Beyond appetite, the picture is less clear. Mounjaro is not a treatment for energy, mood, inflammation or heart health. Some people feel better, some feel unchanged, and some feel worse for a while. A prescriber who knows your history is the right person to talk to about that.

The most direct reason people feel better on Mounjaro is changed appetite. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve how the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals. That supports a reduced-calorie diet. Many people describe the change as less preoccupation with food. "Food noise" is a popular phrase for this, not a clinical term from the product information, so treat it as a description of an experience rather than a measured effect.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1 It does not tell you how you personally will feel on either one.

05 · Energy and tiredness

Can Mounjaro give you more energy, or make you exhausted?

People ask the question in both directions, and both are reasonable.

Some people feel lighter and more active as their weight falls. That is a plausible knock-on of weight loss rather than a proven direct effect of the medicine. Others feel tired, particularly while eating much less than usual. Eating a lot less is a recognised change that comes with this kind of treatment, and it is worth discussing with your prescriber rather than pushing through.

If you feel exhausted, work through the practical points first:

  • Are you eating enough across the day, given that your appetite is lower?
  • Are you drinking enough fluids?
  • Did the tiredness start after a dose change?
  • Do you have other symptoms, such as dizziness or feeling unwell?

Then tell your prescriber. Your monthly review is the right place to do it, and you do not need to wait for it if symptoms are bothering you. Can Mounjaro affect my mood?

Any big change in weight, appetite and routine can affect how you feel.

Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17 Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment. You can also report suspected side effects through the Yellow Card scheme.19

07 · Inflammation and heart

Inflammation and heart benefits

Does Mounjaro reduce inflammation? Reduced inflammation is not a listed side effect or benefit in the UK patient leaflet. Mounjaro is licensed in the UK for weight management in adults and for type 2 diabetes.1 It is not licensed as an anti-inflammatory or a heart treatment.

The same applies to heart benefits. If heart health matters to you, speak to your own clinician. The BNF entry for tirzepatide and the summary of product characteristics are both public.11

What the trial evidence does support is weight loss. In SURMOUNT-1, which included 2,539 adults, people on tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks. The comparison group, using lifestyle changes alone, lost 3.1%. These are trial averages and individual results vary.2 For someone starting at 100 kg, 20.9% is about 21 kg. That is a worked example of arithmetic, not a forecast of your own result.

08 · The first weeks

Is it normal to lose weight in the first week?

The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal. Early changes differ from person to person, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

The first pen is also the starting dose, 2.5mg. Mounjaro comes in six strengths from 2.5mg to 15mg, and your prescriber adjusts the dose over time, so how you feel at the start does not predict how you will feel later.1

It helps to measure progress by more than the scale. Appetite, portion sizes and how long a meal keeps you satisfied are useful early signs. The GLPTrackr companion app is free and helps you log your journey.

How does that compare with Wegovy?

Both medicines are licensed and effective, so the choice is about fit and tolerance. In the head-to-head SURMOUNT-5 trial, adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).15 In the separate STEP 1 trial, semaglutide 2.4mg users lost on average 14.9% over 68 weeks, against 2.4% on placebo.9 Only SURMOUNT-5 supports a direct comparison. None of these figures say how either medicine will make you feel day to day. That depends on your health profile, tolerance and prescriber advice.

09 · Who should not take it

Who should not take Mounjaro?

Mounjaro is only suitable after a prescriber has reviewed your health history. It is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber, and used alongside a reduced-calorie diet and increased physical activity.

Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Three points are worth raising in your consultation. Mounjaro should not be used during pregnancy, and it should be stopped at least one month before trying to conceive.

  • Any medicines you take, particularly insulin or sulfonylureas, whose doses may need adjusting because of the risk of low blood sugar.
  • Oral contraception. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows, so ask your prescriber what to do.7
  • Any existing condition, or a change in how you feel, since your last review.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

12 · NHS or private

NHS or private: how people get Mounjaro

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
