Dose matters because the licensed ladder is built to step up slowly. Everyone starts Mounjaro on 2.5mg once a week. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

The earliest sign is usually a change in appetite rather than the scales. Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Experiences differ widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose. Both are normal.1 Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

Other signs to look for are smaller portions feeling satisfying, fewer food cravings and clothes fitting differently. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.

07 · Speeding up and plateaus

Can you speed up weight loss on Mounjaro?

You can support results within the licence, but you cannot safely force them. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.

If progress stalls, the sensible responses are:

  • Review food intake honestly, because portions drift back up.
  • Protect protein and activity.
  • Check the basics, such as sleep and alcohol.
  • Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.

A plateau is not a reason to double doses or add unlicensed products.

Consistency also helps. Inject on the same day each week. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time; if more than 4 days have passed, skip it and take the next one on your normal day.1 Never take two doses within 3 days of each other.

What to eat, what to avoid and medicines to mention

Mounjaro is used alongside a reduced-calorie diet and increased physical activity, so what you eat still matters. Because appetite falls, smaller, regular meals built around protein tend to feel more manageable than large plates. Protecting protein and activity is part of the licensed-treatment advice for plateaus.

No food is banned on Mounjaro, but alcohol and sleep can undermine progress. If heavy or rich meals leave you feeling sick, that is worth raising with your prescriber, since nausea is among the commonest early effects.

On medicines, tell your prescriber and pharmacist about everything you take, including prescription, over-the-counter and herbal products, so interactions can be checked against the patient information leaflet.13 Do not add unlicensed weight-loss products on your own. If you have a new or unexpected symptom, speak to your prescriber. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

On timing, there is no "best" time of day to inject. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week, and many people choose a weekend morning so early side effects fall on days off. If you use the contraceptive pill, Mounjaro slows stomach emptying, which can reduce how well it is absorbed. Switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.

Mounjaro is available on the NHS, but access is being phased in over several years.6 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began with the highest-need groups first.4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
