What a monthly dose of Mounjaro actually is

A monthly dose of Mounjaro is a pen, and the pen is the unit you buy. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

This is where the phrase causes confusion. You inject once a week, and the pen holds the doses for those weeks. Each KwikPen contains four weekly doses.13 So when a pharmacy quotes a monthly price, it is quoting the cost of a full pen at a particular strength.

The strength printed on the pen is the dose of each weekly injection, not the total in the pen. You do not need to do any sums yourself: the pen is designed so that you dial the dose shown for your prescribed strength each week.

Never try to squeeze extra injections out of a pen. Doing so would mean unknown dosing accuracy and no safety data. If you feel a pen is not lasting, or the pen seems faulty, contact the pharmacy rather than improvising. Dispensed medicines cannot normally be returned, except if faulty, damaged or the wrong item, and you should contact us within the stated window if that happens.

Understanding the pen also helps you compare quotes honestly. A provider that advertises a low weekly figure is still asking you to buy pens, so convert any weekly price into the pen price before you compare.

Eli Lilly set a higher UK list price from September 2025, which changed what all providers pay and reshaped the market. Individual pharmacies then set their own sell prices, partly because of confidential discounts. That is why you will see very different quotes for the same strength across providers. Rather than guess, we track a set of UK pharmacies ourselves. What should you actually compare? First, compare the same strength, because a starter pen and a top-strength pen are not like for like. Second, compare what is included: consultation, prescriber review, delivery, injection supplies and follow-up. Third, check that the provider supplies genuine pens from an official source. Planning the first four pens and the first year

If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A new customer's first order is £40 less, and every later order is £25 less.

Not everyone follows that exact path. Your prescriber may hold you at a strength for longer, or you may settle sooner. The dose you take is decided with your prescriber.

For a full year, the maths depends on where you settle. A first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. Those totals are at standard prices, again before the new-customer and reorder discounts. If you settle at 7.5mg, 10mg or 12.5mg, work from that strength's monthly price: £279.99, £290.99 or £319.99.

Notice that a higher strength is not automatically the goal. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.11 Choosing a strength by price alone would be a mistake in either direction. The right strength is the one your prescriber judges gives you the best balance of response and tolerability.

It is also worth thinking about what happens when you stop. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system, and nothing needs doing while it clears.1 There is no subscription with us, so you only buy a pen when you decide to reorder, and repeat orders re-check suitability each time.

07 · The dose ladder

How your monthly dose changes up the ladder

Your monthly dose changes only when your prescriber steps you up, and the ladder is set by the licence. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

In practice that means your monthly pen changes strength roughly as you move up, but only when you and your prescriber agree. The word minimum matters: four weeks is the earliest a step can happen, not a target. Some people stay longer at a strength because it is working and tolerable, and that is entirely within the licence.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 Think of the ladder as a menu of options rather than a staircase you must climb to the top. There is also a review point built into treatment. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4 That is a conversation, not an automatic cut-off.

The same goes for plateaus. A plateau is not a reason to double doses or add unlicensed products.1 Within the licence, the responses are to review food intake honestly, protect protein and activity, check the basics such as sleep and alcohol, and discuss with your prescriber whether a step-up is appropriate if you are not yet at maximum.

08 · Taking each weekly dose

Taking each weekly dose from the pen

Each weekly dose is an injection under the skin from the same pen. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.13 Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.

There is no perfect time of day. There is no "best" time of day — Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember (many people choose a weekend morning so early side effects fall on days off) and stick to it.13

Because a pen is a monthly purchase, a consistent routine also keeps your reordering tidy. Reorder before the pen runs out so there is no gap, and remember that each repeat order re-checks suitability, so allow time for that review.

If you miss a dose

Missing a dose is common, and the rule is straightforward. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.13 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.13

Do not try to catch up by taking extra, and do not treat a skipped dose as a way to recover pen contents. If you are unsure, ask the prescribing team before injecting.

If you need to change your injection day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.13 That is useful if your routine shifts, for example because of travel or work.

Early on, expect some adjustment. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 If something new or worrying appears, speak to your prescriber, and you can report any suspected side effect through the Yellow Card scheme.19 Severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing: seek immediate medical help.

09 · Storage and travel

Storing your monthly pen and travelling with it

Store your pen in the fridge until you start it, then follow the in-use rule. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13

That in-use window is why a pen suits a monthly rhythm. Because the pen holds four weekly doses and may be kept out of the fridge for up to 30 days (not above 30°C) once started, most people can carry the pen they are using without special arrangements. Discard it after that period even if medicine appears to remain, because sterility and potency are no longer assured.

Unopened pens are different. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.13 Our pens arrive in temperature-controlled packaging, so move them to the fridge when they arrive.

Flying and holidays

You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription.13 In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.

Protect each pen the way you would protect any other temperature-sensitive medicine.

If you are travelling for a long time, speak to your prescriber before you go so reorders, strengths and timing can be planned around your trip. Do not stockpile pens or change your schedule to fit a flight; the licensed weekly interval stays the same wherever you are.

10 · NHS versus private

NHS versus private: what you pay and why

Mounjaro is available on the NHS, but access is being phased in over several years.6 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6

For people who are prescribed it through the NHS in England, the usual prescription charge of £9.90 per item may apply, depending on exemptions. The NHS route has its own eligibility and waiting arrangements, so it is worth asking your GP what applies to you locally.

The private route works differently. You pay the pen price directly, and there is no queue for a place, but you still need a clinical assessment. 11 · What to expect from results

What to expect from each month of treatment

Results build gradually, so each monthly dose should be judged as part of a trend. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.2

For the headline numbers, here is the trial wording. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result.

Early on, experiences differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.2 That matters for spending decisions too: slower early progress on the starter pen does not predict failure, because the dose you respond to may still be ahead of you.

Plateaus are normal as well. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight. If one happens, review food intake honestly, protect protein and activity, check sleep and alcohol, and discuss with your prescriber whether a step-up is appropriate if you are not yet at maximum.

In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises — but they are why prescribers talk about weight management as health management rather than appearance.
