Appetite is the first thing to shift because the medicine acts on gut-hormone signals straight away. Tirzepatide activates the GIP and GLP-1 receptors together, which helps regulate appetite and slows how quickly the stomach empties.1
Feeling fuller sooner does not translate into a lower weight overnight. Body weight reflects weeks of eating patterns, fluid balance and activity, so the scale lags behind the change in hunger.

What to expect in the first week on Mounjaro
In the first week, expect anything from a clear drop in appetite to almost no change. The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in the first week, others feel little until a higher dose, and both are normal.1
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 Many people choose a weekend morning for their injection so early side effects fall on days off.1 There is no best time of day: Mounjaro works the same in the morning or evening, with or without food.
If you notice a new or worrying symptom, speak to your prescriber, and report suspected side effects through the Yellow Card scheme.19 Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 A quick early drop is not proof the medicine is working better than for someone whose number has not moved yet, and a flat first week is not proof it is failing.
06 · Weight loss on 2.5mg
Can you lose weight on 2.5mg of Mounjaro?
Some people do lose weight on 2.5mg, but that is not what the dose is for. Everyone starts Mounjaro on 2.5mg once a week, and this starting dose is about letting your body adjust, not about weight loss itself.1
After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Your prescriber guides the pace, which is why the full titration to the highest strengths takes months rather than weeks.

How the dose ladder shapes the speed of results
The dose ladder is the main reason results build gradually. Each step up happens only after a settling period, so you are on a lower strength for the early months of treatment.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
Those figures were measured at the end of the trial, not after a few weeks. A worked example shows the scale: for someone starting at 100 kg, 20.9% is about 21 kg, but over the full trial period rather than a few weeks.2

What to expect by the end of the first few months
By the end of the first few months, most people are still on the lower rungs of the dose ladder, so losses are modest compared with the headline figures. In SURMOUNT-1, average weight loss built month by month, with the full averages measured at 72 weeks, not at the three-month mark.2
A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.

09 · Plateaus and slow patches
Why progress can feel slow, and what to do
Progress can feel slow because weight loss on Mounjaro is not linear. Plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1
Within the licence, the sensible responses are to review food intake honestly, since portions drift back up, protect protein and activity, and check the basics such as sleep and alcohol. You can also discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.1 A plateau is not a reason to take extra doses or add unlicensed products.
If you are worried about a missed injection slowing you down, the rule is simple: take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

The first signs are usually behavioural rather than numerical: smaller portions feel satisfying, between-meal hunger quietens and food feels less insistent. These are the changes that make a reduced-calorie diet manageable, and they often show up before the scale does.
Clothes fitting differently can be an early clue, and in trials, weight loss came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.
11 · Review and next steps
When your prescriber reviews your response
Your prescriber reviews how you are responding, so you are never left guessing. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

How quick do other GLP-1 medicines work?
Other GLP-1 medicines also build gradually, because each follows its own stepped dose ladder. Wegovy (semaglutide), for example, steps through 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18
Because the trials differ, it is not fair to compare one medicine's early pace with another's.
