A 5mg dose of Mounjaro is set by turning the dose knob until the window shows 5mg. That is the whole instruction.1 Mounjaro in the UK is a pre-filled multi-dose pen called a KwikPen, available in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

The same logic covers 7.5mg. You are not working out a volume, loading a syringe or converting anything. The pen does the measuring, and the number in the window is the only thing you confirm.13

The product information describes the dose in mg and tells you to read the window. A figure copied from somewhere else could belong to a different product. If you want the exact volume the pen delivers, the patient information leaflet in your box is the authoritative place to look.

03 · How the KwikPen works

How the KwikPen sets your dose

The Mounjaro KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks.1

Each pen contains four weekly doses. The pen will not let you dial past the medicine left inside.1 That built-in limit is a safety feature: you cannot accidentally dial more than remains.

If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.1 Do not try to top up from another pen. Contact the pharmacy that supplied it and they can advise.

This design is why people who ask for a ml figure rarely need one. A syringe-and-vial product asks you to measure. A pre-filled pen asks you to read a number and press the button as the instructions describe.

04 · Why online figures conflict

Why you see different figures online

The usual reason is that they mix products. Mounjaro has been sold in different presentations in different countries, including single-dose pens and vials, and a concentration quoted for one is not valid for another.

The UK product is a pre-filled multi-dose KwikPen. That is the product we dispense, genuine KwikPens supplied through official LillyDirect supply from Eli Lilly's UK distribution. A volume taken from a different presentation could mislead you.

The same caution applies to forum calculations. If a figure differs from your leaflet, trust the leaflet.13

05 · 5mg versus 7.5mg

5mg versus 7.5mg: what changes

What changes between 5mg and 7.5mg is the strength of the pen and the number you dial, not a measuring step you perform. Everyone starts Mounjaro on 2.5mg once a week. After at least four weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of four-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1 So moving from 5mg to 7.5mg is a prescriber decision, taken when you have been on 5mg for the minimum period and are tolerating it.

In the SURMOUNT-1 trial of adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2 Your results depend on your own response, not on any volume.

06 · Injecting your dose

Injecting your dose: what the instructions say

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 Licensed sites are the stomach area, at least 5cm from the belly button, the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

Follow the pen's instructions for the injection itself, including how long to keep it in place.

There is no best time of day. Mounjaro works the same morning or evening. What matters is the same day each week, so pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off.1

07 · Missed doses and timing

Missed doses and changing your day

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

This matters for volume questions because some people think they can make up a missed dose by injecting extra from the pen. You cannot. The pen delivers a set dose, and the timing rules above are the only licensed way to catch up.

08 · Storage and travel

Storing and travelling with your pen

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. For hot climates use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly.1

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.13

09 · Dose changes and safety

Why you should not alter the dose

Taking smaller or more spread-out doses than the pen is designed to deliver, sometimes called microdosing, is not licensed, not supported by the pen's design and not endorsed by any UK regulator.1 People who ask about ml per dose are sometimes trying to split a dose. That is the situation this rule exists for.

The licensed dose ladder already starts low at 2.5mg for tolerability. Your prescriber can hold a dose longer or step down within the licence if side effects are the concern. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
