What the SURMOUNT-1 trial shows for 5mg
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.
It says that on the lowest maintenance-level strength, the average participant lost a substantial share of their body weight, far more than the comparison group managed with lifestyle changes alone. It does not say that everyone on 5mg loses that amount, and it does not say the loss arrives quickly. The measurement point was 72 weeks, which is well over a year of treatment.
The trial also reports how many people crossed a basic threshold. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 That tells you the large majority of people on 5mg achieved a meaningful loss, and that moving to higher strengths raised the proportion only modestly at that threshold. The difference between strengths shows up more clearly at the top end of results: half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
Why does this matter on 5mg? Because it shows the dose can be a sensible place to stay. If you reach a loss you and your prescriber are happy with, there is no clinical rule that you must keep climbing the ladder. Equally, if you are tolerating treatment and progress has stalled, a higher strength is an option to discuss rather than something to assume.
Treat the numbers as a range of possibilities, not a forecast. Trial participants were people taking part in a structured study, with regular follow-up. Averages hide a spread: some people lose more than the headline figure, some less, and the trial average cannot tell you which group you will be in.
One more point of honesty. A trial average at 72 weeks should not be divided into a weekly rate. Loss is front-loaded for some people, slower for others, and interrupted by plateaus. Be cautious of pages that quote a fixed per-week number on 5mg.
Finally, remember that the comparison group in the trial also received diet and exercise support, and still lost far less than the treatment groups. That is the clearest sign that the medicine is doing real work, while also confirming that diet and activity remain part of the plan rather than optional extras.
05 · Where 5mg sits
Where 5mg sits on the dose ladder
5mg is the second rung of the Mounjaro ladder and the first strength reached after the starter phase. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

But there is no obligation to step up at the earliest point. Many people settle at a maintenance dose of 5mg, 10mg or 15mg.1
The way you inject does not change with strength. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 Licensed sites are the stomach area, the front of the thigh, or the back of the upper arm if someone else gives the injection, and you should rotate the site with each dose.13
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.1 You can also change your weekly injection day as long as at least 3 days (72 hours) have passed since your last dose.1

Weight loss on Mounjaro is gradual by design, and 5mg is where most people start to see steady progress. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.
The early experience differs widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. By the time you have been on treatment for a while, expectations should be calibrated. Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures: in SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.2 A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.
That framing helps people on 5mg who feel they are behind. If you have only recently moved up from 2.5mg, you are probably earlier in the curve than the headline numbers suggest. 08 · Plateaus and speeding up
Plateaus, and how to speed up weight loss on 5mg
If your weight stops moving for a while on 5mg, that is normal and expected. Weight loss on Mounjaro is not linear: plateaus of a few weeks are normal as the body adapts to a lower weight.2 A plateau is a pause in a trend, not proof that the medicine has stopped working.
People often ask how to speed things up. Within the licence, the responses are practical and unglamorous. First, review food intake honestly, because portions drift back up over time. Second, protect protein and activity, since treatment works alongside a reduced-calorie diet and increased physical activity rather than replacing them. Third, check the basics such as sleep and alcohol, both of which affect appetite and weight. Fourth, discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.2
What a plateau is not is a reason to double doses or add unlicensed products. Altering doses outside the licence means unknown dosing accuracy and no safety data.
It is also worth separating a real plateau from noise. A week or two of flat readings after a good run is common. Fluid shifts, a change in routine, a holiday or a short illness can all mask underlying progress. Look at the direction over several weeks before deciding anything has changed.
Equally, consider timing. There is no best time of day to inject, because Mounjaro works the same morning or evening, with or without food.13 What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off, but that is a convenience, not a way to boost results.

09 · Beyond the scales
Beyond the scales: other signs it is working
The scale is only one signal, and on 5mg several others often move first. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.
So what are the first signs of losing weight on treatment? In practice they tend to be appetite related before they are visible. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Food noise quietens for some, portions shrink without much effort, and clothes may fit differently before the number on the scale shifts much. Waist measurements can move even on weeks when the scale does not.
The mechanism explains this. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 That dual action distinguishes it from single-action GLP-1 medicines such as semaglutide. You will not feel it as a sudden event; it is a steady change in how hungry you feel and how quickly you feel full.
A sensible set of non-scale markers to watch includes:
- How hungry you feel between meals and how quickly you feel full.
- Waist measurement and how clothes fit.
- Energy, sleep and everyday movement, such as stairs or walking.
- Blood pressure, cholesterol and blood-sugar readings if your clinician checks them.
It is also important to report new or worrying symptoms. If something does not feel right on 5mg, tell your prescriber and consider reporting a suspected side effect through the Yellow Card scheme.19 Feeling well matters as much as the number you see, and a regimen you can sustain will almost always outperform one you cannot.
The takeaway is that progress is multi-dimensional. If the scale is slow but appetite is better, your waist is smaller and you feel healthier, the treatment may be working in ways a single weigh-in cannot capture.
10 · How long to stay on 5mg
How long should you stay on 5mg?
There is no single correct length of time on 5mg; it depends on your response, your tolerance and your prescriber's review. The dose can step up after at least 4 weeks, but that is a minimum, not a target.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg, so for some, 5mg is simply where treatment stays.
Here are reasons to discuss a step-up. Progress has plateaued, you are not yet at maximum, and you are keeping to diet and activity. Your prescriber can assess whether increasing is appropriate, and whether that is better than staying put.2
A formal checkpoint also exists. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.2
What about stopping altogether? Talk to your prescriber before stopping, because the decision involves how you will maintain your weight afterwards. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.2 Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system; there is no way to speed this up, and nothing needs doing while it clears.1
Practical points on supplies also matter if you stay on 5mg for months. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.13 Repeat orders re-check suitability, and there is no auto-shipping, so you stay in control of continuing, pausing or changing dose.
11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
12 · NHS and private access
Getting 5mg: NHS and private routes
You can reach 5mg through either the NHS or a regulated private pharmacy, and the route does not change how the medicine works. Mounjaro is available on the NHS, but access is being phased in over several years.5 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
