01 · The core answer

The short answer: there is no unit count to follow

The question assumes you are drawing medicine into a syringe, which is not how Mounjaro is given. Mounjaro is a once-weekly injectable made by Eli Lilly and supplied as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 The pen does the measuring for you.

You turn the dose knob until the dose indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks.13 That window reading is the only check that matters. Counting clicks is not a reliable way to measure a dose, so rely on the window reading.

02 · Where the confusion comes from

Why you keep seeing unit figures online

Unit figures belong to insulin-style syringes, which are marked in units and used with vials. Some websites, social posts and even automated search summaries apply that logic to Mounjaro and quote a single syringe number for every dose. That is a conversion exercise, not how the KwikPen is designed to be used.

There is a second problem. Even where a volume figure is quoted, it tells you nothing useful about your dose, because your dose is a strength in milligrams, set on the pen. A summary that says the injection "is always the same amount of liquid" and then tells you to read it off a syringe is mixing two different products. Follow your Patient Information Leaflet and your prescriber's instructions instead.13

03 · Your next step

What to do if you were told a unit number

If a friend, forum post or app has given you a unit figure, do not act on it. Check the dose window on your pen against the strength on your prescription label. If they do not match, contact your pharmacy or prescriber before injecting.

04 · Reading the pen dial

How the KwikPen dial works

The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.13 That is the whole process. You are not converting between units, millilitres and milligrams. The number you see is the number you inject.

Each KwikPen contains four weekly doses.1 The pen will not let you dial past the medicine left inside. If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.13

This is a useful safety feature. If you reach the end of the pen and the dial stops short, that pen is finished. Start your next pen rather than trying to top up from what remains.

05 · Strength versus amount

Strength is what changes, not what you measure

Mounjaro strengths run from 2.5mg to 15mg, and each strength is its own pen. You never need to work out a different injection volume for yourself, because each strength is its own pen.

The dose ladder is set by your prescriber. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.11

06 · Why not to improvise

Why you should not improvise doses

"Microdosing" means taking smaller or more spread-out doses than the pen is designed to deliver. It is not licensed, not supported by the pen's design and not endorsed by any UK regulator.1

The licensed dose ladder already starts low (2.5mg) for tolerability, and your prescriber can hold a dose longer or step down within the licence if side effects are the concern.1 Altering doses outside the licence means unknown dosing accuracy and no safety data.

07 · Injecting step by step

Injection basics once the dose is set

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

08 · Missed doses and storage

Missed doses and storing your pen

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1 Dial the usual dose and check the window as normal; there is no unit adjustment for a late injection.

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, useful for short trips, and must then be thrown away. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.13 If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.

09 · Travel and everyday handling

Travelling with your pen

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly.13

Because the dose is fixed by the dial, you do not need to carry syringes, vials or a conversion chart. The pen is the whole system, which makes travel simpler than people expect.

10 · What to expect from treatment

What to expect once you are on the right dose

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.2

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4

Mounjaro is used alongside a reduced-calorie diet and increased physical activity. Individual results vary.

The simplest safe routine is to check the strength on the label, set the dial, confirm the window and inject. If anything about that sequence does not match, stop and ask.
