A quick overview

What to know
before you start.

  • The 2.5mg starter weeks are for adjustment, not weight loss, so feeling little or feeling mild side effects are both normal.
  • Mild nausea, altered appetite and tiredness after each injection are the commonest early effects and usually settle.
  • Judge progress by the trend over months, not by single weigh-ins, because early loss includes water weight.
  • Report any symptom that worries you to your prescriber, and report side effects through the Yellow Card scheme.
In this guide

01 · The short answer

What actually helps you feel better on Mounjaro

The habits that help most are the unglamorous ones: a consistent weekly routine, a gradual dose increase and honest conversations with your prescriber. Mounjaro is a once-weekly injection of tirzepatide, which activates two gut-hormone receptors (GIP and GLP-1) that help regulate appetite and slow how quickly the stomach empties.1

  • Same day every week. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.1
  • Rotate the site. Use the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection, and rotate with each dose.13
  • Respect the dose ladder. Everyone starts on 2.5mg. The dose rises to 5mg after at least 4 weeks and then in 2.5mg steps at minimum 4-week intervals, depending on response and tolerance.1
  • Ask for a slower pace if you need one. Your prescriber can hold the current dose longer before stepping up.
  • Keep the diet and activity side going. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Your prescriber can tailor advice to your health profile.

02 · Comparison

Early weeks versus higher doses: what to expect

How you feel depends heavily on where you are on the dose ladder.

StageWhat it is forWhat is normalWhat to do
2.5mg (starter)Adjustment, not weight loss itself1Some notice appetite change and early loss in week one; others feel little. Both are normal.1Keep the routine; do not judge the medicine yet.
5mg onwardMost measurable progress builds from here, month by month4Mild nausea, altered appetite or tiredness can return for a few days after a step up.1Tell your prescriber if effects do not settle; ask about holding the dose.
10mg and 15mgMaintenance for those who need it; higher is not automatically better1SURMOUNT-1 averages: 19.5% on 10mg and 20.9% on 15mg over 72 weeks; individual results vary.2Settle at the dose you tolerate and respond to, guided by your prescriber.

For comparison, 5mg averaged 15% weight loss in SURMOUNT-1, against 3.1% with lifestyle changes alone.2 These are trial averages and do not predict your own result.

03 · The other meaning

Why some people feel better on Mounjaro

Some people feel better because appetite, fullness and eating patterns change. Tirzepatide helps regulate appetite, slows stomach emptying and improves how the body responds to insulin, and in practice most people feel fuller sooner and less hungry between meals.1 For someone who has struggled with constant hunger, that can feel like a real relief.

If you want to go deeper on this question, our guide on whether Mounjaro makes you feel better goes through what is and is not known, and our page on Mounjaro and sleeping better covers another common experience.

04 · Nausea and feeling sick

How to avoid feeling sick on Mounjaro

You cannot reliably avoid all nausea, but a gradual dose ladder is built to keep it manageable. Mild nausea in the first days after each injection is among the commonest early effects and usually settles.1 The starting dose of 2.5mg exists for tolerability, and the dose only rises after at least 4 weeks.1

What you can do within the licence is practical rather than dramatic. Inject on the day that suits you, so the first rough days land on a weekend. Rotate sites. Do not rush to the next dose if you are still unsettled, and ask your prescriber whether to stay on the current strength for longer.

If side effects are the concern, the safe route is a conversation with your prescriber.

If you want more detail on this symptom, see our guides on feeling sick on Mounjaro and feeling nauseous on Mounjaro. Any side effect, including ones not listed, can be reported through the MHRA Yellow Card scheme.19

05 · Fatigue and tiredness

Mounjaro tiredness: how long it lasts and what to do

Tiredness is one of the commonest early effects and is usually concentrated in the first days after each injection.1 Tell your prescriber if it is not settling.

Practical steps that can help:

  • Keep the injection on a fixed day so you can see whether tiredness follows a predictable weekly rhythm.
  • Choose a day that lets you rest afterwards, such as a weekend morning.1
  • Note whether tiredness clusters after a dose increase, and tell your prescriber, who can decide whether to hold the dose.
  • If tiredness is severe, constant or comes with other symptoms such as dizziness, speak to your prescriber rather than waiting it out.

Feeling dizzy deserves its own attention; our page on why you might feel dizzy on Mounjaro explains when to seek advice. Your prescriber or a registered dietitian can advise on your own diet.

06 · Injection routine

Getting the weekly routine right

A steady routine removes avoidable problems. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Each KwikPen contains four weekly doses.13

Three rules cover most real-life slip-ups:

  • Missed dose. Take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on as usual. If more than 4 days have passed, skip it and take the next one on your normal day. Never take two doses within 3 days of each other.1
  • Changing day. You can move your injection day as long as at least 3 days (72 hours) have passed since your last dose.1
  • Dialling the dose. Turn the dose knob until the correct dose shows in the window.

Store pens in the fridge at 2–8°C and never freeze them. Once in use, a pen can be kept not above 30°C for up to 30 days, then discarded.1 Getting these basics right helps you feel confident, and confidence is part of feeling better.

07 · Realistic pace

Is slow weight loss normal on Mounjaro?

Yes, gradual loss is by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.4 Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.

The trials report averages over 72 weeks rather than weekly rates, so there is no per-week target. The review point is this: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.

For context, in SURMOUNT-1, 85% of people on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight at 72 weeks.2 As a worked example, for someone starting at 100 kg, the 20.9% average on 15mg is about 21 kg. Individual results vary, and you should not treat that as a target.

Early loss includes water weight, so a big first week and a flat second week do not mean the medicine has stopped working.1

08 · Eating enough and bloating

Starvation mode, protein and bloating

Mounjaro slows how quickly the stomach empties and helps you feel fuller sooner, which supports a reduced-calorie diet rather than replacing the need for one.1

If you are eating very little, struggling to eat at all, or feel weak, tell your prescriber. They can review your dose and your diet in the context of your health.

The same slowed stomach emptying is why some people feel full or bloated. Our guide on why you may feel bloated on Mounjaro explains what is known and when to check in with a clinician.

09 · Getting support

When to contact your prescriber

Contact your prescriber whenever a side effect is persistent, worsening or simply worrying you. You do not need to wait for a scheduled review.

With us, ongoing monthly reviews, dose titration and follow-up assessments by the prescribing team are part of the service, as is side-effect support. Support is available Monday to Friday, 9am to 5pm, by email at support@everydaymeds.co.uk, through the account portal chat or on 0333 207 0413. Treatment is reviewed by a GPhC-registered prescriber, and repeat orders re-check suitability with no auto-shipping.

You can start your free consultation with no obligation, or compare all weight loss treatments.

Mounjaro (tirzepatide) is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary. This page is information, not a substitute for medical advice.

10 · If you stop

Feeling better after stopping

If you stop Mounjaro, tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Effects you noticed may therefore fade gradually, not overnight.

Our guide on how long after stopping Mounjaro you will feel better covers what to expect. Please speak to your prescriber before deciding to stop, particularly if side effects are the reason, because there may be a gentler option within the licence.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.
