 5mg, then steps up by 2.5mg at minimum 4-week intervals (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on response and tolerability.1 Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better.

08 · If side effects are the issue

If side effects are why you want a lower dose

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 If they do not, tell your prescriber rather than adjusting the pen yourself. They can hold your dose longer or step you down within the licence.

09 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
